Disseminated histoplasmosis is a major issue among persons with advanced HIV in the Americas; it might also affect persons in sub-Saharan Africa, the Caribbean, and Asia and can be mistaken for other infections. By using 1992-2021 data from the French hospital database on HIV, we analyzed 198,798 persons with HIV follow-up in France and its overseas territories, identifying 553 (2.8/1,000 person-years) first episodes of disseminated histoplasmosis. Incidence rates varied by site of follow-up: 9.41 in French Guiana, 0.76 in Guadeloupe, 0.62 in Martinique, and 0.079 in mainland France. Incidence

The Guillain-Barré syndrome (GBS) has been reported as a possible complication of acute chikungunya infection. The chikungunya epidemics, which occurred in Martinique and Guadeloupe in 2014, affected 308,000 people in these two islands. GBS occurred during or immediately after acute chikungunya infection in 13 patients (10 men, three women; mean age: 61 years). Median time from acute chikungunya to GBS onset was 9 days. Twelve patients were treated with intravenous polyvalent immunoglobulins, nine of whom improved within 7 days. Five of 13 patients required mechanical ventilation. Two patients

First observed in France in 1999, Aedes albopictus (tiger mosquito) is now established in almost all French departments. As a vector of arboviruses, in particular dengue fever, A. albopictus represents a real threat in Europe, where 304 autochthonous cases of dengue fever were reported in 2024, four times more than the total number of cases reported during the entire 2010-2021 period. Although dengue is asymptomatic in the majority of cases, it can cause severe forms of the disease as hemorrhagic fever (less than 5% of cases). In the context of an unprecedented number of imported cases since t

INTRODUCTION: Acute pulmonary histoplasmosis (APH) is rare in an immunocompetent patient. We report a case of APH diagnosed by culture of broncho-alveolar lavage (BAL) in a patient presenting a pseudo-tumoral form with nodules progressing to cavitation.
OBSERVATION: A 41 year-old male smoker was hospitalized with a persistent fever, dry cough and dyspnoea on exertion. The first CT scan showed a reticulo-nodular interstitial infiltrate with lymphadenopathy that progressed rapidly to multiple pulmonary nodules with central cavitation. Bronchial endoscopy, with BAL culture, provided the diagnosi

Desert dust transported from the Sahara to the Caribbean generates peak exposures to particulate matter ≤ 10 μm (PM10). We investigated the impact of dust episodes on severe small for gestational-age births in Guadeloupe. The study sample consisted of 911 pregnant women enrolled in the Timoun mother-child cohort. Desert dust exposure was assessed through the mean daily PM10 concentrations averaged over the entire pregnancy. Numerous sociodemographic and medical risk factors were considered as covariates and introduced into multinomial logistic regression models. Odds ratios (ORs) were calculat

BACKGROUND: Collapsing glomerulopathy is a form of focal and segmental glomerulosclerosis which occurs preferentially in black people. It causes severe nephrotic syndrome and quickly progresses towards end-stage renal failure.
CASE REPORT: We report the case of a 16-year-old black girl from Guadeloupe who was admitted for tetanus and edema in 1996. She had hypoparathyroidism, renal failure and a nephrotic syndrome as well as cytomegalovirus infection. Renal biopsy showed collapsing glomerulopathy. The renal function improved on glucocorticoid and ganciclovir therapy and her serum creatinine s

Paraneoplastic pemphigus (PNP) is an autoimmune blistering skin disease of humans that consists of characteristic skin lesions associated with concurrent neoplasia. In this study we provide histologic and serologic evidence to support a diagnosis of PNP in a dog with splenic sarcoma. Skin lesions consisted of widespread erosions involving haired skin, mucocutaneous junctions, and oral mucosa. Microscopic examination of skin and mucosae revealed lesions consistent with both pemphigus vulgaris and erythema multiforme. Immunoprecipitation confirmed that circulating IgG autoantibodies from this pa

Ciguatera is a common seafood poisoning in Western Atlantic and French West Indies. Ciguatera fish poisoning in the Caribbean is a public health problem. A toxicological study was carried out on 178 Caribbean fish specimens (26 species) captured off Guadeloupe and Saint Barthelemy between 1993 and 1999. The mouse bioassay and the chick feeding test were used to control fish edibility. Ciguatoxins presence was assumed when symptomatology was typical of ciguatera in mouse and chick. Fishes were classified in three groups: non toxic fish (edible), low toxic fish (not edible) and toxic fish (not e

Since 2011, the Caribbean region-including Martinique, Guadeloupe, and French Guiana-has experienced increasing invasions of Sargassum seaweed, leading to significant environmental, economic, and public health challenges. This study evaluates the health impacts of hydrogen sulfide (H₂S) and ammonia (NH₃) emissions from decomposing Sargassum on affected populations. Data were collected from 850 patients admitted to the University Hospital of Martinique between January 2018 and September 2024, all of whom had been exposed to Sargassum emissions for an average of 2-3 months annually. Reported sym

