The coexistence of rheumatoid arthritis with Reiter's syndrome and/or ankylosing spondylitis: a model of dual HLA-associated disease susceptibility and expression.

In a prospective study of 184 patients with ankylosing spondylitis and/or Reiter's disease, we have examined the frequency of concomitant rheumatoid arthritis (RA) and determined the HLA profile of these patients. Five men (2.7%), 3 with ankylosing spondylitis and 2 with Reiter's disease, had coexistent classical RA and 2 of these also had Felty's syndrome. Immunogenetic analysis demonstrated that all 5 patients had either HLA-R27 or another B-7-cross-reactive antigen and that 3 also possessed HLA-DR4. Thus, 2 or more HLA-associated genetic predispositions may occur in the same individual, and

HLA-D related (DR) antigens in various kinds of myositis.

Forty-eight patients with various kinds of myositis were studied for HLA-A, B, C, and DR antigens and compared to both local and 7th International Workshop controls. No A, B, or C antigens were increased except for B8 in white patients (7/13 or 54%) (p = 0.005, pc = NS) and for B7 in black patients (6/9 or 67%) (p = 0.02, pc = NS). HLA-DR3 occurred in 8/12, or 67% of whites with polymyositis (PM) as compared to 23% of white controls (p = 0.003, pc = 0.02). HLA-DRw6 was found in 7/9, or 78%, of blacks with PM compared to 26% of black local controls (p = 0.005, pc = 0.04) and 38% of black Worksh

A possible haplotype association in Felty's syndrome.

The HLA-B7/DR association was examined in a normal British population and in seven HLA-B7-positive patients with Felty's syndrome. After the exclusion of the most frequent A3-B7-DR2 association, a significant A2-B7-DR4 association was evident. This was present in six of the seven HLA-B7-positive Felty's patients and might indicate that the A2-B7-DR4 haplotype is prevalent in some forms of rheumatoid arthritis.

Donor-specific transfusions. Donor-recipient HLA compatibility, recipient HLA haplotype, and antibody production.

HLA profiles of 25 donor-specific transfusion (DST) kidney donor-recipient pairs were analyzed for HLA antigen compatibility. Serum samples collected during and after DST were tested for cytotoxic antibodies against T and B lymphocytes of the donors and 30 normal individuals. Eleven recipients did not produce cytotoxic antibodies to the antigens of their DST donors, and eight produced cold and/or warm, broadly reactive B-cell antibodies. Six patients (24%) produced HLA-A, B, C, and/or DR antibodies. Three of these individuals produced antibodies after two immunizations, while others required t

HLA-DRB1*04 and DRB1*14 alleles are associated with susceptibility to pemphigus among Japanese.

It has previously been demonstrated that susceptibility to pemphigus vulgaris is associated with human leukocyte antigen (HLA)-DR4 serologic specificity among Ashkenase Jews, and with DR4 as well as DR6 (DR14) in other ethnic groups. We genotyped HLA-DRB1, DQA1, DQB1, and DPB1 alleles in 16 patients with pemphigus by polymerase chain reaction-restriction fragment length polymorphism, to find evidence of potential HLA class II allele associations with pemphigus in Japanese patients who have a relatively homogeneous ethnic background. All nine patients with pemphigus vulgaris and five of seven p

Association of treatment-resistant chronic Lyme arthritis with HLA-DR4 and antibody reactivity to OspA and OspB of Borrelia burgdorferi.

Chronic Lyme arthritis that is unresponsive to antibiotic therapy is associated with an increased frequency of the HLA-DR4 specificity. To determine whether the immune response to a particular polypeptide of Borrelia burgdorferi may be associated with treatment-resistant chronic Lyme arthritis, we correlated the clinical courses and HLA-DR specificities of 128 patients with Lyme disease with their antibody responses to spirochetal polypeptides. Antibody reactivity was determined by Western blotting (immunoblotting) with sonicated whole B. burgdorferi and recombinant forms of its outer surface

Langerhans cell histiocytosis patients have HLA Cw7 and DR4 types associated with specific clinical presentations and no increased frequency in polymorphisms of the tumor necrosis factor alpha promoter.

