Abnormal calcium signalling and the caffeine-halothane contracture test.

BACKGROUND: The variable clinical presentation of malignant hyperthermia (MH), a disorder of calcium signalling, hinders its diagnosis and management. Diagnosis relies on the caffeine-halothane contracture test, measuring contraction forces upon exposure of muscle to caffeine or halothane (FC and FH, respectively). Patients with above-threshold FC or FH are diagnosed as MH susceptible. Many patients test positive to halothane only (termed 'HH'). Our objective was to determine the characteristics of these HH patients, including their clinical symptoms and features of cytosolic Ca2+ signalling r

Jaw relaxation after a halothane/succinylcholine sequence in children.

BACKGROUND: Lack of complete jaw relaxation after a halothane-succinylcholine sequence has been described in the literature. To date, however, most existing studies are retrospective, and lack agreement on the magnitude and incidence of this phenomenon. This prospective study examined the incidence and degree of incomplete jaw relaxation in 500 children who were given intravenous succinylcholine during halothane anesthesia. METHODS: Five hundred consecutive unmedicated children received a minimum dose of 2 mg/kg intravenous succinylcholine after induction of anesthesia with halothane. The deg

Halothane anesthesia causes state-dependent retrieval failure in mice.

Effects of exposure to halothane on memory processing was studied using single-trial inhibitory avoidance learning to measure retention. Mice were anesthetized with halothane either before training, immediately after training, or both before training and before testing. Results showed that memory was not impaired by posttraining halothane exposure, indicating that the anesthetic does not cause retrograde amnesia. Mice trained after recovery from halothane showed a robust memory loss 24 h later. This deficit could be alleviated by reexposure to the anesthetic before the retention test. Mice giv

Halothane anesthesia abolishes pulmonary vascular responses to neural antagonists.

We investigated the effects of the inhalational anesthetic halothane on autonomic nervous system (ANS) regulation of the baseline pulmonary vascular pressure-flow (P/Q) relationship compared with that measured in the conscious state. Multipoint pulmonary vascular P/Q plots were constructed by stepwise constriction of the thoracic inferior vena cava to decrease venous return and Q. P/Q plots were generated in the same dogs in the conscious state and during halothane anesthesia (approximately 1.2% end tidal) in the intact (no drug) condition and after administration of selective ANS antagonists.

Suspected malignant hyperthermia after halothane anesthesia in a cat.

A 6 month old domestic shorthaired cat died of suspected malignant hyperthermia. Anesthesia was induced and maintained with halothane vaporized in oxygen and nitrous oxide, after acepromazine premedication. Before an incision was made, the cat's heart rate dropped from 140 to 90 beats/min concomitant with a drop in blood pressure. Glycopyrrolate administration resulted in severe ventricular tachycardia (340 beats/min). Halothane and nitrous oxide were discontinued and the surgery was abandoned. Lidocaine administration resulted in a normal sinus rhythm. In recovery, the cat was tachypneic and

Porcine malignant hyperthermia: false negatives in the halothane test.

Purebred Pietrain pigs presumed (on the basis of pedigree) to be homozygous for malignant hyperthermia (MH) susceptibility were subjected to a 3% halothane challenge test. A few (6%) pigs that should have been MH susceptible on the basis of parental genotype did not develop muscle rigidity in response to repeated halothane tests. Three of these animals were brought into the laboratory, and muscle biopsy specimens were obtained for in vitro analysis. Bundles of intact muscle cells dissected from biopsy specimens were electrically stimulated, and mechanical responses were monitored during exposu

[Modification of postoperative somnolence following halothane anesthesia by physostigmine].

In a prospective study the influence on awakening after halothane anaesthesia of physostigmine was investigated. Of a total of 62 patients 29 received physostigmine 2 mg i.v. prior to extubation, 33 were left without. The degree of awareness in the patients was determined by Erzigkeit's "Syndrom-Kurz-Test" (Quick Syndrome Test) (SKT). On preoperative testing by SKT both groups showed comparable degrees of awareness. At 45 and 90 min after extubation no statistical differences in the SKT were seen between the groups. If broken up for age over or below 60 yr, a tendency towards improved alertnes

[Halothane and antituberculous drugs--a hepatotoxic combination? (author's transl)].

A case report is given on two patients receiving halothane anesthesia while beeing treated with isoniacid, ethambutol and rifampicin. Following halothane anesthesia, both patients developed a severe liver disease with encephalopathy grade III. We observed a moderate increase of bilirubin and SGOT and a more severe increase of serum ammonia. Histologically, both patients had alterations compatible with drug hepatitis. Within 14 days remission occurred spontaneously. The two case reports do not fit with typical isoniacid hepatitis or typical halothane hepatitis. The possibility of combined drug

Intraocular pressures in children with glaucoma during halothane anesthesia.