OBJECTIVES: Characterize a NDM-1 producing K. pneumoniae isolate recovered from a patient hospitalized in Guadeloupe, French West Indies, after its transfer from Cuba
METHODS: Antibiotic susceptibilities were determined by the disk diffusion method, and E-test. Carbapenemase production was assessed using the Carba NP test. Antibiotic resistance determinants and their surrounding structures were characterized by PCR mapping and DNA sequencing. Transfer of the β-lactam resistance marker was attempted by liquid mating-out assays
RESULTS: Here we reported the first NDM-1 producing enterobacteria

Complex I is the first protein component of the mitochondrial respiratory chain and as such plays a crucial role in ATP production and mitochondrial function in general. Mitochondrial dysfunction has been identified in a number of neurodegenerative diseases. In some of these the mitochondrial abnormality is primary and in others secondary. Mitochondrial toxins are capable of producing relatively selective neuronal cell death and have been used to produce models of human neurodegenerative diseases e.g. 1-methyl 4-phenyl 1,2,3,6 tetrahydropyridine (MPTP) for Parkinson's disease, and 3-nitropropi

BACKGROUND: In Guadeloupe, cases of neurocysticercosis have been rarely observed in people recently immigrated from Haiti. We followed 2 women with seizures, associated with meningitis or encephalitis.
CASE REPORT: Computed tomography scan and muscle X-ray have displayed specific features of cysticercosis with small diffuse calcifications. This was associated with positive antibodies for cysticercosis. After respectively 5 and 17 years, these patients developed recurrent manifestations of neurocysticercosis with subacute, quite silent meningitis after pregnancy, with CSF increased cells count

The most common systemic mycoses imported into France are the histoplasmosis caused by H. capsulatum, which comes from Black Africa and from the French overseas departments and territories (Guyane, Martinique, Guadeloupe, New Caledonia), and the histoplasmosis caused by the larger H. duboisii, which is exclusively Central African. In recent years, histoplasmosis has become an opportunistic infection in immunocompromised patients, particularly those with AIDS. Blastomycosis, imported from North America as well as from North Africa and Central Africa, and paracoccidioidomycosis, imported from La

Is the genetic diversity of parasites infecting male and female hosts equal or different? This is the question we address in this paper by studying the neutral genetic variability of the plathyhelminth trematode Schistosoma mansoni within males and females of its natural murine host Rattus rattus in the marshy forest focus of Guadeloupe (French West Indies). Using seven microsatellite markers, we demonstrate that parasites from male hosts are genetically more diversified than parasites from female hosts. Three hypotheses are discussed that could explain this pattern: 1) a host sex-specific dur

A simple mathematical model was built to investigate the population biology of Schistosoma mansoni in its natural definitive host, the black rat (Rattus rattus). Prevalence and parasite abundance over 13 years from field studies and data from laboratory experiments were used to set up the model. Sensitivity analysis showed that the abundance of parasites is strongly influenced by variation in the values of infection parameters. The model shows that the parasite is able to control populations of definitive hosts. We discuss the factors that may explain the long-term persistence of S. mansoni am

The presence of schistosomiasis mansoni is known in Martinique since the beginning of the XXth century. A general survey of the distribution of the disease was carried out in 1977 and showed a mean prevalence of 12% (coprology and serology taken together) in the whole of the island. Following this survey, an integrated control programme associating sanitary education, detection and treatment of patients and improved sanitation, was developed. In addition, a biological control programme against the intermediate snail host, Biomphalaria glabrata using the competitor snail, Melanoides tuberculata

The localization of adult Schistosoma mansoni originating from Guadeloupe (West Indies) was analysed in the natural host Rattus rattus in experimental and natural infections. A transfer of schistosomes from the porto-mesenteric system to the lungs occurs between the 4th and the 20th weeks post-infection, with a peak between 4 and 8 weeks; it should be noted that the worms start laying eggs at 4 weeks post-infection. In both experimentally and naturally infected R. rattus the relocation of schistosomes in the lungs is correlated with the total worm burden. Between 6 and 12 weeks post-infection

The distribution of genetic diversity in a local population of the trematode Schistosoma mansoni was determined within and between individual wild rats at a microspatial geographic scale of a standing water transmission site. Using RAPD markers, molecular variance and canonical correspondence analysis were performed to test the significance of genetic differentiation between infrapopulations. Of total gene diversity, 8 and 11% was partitioned between hosts trapped at few metres distance from each other. Significant temporal differentiation (2%) was also detected among schistosomes sampled at 6

The risks related to dengue virus infection during pregnancy have been increasingly better described over the past 10 years. The possibility of maternal-fetal transmission is now recognized, but the diagnosis is still too late because of a wide range of clinical signs that the infected newborn child can present. From December 2009 to October 2010, Guadeloupe Island underwent an exceptional dengue epidemic. During this epidemic, at least four cases of vertical virus transmission were biologically proved. The purpose of this article is to describe the clinical aspects of these cases, some of whi

Although larval stages ensure most connectivity between demersal fish populations, dispersal traits are increasingly less measured, despite the importance of correctly parametrizing biophysical models for spatial conservation planning. Moreover, existing data are scattered across a vast multidisciplinary literature. To address this mismatch between need and availability, we compiled data from 3,201 references into multiple datasets covering 35 key pelagic dispersal traits across fish early-life stages. We specifically compiled data on swimming speed, vertical position, and delayed settlement i
