Coordination of multi-joint arm movements in cerebellar ataxia: analysis of hand and angular kinematics.

Kinematic abnormalities of fast multijoint movements in cerebellar ataxia include abnormally increased curvature of hand trajectories and an increased hand path and are thought to originate from an impairment in generating appropriate levels of muscle torques to support normal coordination between shoulder and elbow joints. Such a mechanism predicts that kinematic abnormalities are pronounced when fast movements are performed and large muscular torques are required. Experimental evidence that systematically explores the effects of increasing movement velocities on movement kinematics in cerebe

Associations of trabecular bone texture with anatomical phases of osteoarthritis evolution and its progression in finger joints: Results from the Nor-Hand study.

OBJECTIVE: To explore differences in trabecular bone (TB) texture with anatomical phases in the evolution of hand osteoarthritis (OA), and association of baseline TB texture with a progression to erosive OA (EOA) at follow-up. DESIGN: The Norwegian Hand OA (Nor-Hand) study (300 persons) was used. Distal (DIP) and proximal (PIP) interphalangeal joints on hand radiographs were graded according to the Verbruggen-Veys anatomical phase scoring system (normal, stationary, joint space narrowing (JSN), erosive, remodeled) at baseline and 3.5 years follow-up. Fractal dimensions (FDs) along the horizon

Are Preoperative Corticosteroid Injections in Large or Intermediate Joints Associated with Surgical Site Infection After Soft Tissue Hand Surgery? A Retrospective Database Analysis.

PURPOSE: In this study, we asked the following question: Is intra-articular corticosteroid injection at a distant site prior to hand surgery associated with an increased risk of (1) surgical site infection, (2) reoperation for infection, or (3) wound dehiscence? METHODS: Using a national administrative claims database, we identified adult patients undergoing carpal tunnel, trigger finger, or DeQuervain release. Patients were divided into four cohorts: intra-articular corticosteroid injection between 0 and 30, 31 and 60, or 61 and 90 days before surgery or no injection within 90 days prior to

Detecting hand joint ankylosis and subluxation in radiographic images using deep learning: A step in the development of an automatic radiographic scoring system for joint destruction.

We propose a wrist joint subluxation/ankylosis classification model for an automatic radiographic scoring system for X-ray images. In managing rheumatoid arthritis, the evaluation of joint destruction is important. The modified total Sharp score (mTSS), which is conventionally used to evaluate joint destruction of the hands and feet, should ideally be automated because the required time depends on the skill of the evaluator, and there is variability between evaluators. Since joint subluxation and ankylosis are given a large score in mTSS, we aimed to estimate subluxation and ankylosis using a

Elbow joint loading and spatiotemporal performance under different hand positions during the forward handspring.

This study examined how different hand positions influence key biomechanical characteristics of elbow joint loading and performance during the forward handspring. Nine adolescent male gymnasts performed five trials using parallel (PHP) and outward (OHP) hand positions. Kinematic and kinetic data were collected using eight infrared cameras (250 Hz) and two force plates (1,000 Hz), with analysis conducted on the arm in contact with the force plate. The OHP produced greater maximum vertical and resultant ground reaction forces and net vertical impulse than the PHP, while exhibiting a shorter cont

Prospective validation of mean metacarpophalangeal joint extension as a measure of diabetes-related fibrotic hand manifestations.

INTRODUCTION: Hand conditions in diabetes, namely, limited joint mobility (LJM), flexor tenosynovitis (FT), carpal tunnel syndrome (CTS), and Dupuytren disease (DD), share a common pathophysiological process involving pro-fibrotic inflammation in flexor structures. A unified, quantitative measure of disease severity across these conditions is lacking, limiting correlational research. We evaluated mean metacarpophalangeal (MCP) joint extension as a potential measure of severity. METHODS: We assessed 2,405 adults, including individuals with type 1 diabetes (n=291), type 2 diabetes (n=877), pred

Changing patterns in joint replacement surgery in the hand in Sweden: a population-based study of 5382 patients.