Thirty measurements of intraocular pressure (IOP) were performed on 10 children either with congenital or secondary glaucoma, before and after halothane anesthesia. In 7 normal eyes, IOP decreased a mean of 3 mm Hg, while in 23 glaucomatous eyes, mean IOP decreased from 43 to 34 mm Hg. In 5 of the 23 glaucomatous eyes, IOP decreased below 25 mm Hg and in only one patient below 21 mm Hg. Our findings indicate that an abnormally elevated IOP during halothane anesthesia suggests a glaucomatous process. A normal IOP during halothane does not absolutely preclude the absence of glaucoma. Halothane-induced porcine malignant hyperthermia: metabolic and hemodynamic changes.

Metabolic, hemodynamic and neuroendocrine responses to halothane were measured in five normal and five malignant hyperthermia-susceptible (MHS) swine. Constant-volume ventilation was used. There was no therapeutic intervention. In NHS animals, blood lactate concentrations increased first, and the initial increases appeared to be non-hypoxic in origin. Lactate concentrations increased progressively to more than 20 mum/ml. Whole-body oxygen consumption increased almost twofold, and hind limb muscle oxygen consumption increased almost threefold. Extrapolated increases in muscle oxygen consumption

Optical reversal of halothane-induced immobility in C. elegans.

Volatile anesthetics (VAs) cause profound neurological effects, including reversible loss of consciousness and immobility. Despite their widespread use, the mechanism of action of VAs remains one of the unsolved puzzles of neuroscience [1, 2]. Genetic studies in Caenorhabditis elegans [3, 4], Drosophila [3, 5], and mice [6-9] indicate that ion channels controlling the neuronal resting membrane potential (RMP) also control anesthetic sensitivity. Leak channels selective for K(+) [10-13] or permeable to Na(+) [14] are critical for establishing RMP. We hypothesized that halothane, a VA, caused im

Interactions between neuronal histamine and halothane anesthesia in rats.

Using an in vivo microdialysis method, we measured the release of histamine in the anterior hypothalamic area (AHy) of rats under several concentrations of halothane anesthesia (1, 0.5, and 0.2%). The release of histamine increased to 341 and 325% at halothane concentrations of 0.5 and 0.2%, compared with the basal level at anesthesia induced by 1% halothane. alpha-Fluoromethylhistidine (100 mg/kg i.v.), a specific and irreversible inhibitor of histidine decarboxylase, reduced the histamine release to <35% of the basal value at 1% halothane anesthesia in the AHy, and also decreased the anesthe

Phrenic nerve stimulation during halothane anesthesia. Effects of atelectasis.

BACKGROUND: Atelectasis formation during anesthesia may be due to loss of respiratory muscle tone, in particular that of the diaphragm. This was tested by tensing the diaphragm by phrenic nerve stimulation (PNS) and observing the effect on atelectasis. METHODS: Twelve patients (mean age 48 yr) without preexisting lung disease were studied during halothane anesthesia. PNS was executed with an external electrode on the right side of the neck. Chest dimensions and area of atelectasis were studied by computed tomography of the chest. RESULTS: Right-sided PNS against an occluded airway at functio

Malignant hyperthermia and severe hypoglycemia after reexposure to halothane.

A four year child presented with an episode of malignant hyperthermia induced by reexposure to halothane. The episode was associated with early onset of a severe hypoglycemia and liver enzymatic perturbances. Etiology of these perturbances is hard to state precisely in this clinical context, but a hepatic toxicity of halothane and/or by dantrolene is considered.

Halothane anesthesia and aqueous humor dynamics in laboratory animals.

Halothane anesthesia decreased intraocular pressure (IOP) by approximately 4 mm Hg in both normal rabbits and monkeys. The administration of nitrous oxide or 100% oxygen had no effect on IOP. Although PO2 and PCO2 increased following halothane administration in rabbits, similar blood changes occurred without IOP reduction in control rabbits receiving 100% oxygen. Outflow facility in both species as measured by tonography remained unchanged by halothane administration. Aqueous humor flow as estimated by tonography decreased by about 39% in rabbits and 31% in monkeys. Posterior chamber aqueous h

Arrhythmias during halothane anesthesia. III. The influence of barbiturates.

The effect of barbiturates on cardiac rate and rhythm during halothane anesthesia without intubation was studied in a triple-blind, randomized trial with 28 patients. The test group was premedicated with pentobarbital and induced with thiopental, while the control group was anesthetized with halothane and nitrous oxide only. There was a significant preponderance of sinus-bradycardia and supraventricular disturbances in the control group. It is concluded that the use of barbiturates is advantageous because of their ability to suppress enhanced vagal tone.
