Electrocautery has all but disappeared from the clinical dental setting. Through technical advances, there have been developed new instruments which must now challenge the profession to re-examine the use of electrocautery as a valuable adjunct, particularly in the field of orthodontics. This article will describe a simple yet effective battery-operated electrocautery unit. Indications and examples of its use will also be presented.

An esthetic smile plays a pivotal role in enhancing patient confidence and social well-being. Smile esthetics are influenced by multiple factors, including maxillary anterior tooth morphology, lip length and mobility, periodontal health, and the degree of gingival display. Short clinical crowns associated with excessive gingival display are generally perceived as unesthetic and frequently require esthetic crown lengthening. Altered active eruption (AAE) and altered passive eruption (APE) represent common etiologic factors in such presentations. Management typically involves gingivectomy with o

A renewed interest in conservative surgical techniques has been fueled by new technology, changes in referral patterns to periodontists and a desire to achieve periodontal health in the least invasive, most cost-efficient manner possible. Trends suggest that an increasing amount of periodontal care is being provided in the offices of general dentists. If true, it is likely that patients receiving care in these offices will be offered simpler surgical treatment modalities that do not require an extensive armamentarium. The purpose of this article was to review the effectiveness of six relativel

Mucopolysaccharidosis type VI (MPS VI), a rare metabolic disorder, results from glycosaminoglycan accumulation in tissues, frequently leading to hyperplastic gingival alterations that necessitate comprehensive intervention. An 11-year-old male patient diagnosed with MPS VI required hospitalization due to complications associated with a ventriculoperitoneal shunt (VPS) valve. A dental evaluation identified bone malformation in the maxillomandibular complex accompanied by pronounced gingival hyperplasia. The surgical team performed a gingivoplasty in the operating room under general anesthesia,

Various pieces of information, including the age of the patient, the site involved, symptoms, intraoral findings, and X-ray findings are necessary in making a clinical diagnosis. This report describes a case in which the initial diagnosis was eruption sequestrum based on clinical information but which was later found to be a complex odontoma based on histopathological findings. The patient was an 8-year-old girl who presented at our hospital with the chief complaint of pain in the right mandibular molar region. The distal aspect of the right mandibular first molar, which had partially erupted,

One of the most frequent causes of gummy smile is an altered eruption, ie, the gingival margins are located at a more coronal position than normal. These cases can be successfully treated with a periodontal surgical approach. Several techniques may be used, depending on the different anatomic variations. The surgery must be carefully planned so as to avoid discrepancies between the soft tissues and the teeth. Clinical examination and radiographic evaluation will allow clinicians to determine the position of the gingival margin, bone crest, and cemento-enamel junction and thus elicit a correct

Gingival enlargements may adversely affect speech, mastication, tooth eruption, and esthetics. These enlargements can occur as a result of the administration of certain anticonvulsants, immunosuppressants, and calcium channel blockers. The present case report describes the treatment of a patient with a phenytoin-induced gingival enlargement. A case of gingival enlargement should be treated in a step-wise manner, including consultation with the patient's physician, substitution of the drug, nonsurgical therapy, surgical therapy (if needed), and supportive periodontal therapy after every 3 month

A common cause for the condition described as excessive gingival display is the phenomenon known as altered passive eruption, a coronally situated gingival complex that failed to recede normally during eruption of an otherwise healthy and normal dentition. Orthodontic movement in such conditions may set the stage for additional coronal growth of hyperplastic gingival complex. The objective of this article is to describe, through a detailed case presentation, the surgical methods used to achieve healthy and aesthetic gingival contours in such conditions.