Over a 16-year period (2008-2023), data from the Swedish National Patient Register reveal shifting trends in operation for hand joint replacements linked to demographic, healthcare and surgical advances. Among 5382 identified cases, 63% were women, with the incidence peaking in the 65-74 age bracket, highlighting a marked gender gap in middle and older age groups. Regional analyses indicate significant disparities, as Örebro and Halland had rates exceeding 10 per 100,000, while Stockholm and Blekinge fell below 5 per 100,000. The adoption of total prostheses without cement declined by 22%, ref

Ensemble detection of hand joint ankylosis and subluxation in radiographic images using deep neural networks.

The modified total Sharp score (mTSS) is often used as an evaluation index for joint destruction caused by rheumatoid arthritis. In this study, special findings (ankylosis, subluxation, and dislocation) are detected to estimate the efficacy of mTSS by using deep neural networks (DNNs). The proposed method detects and classifies finger joint regions using an ensemble mechanism. This integrates multiple DNN detection models, specifically single shot multibox detectors, using different training data for each special finding. For the learning phase, we prepared a total of 260 hand X-ray images, in

Biomechanical Analysis of the Effect of Finger Joint Configuration on Hand Grasping Performance: Rigid vs Flexible.

Human finger joints are conventionally simplified as rigid joints in robotic hand design and biomechanical hand modelling, due to their anatomic and morphologic complexity. However, our understanding of the effect of the finger joint configuration on the resulting hand performance is still primitive. In this study, we systematically investigate the grasping performance of the hands with the conventional rigid joints and the biomechanical flexible joints based on a computational human hand model. The measured muscle electromyography (EMG) and hand kinematic data during grasping are used as inpu

Finger orthoses for management of joint hypermobility disorders: Relative effects on hand function and cognitive load.

BACKGROUND: Joint hypermobility refers to joints that move beyond their normal limits. Individuals with hypermobility of the fingers experience difficulties in activities of daily living. Finger orthoses are available for managing hypermobility of the fingers, but their effectiveness has received little attention in scholarly literature. OBJECTIVES: To determine if use of custom fit finger orthoses leads to improvements in time needed to perform standardised hand function tests, and attentional demand required to perform these tests, in individuals with joint hypermobility syndrome, Hypermobi

National Prevalence of Complications and Cost of Small Joint Arthroplasty for Hand Osteoarthritis and Post-Traumatic Arthritis.

PURPOSE: Osteoarthritis (OA) of the hand is commonly treated using implant arthroplasty. Despite the increasing prevalence of hand OA, population-based evidence regarding the complication profile and associated cost for patients undergoing proximal interphalangeal (PIP) joint and metacarpophalangeal (MCP) joint arthroplasty are lacking. Therefore, we aimed to evaluate the complication profiles and variation in cost of care for patients undergoing PIP and MCP joint arthroplasty. METHODS: We analyzed insurance claims from 2009 to 2016 using the Truven MarketScan Databases for adult patients und

Night-time immobilization of the distal interphalangeal joint reduces pain and extension deformity in hand osteoarthritis.

OBJECTIVE: DIP joint OA is common but has few cost-effective, evidence-based interventions. Pain and deformity [radial or ulnar deviation of the joint or loss of full extension (extension lag)] frequently lead to functional and cosmetic issues. We investigated whether splinting the DIP joint would improve pain, function and deformity. METHODS: A prospective, radiologist-blinded, non-randomized, internally controlled trial of custom splinting of the DIP joint was carried out. Twenty-six subjects with painful, deforming DIP joint hand OA gave written, informed consent. One intervention joint an

Free hand-to-toe transfer: a method to minimise donor-site morbidity in free joint transfers.

Reconstruction of a congenital hand anomaly in a child using single free vascularised transfer of the proximal interphalangeal joint of a second toe with the simultaneous microvascular reconstruction of the donor toe using the stiff joint and its dorsal skin paddle from the hand is described. This is not the first reported case of a toe-finger switch, but it is the first in a free joint transfer, for which it is especially indicated.