BACKGROUND: Current theory on the etiology of Langerhans cell histiocytosis (LCH), formerly Histiocytosis-X, is that abnormality(ies) of the immune system are responsible for dysregulation of Langerhans cells (LC) in patients' lesions. Among the known abnormalities in LCH patients are increased amounts of tumor necrosis factor alpha (TNF-alpha) and other cytokines in the lesions. PROCEDURE: We investigated the human leukocyte antigen (HLA) phenotypes of 29 patients and 37 healthy family members to determine if any haplotypes segregate with the presence or locations of the disease. The lymphoc

HSP70-2 9.0 kb variant is in linkage disequilibrium with the HLA-B and DRB1* alleles associated with clozapine-induced agranulocytosis.

In order to extend the analysis of the association between class I and class II HLA markers and HSP-70 alleles, we studied the genetic polymorphism of HSP70-2 genes by restriction fragment length polymorphism analysis in a panel of HLA-homozygous cell lines carrying the HLA-B alleles known to be associated with clozapine-induced agranulocytosis. We have found a linkage disequilibrium between the 9.0 kb variant of HSP70-2 with the class I antigens HLA-B7, B38, and B44 and with the class II antigens HLA-DR2 and DR4. We discuss the importance of analyzing the variants of HSP70-2 in patients with

Vogt-Koyanagi-Harada disease.

Vogt-Koyanagi-Harada disease (VKH) is a multisystem autoimmune disorder principally affecting pigmented tissues in the ocular, auditory, integumentary and central nervous systems. Patients are typically 20 to 50 years old and have no history of either surgical or accidental ocular trauma. Pigmented races are more commonly affected. Depending on revised diagnostic criteria, the disease is classified as complete, incomplete or probable based on the presence of extraocular findings (neurological, auditory and integumentary). The clinical course of VKH is divided into four phases: prodromal (mimic

The immunogenetics of juvenile rheumatoid arthritis.

Recent major advances in understanding the genetic structure of the human leukocyte antigen (HLA) region and how HLA molecules contribute to immune responses have been paralleled by more precise identification of specific HLA genes conferring susceptibility to the various forms of juvenile rheumatoid arthritis (JRA). This article presents current models for HLA-associated autoimmune disease susceptibility and summarizes the HLA Class II alleles currently known to be associated with JRA: primarily DR8, DR5, DR6, and DPw2.1 in pauciarticular onset JRA; and DR4 in rheumatoid factor-positive polya

The HLA-DR phenotype of the responder is predictive of humoral response against HLA class I antigens.

Recent studies suggest that the immunogenicity of an human leukocyte antigen (HLA) incompatibility should be considered in the context of the HLA phenotype of the recipient. The HLA-DR phenotype of the responder is thought to be predictive for the strength of the alloimmune response. In order to analyze the humoral response against HLA class I antigens in the context of the HLA-DR phenotype of the responder, we selected all HLA-DR homozygous Dutch patients that were present on the Eurotransplant waiting list between 1967 and 2000 (n=1,317 patients). By logistic regression it was determined whe

HLA-DQA1 genotyping in patients with rheumatoid arthritis in Taiwan.

To investigate the role of HLA-DQA1 genotypes and their interaction with HLA-DRB1 in the pathogenesis of rheumatoid arthritis (RA) in Taiwan, HLA-DQA1 was determined in 71 patients with RA and 108 healthy controls by SSP-PCR method. HLA-DRB1 and HLA-DQA1 were simultaneously detected in 55 RA patients and 101 healthy controls. PCR/SSOP method was used to determine the HLA-DRB1 genotypes, and the subtypes of HLA-DR4 were determined by cloning and sequencing. The phenotypic frequency of HLA-DQA1*0301 was significantly lower in RA than in controls, and, in contrast, the HLA-DQA1*0302 and DQA1*0303

Transplantation of mouse HSCs genetically modified to express a CD4-restricted TCR results in long-term immunity that destroys tumors and initiates spontaneous autoimmunity.