BACKGROUND/AIMS: Hereditary gingival fibromatosis is characterized by various degrees of attached gingival overgrowth. It usually develops as an isolated disorder but can be one feature of a syndrome. A case of a 38-year-old female is reported who presented a generalized severe gingival overgrowth, involving the maxillary and mandibular arches and covering almost all teeth. The clinical differential diagnosis included drug-induced overgrowth as well as idiopathic gingival fibromatosis.
TREATMENT: Excess gingival tissue was removed by conventional gingivectomy. As the gingival enlargement was

Finding a solution to the short clinical crown is based on an appreciation of the biological width of attachment plus the dimensional requirements of sound restorative procedures. The collective talents of restorative dentist, orthodontist, and periodontist often are needed to cope with the problem.

This case describes the development of subcutaneous emphysema following restorative dentistry performed under general anesthesia. Initial treatment consisted of intravenous epinephrine and dexamethasone due to difficulty in breathing and laryngeal stridor. Dexamethasone and other adjunctive drugs were administered over the 4 days following surgery while the symptoms subsided. The author emphasizes the importance of early recognition and prompt management in managing this unusual complication.

Cherubism is a congenital childhood disease of autosomal dominant inheritance. It is a benign, familial giant cell lesion characterized by gradually progressive painless swelling of the jaws. Idiopathic gingival enlargement is a rare condition and may be associated with some uncommon syndromes. This case report describes an 11-year-old patient with unusual clinical form of gingival enlargement, cherubic facial appearance. Clinical examination revealed the presence of the hyperplastic gingiva, which completely covered all teeth. The bilateral swelling of mandible and the appearance of the scler

Gingival melanoacanthoma is a rare, benign pigmented lesion characterized clinically by sudden onset and rapid growth of a macular brown black lesion and histologically by acanthosis of superficial epithelium and proliferation of dendritic melanocytes. This article reports a previously undescribed case of pigmented unilateral diffuse gingival enlargement, which on histopathological examination proved to be melanoacanthoma. Intraoral examination revealed pigmented unilateral diffuse gingival enlargement in relation to second and third quadrants buccally, palatally/lingually. Based on these clin

Hereditary gingival fibromatosis (HGF) and amelogenesis imperfecta (AI) are two rare oral conditions with genetic etiologies. The case of a 17-year-old boy affected by HGF, AI, anterior open bite, and pyramidal impaction of the maxillary molars is reported. Internal bevel gingivectomies were carried out to reduce gingival overgrowth. Clinical examination of the family revealed the presence of HGF and AI in his 12-year-old sister (both in milder forms) and of HGF in his older half brother. Genetic sequencing analyses were performed to detect any of the known mutations leading to HGF and AI. His

Crohn's disease is a granulomatous inflammatory bowel disease and was described in 1932 as a chronic granulomatous disorder of the terminal ileum and is now considered a distinct member of the inflammatory bowel disease family. It may affect any part of the gastrointestinal tract. Oral Crohn's disease has been reported frequently in the last three decades with or without intestinal manifestations. In the latter case, it is considered as one of the orofacial granulomatosis. There has been much doubt whether intestinal manifestations of Crohn's disease will eventually develop in the orofacial gr

INTRODUCTION: Smile aesthetics is based on numerical, physical, physiological and psychological data regarding beauty, while taking into account the desires of the patient. It is determined by the shape, colour and position of the lips, teeth and gingival tissues. Periodontal examination in both the facial and labial settings supports analysis of the gingival display during natural and forced smiling, the health of the periodontium, the gingival contours, the aesthetic gingival line, and the presence of the papillae.
MATERIALS AND METHODS: All these data will help establish a gingival aesthet

BACKGROUND: Pigmentations of the oral cavity are commonly noted in clinical practice and may have physiologic, non-physiologic, or pathologic etiologies. The most common non-physiologic localized pigmentation is the amalgam tattoo; another, less common, non-physiologic pigmentation is the graphite tattoo. Graphite tattoos may easily be confused with amalgam tattoos but have only infrequently been reported in the literature.
METHODS: This case report discusses a patient with a localized pigmented lesion involving the free gingiva, attached gingiva, and alveolar mucosa. The patient was referred
