An adjustable gravitational valve for initial VP-shunt treatment in hydrocephalic preterm neonates and infants below 1 year of age.

OBJECTIVE: Shunt treatment for hydrocephalus in children should aim for sustainable flexibility in regard to optional, perspective pressure level adjustment during advancing physical and mental development. Gravitation-assisted shunt valves are designed to prevent hydrostatic over-drainage frequently observed in the long course of shunt-treated hydrocephalus. We prospectively studied and analyzed the implication, safety, and feasibility for an adjustable gravitational unit combined with a fixed differential-pressure (DP) valve for neonates and infants primary shunted within the first 12 months

[Contractile characteristics of single skinned rat soleus muscle fibers under gravitational load: effects of a calcium-binding agent].

Excessive intracellular calcium accumulation is believed to trigger the development of functional and structural changes in muscle fibers under microgravity conditions. The hypothesis was testified in the 14-day hindlimb suspension study with the application of a Ca(2+)-binding agent (10% EGTA). Twenty one rats were divided into 3 groups: cage controls (7), hindlimb-suspended rats that received intraperitoneal injections of saline (7), and hindlimb-suspended rats with EGTA treatment. Whereas the diameter of muscle fibers of unloaded rat soleus muscle was 20% less than in the control group (and

Acute Gravitational Stress Selectively Impairs Dynamic Cerebrovascular Reactivity in the Anterior Circulation Independent of Changes to the Central Respiratory Chemoreflex.

Cerebrovascular reactivity (CVR) to changes in the partial pressure of arterial carbon dioxide (PaCO2) is an important mechanism that maintains CO2 or pH homeostasis in the brain. To what extent this is influenced by gravitational stress and corresponding implications for the regulation of cerebral blood flow (CBF) remain unclear. The present study examined the onset responses of pulmonary ventilation (V̇E) and anterior middle (MCA) and posterior (PCA) cerebral artery mean blood velocity (Vmean) responses to acute hypercapnia (5% CO2) to infer dynamic changes in the central respiratory chemore

Is self-image, in reference to the gravitational vertical, altered in adolescent idiopathic scoliosis? A multicenter, single-blind, case-control study.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is associated with perturbation of orthostatic postural control. In humans, orthostatic postural control is organized around the gravitational vertical (GV). Perception of the GV involves a bottom-up process (central integration of multisensorial information) and a top-down process that uses cognitive mechanisms relying on body image. This study hypothesized that AIS would be associated with an erroneous central representation of verticality. The objective was to demonstrate an altered top-down process of sense of verticality in AIS. METHODS: Thi

Intrinsic functional connectivity reduces after first-time exposure to short-term gravitational alterations induced by parabolic flight.

Spaceflight severely impacts the human body. However, little is known about how gravity and gravitational alterations affect the human brain. Here, we aimed at measuring the effects of acute exposure to gravity transitions. We exposed 28 naïve participants to repetitive alterations between normal, hyper- and microgravity induced by a parabolic flight (PF) and measured functional MRI connectivity changes. Scans were acquired before and after the PF. To mitigate motion sickness, PF participants received scopolamine prior to PF. To account for the scopolamine effects, 12 non-PF controls were scan

Postmortem computed tomography assessment of intracardiac and intravascular blood clots and gravitational sedimentation: clinical and laboratory associations in 114 in-hospital deaths.

PURPOSE: Postmortem computed tomography (PMCT) typically reveals blood clots and sedimentation in cardiac and vascular structures. We examined the associations between these postmortem findings and antemortem clinical and laboratory parameters in in-hospital death. MATERIAL AND METHODS: This prospective study included 114 non-traumatic in-hospital deaths where PMCT was performed within 24 h postmortem. Two radiologists evaluated high-density areas in the right and left atria, pulmonary artery, and thoracic aorta, and classified them as blood clots or gravitational sedimentation. The clinical

Continuous Low-Thrust Maneuver Planning for Space Gravitational Wave Formation Reconfiguration Based on Improved Particle Swarm Optimization Algorithm.

This study proposes a three-spacecraft formation reconfiguration strategy of minimum fuel for space gravitational wave detection missions in the high Earth orbit (105 km). For solving the limitations of measurement and communication in long baseline formations, a control strategy of a virtual formation is applied. The virtual reference spacecraft provides a desired relative state between the satellites, which is then used to control the motion of the physical spacecraft to maintain the desired formation. A linear dynamics model based on relative orbit elements' parameterization is used to desc

In vivo evaluation of the effects of gravitational force (+Gz) on over-the-wire stainless steel Greenfield inferior vena cava filter in swine.