The development of effective cancer immunotherapies has been consistently hampered by several factors, including an inability to instigate long-term effective functional antitumor immunity. This is particularly true for immunotherapies that focus on the adoptive transfer of activated or genetically modified mature CD8+ T cells. In this study, we sought to alter and enhance long-term host immunity by genetically modifying, then transplanting, mouse HSCs. We first cloned a previously identified tumor-reactive HLA-DR4-restricted CD4+ TCR specific for the melanocyte differentiation antigen tyrosin

Seronegative rheumatoid arthritis. A distinct immunogenetic disease?

Measurement of serum rheumatoid factor (RF) by conventional methods in patients with rheumatoid arthritis (RA) has repeatedly identified a subpopulation of patients without detectable RF. Previous investigators have consistently confirmed the association of HLA-DR4 with seropositive RA, but studies of seronegative RA have been limited and contradictory. We studied 140 randomly selected patients from Alabama, all of whom had either classic or definite RA, and we were able to complete HLA typing for 110 of these individuals. Eighty were consistently seropositive (on at least 2 separate occasions

Paternal histocompatibility (HLA) antigens and maternal anti-HLA antibodies in herpes gestationis.

It has been suggested that anti-HLA antibodies might be involved in the pathogenesis of herpes gestationis (HG). Accordingly, we have studied the frequency and specificity of such antibodies in 26 female patients with immunologically proven HG. In addition, to further investigate the potential association of the husband's antigens in the development of this disorder, we have performed HLA typing in 20 of the husbands of these women. HLA-DR2 was found in 50% of the husbands (controls 25%, p = 0.04). The increase was more pronounced in the husbands of patients with the HLA-DR3, DR4 combination (

Histocompatibility antigens in primary biliary cirrhosis.

HLA-A, -B, and -DR antigens were examined in 71 patients with primary biliary cirrhosis (PBC) enrolled in a randomized, double-blind trial of colchicine versus placebo. All patients had typical laboratory and histological features of PBC, except that six had a negative test for antimitochondrial antibody. Frequencies of these antigens were compared with the international Caucasian panel of the Eighth International Workshop and with a local Caucasian panel. Antigen frequencies were compared using the chi 2 test, with a correction for the number of antigens tested. The PBC patients had a signifi

An immunogenetic study of older age onset rheumatoid arthritis.

Thirty-two patients with older age onset rheumatoid arthritis (ORA), defined as disease onset after age 60, were selected for HLA typing. The majority (69%) were rheumatoid factor (RF) seronegative. An increase in HLA-DR4, though not statistically significant, was seen in ORA (38% vs 17% normals). This antigen was strongly associated with RF seropositivity (70%, p less than 0.01) and rheumatoid nodules (75%, p less than 0.01) in ORA. We conclude that the immunogenetics of ORA are similar to that described for adult RA in general.

Analysis of HLA genes in families with autoimmune diabetes and thyroiditis.

Type 1 diabetes (T1D) and autoimmune thyroid disease (AITD) are the most common autoimmune endocrine disorders. The similar pathogenesis of T1D and AITD and their tendency to occur together suggest that their etiology may involve common genetic factors. We hypothesized that the human leukocyte antigen (HLA) locus may contribute in part to the joint susceptibility to T1D and AITD. We therefore analyzed a data set of 40 multiplex families in which T1D and AITD clustered ("T1D-AITD families") for linkage and association with the HLA class II locus. We found evidence for linkage of the HLA region

HLA-DPB1 alleles in patients with rheumatoid arthritis.

The distribution of HLA-DPB1 alleles was studied in 94 adult Caucasian RA patients (65 = seropositive, 29 = seronegative) and 40 normal controls. The DPB1 alleles were defined by oligonucleotide typing of polymerase chain (PCR)-amplified genomic DNA. The prevalence of the DPB1*0402 allele was higher in seropositive RA patients and DPB1*0201 was higher in seronegative RA patients. These differences were not significant, however. It is likely that DPB1 alleles do not play an important role in susceptibility to RA.
