Transcranial magnetic stimulation mitigates perioperative neurocognitive disorders by regulating the function of the glymphatic system.

The incidence of perioperative neurocognitive disorders (PND) increase with age, especially within those countries facing great challenge of aging population. However, the mechanism of PND remains elusive, and the lack of precautions has resulted in extended recovery among the elderly. Transcranial magnetic stimulation (TMS) has shown promising therapeutic potential in many neurological disorders such as depression and Alzheimer’s disease. This study aimed to explore the therapeutic potential of TMS on PND mouse model and aged patients. PND mice model were established through exploratory lapar

Glymphatic-meningeal lymphatic system imbalance: a peripheral-to-central inflammatory bridge in perioperative neurocognitive disorders.

Perioperative neurocognitive disorders (PNDs) are common postoperative complications, particularly in elderly patients. While surgical trauma is known to trigger systemic inflammation, the mechanisms linking peripheral immune activation to perioperative neurocognitive dysfunction remain not fully elucidated. The glymphatic-meningeal lymphatic system is crucial for maintaining homeostasis because it facilitates the exchange of cerebrospinal fluid and interstitial fluid, clears metabolic waste, and eliminates immune mediators. Recent studies have indicated that dysfunction of this clearance axis

Therapeutic approaches to CNS diseases via the meningeal lymphatic and glymphatic system: prospects and challenges.

The brain has traditionally been considered an "immune-privileged" organ lacking a lymphatic system. However, recent studies have challenged this view by identifying the presence of the glymphatic system and meningeal lymphatic vessels (MLVs). These discoveries offer new opportunities for waste clearance and treatment of central nervous system (CNS) diseases. Various strategies have been developed based on these pathways, including modulation of glymphatic system function, enhancement of meningeal lymphatic drainage, and utilization of these routes for drug delivery. Consequently, this review

Loss of aquaporin-4 results in glymphatic system dysfunction via brain-wide interstitial fluid stagnation.

The glymphatic system is a fluid transport network of cerebrospinal fluid (CSF) entering the brain along arterial perivascular spaces, exchanging with interstitial fluid (ISF), ultimately establishing directional clearance of interstitial solutes. CSF transport is facilitated by the expression of aquaporin-4 (AQP4) water channels on the perivascular endfeet of astrocytes. Mice with genetic deletion of AQP4 (AQP4 KO) exhibit abnormalities in the brain structure and molecular water transport. Yet, no studies have systematically examined how these abnormalities in structure and water transport co

Cerebrospinal Fluid Level of Aquaporin4: A New Window on Glymphatic System Involvement in Neurodegenerative Disease?

Aquaporin4 (AQP4) is a water channel protein located at astrocyte foot processes that plays a role in glymphatic system, a highly organized fluid transport pathway which seems to be involved in Alzheimer's disease (AD) and normal pressure hydrocephalus (NPH) pathophysiology. Cerebrospinal fluid (CSF) AQP4 levels were determined in 11 patients with AD, 10 patients with NPH, and 9 controls. We found significantly reduced AQP4 in AD patients, a trend in reduction in NPH patients, and a correlation between AQP4 and amyloid-β CSF levels. This study indicates the potential role of AQP4 and glymphati

Impaired glymphatic system is associated with secondary neuronal injury in the thalamus following cerebral cortical infarction.

Focal cerebral infarction leads to abnormal amyloid-β (Aβ) deposits, which relates to secondary neuronal injury in the ipsilateral thalamus and hinders post-stroke functional recovery. However, the mechanisms underlying Aβ-pathology in the thalamus remain unclear. This study was designed to investigate the potential associations of glymphatic system with Aβ deposits and delayed neuronal damage of the ipsilateral thalamus secondary to cerebral infarction. Cortical infarction was induced with middle cerebral artery occlusion (MCAO). Secondary neuronal damage, Aβ deposits and aquaporin 4 (AQP4) p

The glymphatic system in neurodegenerative diseases and brain tumors: mechanistic insights, biomarker advances, and therapeutic opportunities.

Dysfunction of the glymphatic system (GS), a brain-wide waste clearance pathway dependent on polarized aquaporin-4 (AQP4) water channels on astrocytic endfeet, is increasingly recognized as a critical mechanism in both neurodegenerative diseases and brain tumors. In Alzheimer's (AD) and Parkinson's (PD) diseases, impaired glymphatic function leads to the accumulation of neurotoxic proteins, including amyloid-β (Aβ), tau, and α-synuclein (α-syn). Contributing factors include loss of AQP4 polarization, reduced arterial pulsatility, genetic risks (e.g., APOE4, FAM171A2 mutations), and sleep distu

Subcortical nuclei and glymphatic system alterations in migraine with aura: a comparative 7T multimodal MRI analysis.

BACKGROUND: Although cortical spreading depression is well-established in migraine with aura (MA), the potential involvement of subcortical nuclei and the glymphatic system (GS) in its pathogenesis—and their utility in distinguishing MA from migraine without aura (MO)—remains poorly investigated. Therefore, we employed multimodal 7T MRI—integrating functional, structural, quantitative susceptibility mapping (QSM), and GS metrics (perivascular space (PVS) morphology, choroid plexus volume, and analysis along the perivascular space (ALPS) index)—to enhance the understanding of MA and to characte

Fingolimod improves diffuse brain injury by promoting AQP4 polarization and functional recovery of the glymphatic system.

BACKGROUND: Diffuse brain injury (DBI) models are characterized by intense global brain inflammation and edema, which characterize the most severe form of TBI. In a previous experiment, we found that fingolimod promoted recovery after controlled cortical impact injury (CCI) by modulating inflammation around brain lesions. However, it remains unclear whether fingolimod can also attenuate DBI because of its different injury mechanisms. Furthermore, whether fingolimod has additional underlying effects on repairing DBI is unknown. METHODS: The impact acceleration model of DBI was established in a