This study was done to determine the effect of exposure to gravitational force (acceleration stress) on in vivo over-the-wire stainless steel Greenfield inferior vena cava filters. Fifteen pigs underwent venous cut down and placement of a stainless steel Greenfield filter. A 4-week observation period simulated realistic convalescence and allowed sufficient time for epithelialization. Ten pigs were exposed to acceleration stress in a centrifuge (3G run for 15 sec followed by rest until return to baseline heart rate, then a 9G run for 15 sec), with inertial loading in a head-to-tail direction (+

Combining a gravitational search algorithm, particle swarm optimization, and fuzzy rules to improve the classification performance of a feed-forward neural network.

BACKGROUND AND OBJECTIVE: A feed-forward neural network (FNN) is a type of artificial neural network that has been widely used in medical diagnosis, data mining, stock market analysis, and other fields. Many studies have used FNN to develop medical decision-making systems to assist doctors in clinical diagnosis. The aim of the learning process in FNN is to find the best combination of connection weights and biases to achieve the minimum error. However, in many cases, FNNs converge to the local optimum but not the global optimum. Using open disease datasets, the purpose of this study was to opt

Effects of gravitational stresses on mitral valve prolapse. I. Changes in auscultatory findings produced by progressive passive head-up tilt.

The effects of passive head-up tilt on systolic time intervals were assessed in 18 patients with mitral valve prolapse. In addition to causing prolongation of the pre-ejection period and shortening of left ventricular ejection time, this circulatory stress led to progressive shortening of the Q to click interval. In 1 patient, a systolic click became audible which had not been heard in the supine posture. In 7 patients the click disappeared during head-up tilt, usually at 60 degrees or 90 degrees. In 2 patients without a murmur while supine, a mid-late systolic murmur appeared with tilt; 1 of

Changes of head circumference and ventricular width in infant hydrocephalus managed with adjustable shunt valves and gravitational assistance-a comparative bi-center study.

OBJECTIVE: Cerebrospinal fluid (CSF) drainage via a ventriculoperitoneal shunt remains to be the most common treatment for infantile hydrocephalus. Since head growth is particularly dynamic and susceptible to intracranial pressure changes during the first months of life, shunting can have a considerable impact. This study aimed to examine changes in head circumference and ventricular width and evaluate their interdependence in children who underwent shunting during infancy with either of two different adjustable valve regimens. METHODS: We conducted a retrospective analysis on synchronized pa

Safety and efficacy of gravitational shunt valves in patients with idiopathic normal pressure hydrocephalus: a pragmatic, randomised, open label, multicentre trial (SVASONA).

OBJECTIVES: To investigate whether gravitational valves reduce the risk of overdrainage complications compared with programmable valves in ventriculoperitoneal (VP) shunt surgery for idiopathic normal pressure hydrocephalus (iNPH). BACKGROUND: Patients with iNPH may benefit from VP shunting but are prone to overdrainage complications during posture changes. Gravitational valves with tantalum balls are considered to reduce the risk of overdrainage but their clinical effectiveness is unclear. METHODS: We conducted a pragmatic, randomised, multicentre trial comparing gravitational with non-grav

Is it possible to minimize overdrainage complications with gravitational units in patients with idiopathic normal pressure hydrocephalus? Protocol of the randomized controlled SVASONA Trial (ISRCTN51046698).

Overdrainage is a common complication observed after shunting patients with idiopathic normal-pressure hydrocephalus (iNPH), with an estimated incidence up to 25%. Gravitational units that counterbalance intracranial pressure changes were developed to overcome this problem. We will set out to investigate whether the combination of a programmable valve and a gravitational unit (proGAV, Aesculap/Miethke, Germany) is capable of reducing the incidence of overdrainage and improving patient-centered outcomes compared to a conventional programmable valve (Medos-Codman, Johnson & Johnson, Germany). Lumboperitoneal Shunts for the Treatment of Idiopathic Normal Pressure Hydrocephalus: A Comparison of Small-Lumen Abdominal Catheters to Gravitational Add-On Valves in a Single Center.

BACKGROUND: Treating idiopathic normal pressure hydrocephalus (iNPH) with lumboperitoneal shunts (LPSs) may cause cerebrospinal fluid (CSF) overdrainage. OBJECTIVE: To investigate whether LPSs, including gravitational "add-on" and programmable pressure valves (PPVs/+GVs), reduce complications and improve outcomes. METHODS: We compared PPVs/+small lumen abdominal catheters (SLs) to PPVs/+GVs using different opening pressures for supine and standing positions. We analyzed 115 patients with iNPH in 2 consequent cohorts: 48 patients receiving LPSs with PPVs/+SLs and 67 patients receiving LPSs wi
