Split hand/split foot malformation with hearing loss: first report of families linked to the SHFM1 locus in 7q21.

Developmental anomalies of the appendicular skeleton are among the most common and easily ascertained birth defects. Split hand/split foot malformations, distinctive in having deficiency of the central rays, occur as isolated anomalies and as one component of multisystem syndromes. The clinical and molecular characterization of a new syndrome, found in two unrelated families, consisting of split foot with hearing loss, is presented here. As in other split hand/split foot conditions, variable expression and reduced penetrance is notable. In the larger family, variably expressed split foot malfo

Vibration syndrome and autonomic nervous system.

It is well known that hand-arm vibration affects the hand and arm which are directly exposed to vibration. However, through the sympathetic nervous system, hand-arm vibration can affect the foot which is not directly exposed to vibration. Hand-arm vibration activates the sympathetic nervous system, and induces vasoconstriction in the four extremities. And patients with vibration syndrome have circulatory disturbances of the foot as well; some patients had Raynaud's phenomenon of both fingers and toes, those with VWF were likely to complain of coldness in the feet as well as the hands, and had

Lobster-claw syndrome.

Lobster-Claw syndrome is a rare autosomal dominant, hand-foot malformation with Oro-dental features. This is a rare condition and from the available reports so far, cases being reported are less than 1%. Most reports have focused on the hand-foot deformity of this syndrome. This paper highlights the typical Oro-dental features associated with this syndrome such as retained deciduous teeth, hypodontia and variation in crown size, arch length and arch width.

Trismus-pseudocamptodctyly syndrome: report of one case.

Trismus-pseudocamptodactyly syndrome is a rare neuromuscular disorder that manifests as a combination of hand, foot, and mouth anomalies. Limited range of motion for hands, feet, and mouth are typical presentations. A male term newborn baby was noted to have trismus, ulnar deviation of both wrist joints, limited stretch of digits during hand dorsiflexion but with full digital extension during volarflexion, and talipes equinovarus of the right foot. These anomalies were not seen in his parents. Hecht syndrome, or trismus-pseudocamptodactyly syndrome, was diagnosed with a negative result of elec

Bringing the feet in from the cold: thermal biofeedback training of foot-warming in Raynaud's syndrome.

The biofeedback-assisted treatment of a case of Raynaud's syndrome was examined in order to determine the relationship between learning of the hand-warming response and the subsequent ability to produce foot-warming responses. It was found that fluctuations in dermal hand and foot temperatures were not significantly related, either within or across treatment sessions. The obtained hand-warming response was of high magnitude and rapidly learned, while the foot-warming response was more modest and took substantially more trials to learn. These results indicate that an easy generalization of the

Pollicization of Middle Finger in a Cleft Hand Associated with Acrorenal Syndrome.

Introduction  Acrorenal syndrome is autosomal recessive inherited disorder commonly associated with congenital renal disorders and ipsilateral hand/foot anomalies. The hand and foot deformities corrections are challenging to achieve a good functional and cosmetic result. We described a case of acrorenal syndrome with suppressed radial elements and absent thumb in whom we were able to reconstruct the hand. Case Report  A 5-year-old girl presented to us with right renal agenesis and right hand/foot deformity. The hand showed a cleft hand with central deficiency, index finger hypoplastic, and syn

Novel genetic syndrome manifesting with cerebral atrophy, cataract, hypoacusis, diabetes, and brachy-/syndactyly.

Genetic disorders manifest clinically in a variety of phenotypes. A patient with cataract, hypoacusis, hand and foot deformities, and diabetes was not reported. The patient is a 74-year-old male with a history of congenital foot and hand deformities manifesting as brachydactyly of fingers 4 and 5 on the right side and brachydactyly and syndactyly of the fingers 1-5 on the left side. Foot deformities required orthopedic surgery from an early age. He later developed cataract that required surgery, hypoacusis with tinnitus on the left side, and diabetes. Cerebral MRI revealed generalized atrophy.

Preliminary assessment of the reliability and validity of infrared skin temperature measurements in Rett syndrome.

BACKGROUND: As clinical trials for Rett syndrome are underway, there is a need to validate potential supplemental outcome measures that reflect important signs and symptoms. Autonomic dysfunction, particularly vasomotor dysfunction, is one potential area for which biomarkers could be developed. METHODS: In the current study, infrared thermal images of hands and feet from 26 females with Rett syndrome (aged 62 months to 39 years), and 17 females without known intellectual, genetic or neurological disorders (aged 55 months to 39 years) were collected. Between-group differences in skin temperatu

A 2q24.3q31.1 microdeletion found in a patient with Filippi-like syndrome phenotype: a case report.

Filippi syndrome is characterized by developmental delay, growth failure, cryptorchidism, bilateral hand and foot syndactyly, and facial dysmorphism. The 2q24q31 contiguous deletion syndrome has similarly been associated with hand and foot anomalies, growth retardation, microcephaly, characteristic facies with a broad prominent nasal root and thin alae nasi, and intellectual disability. We present a patient with this deletion who has a Filippi-like phenotype, which may be the first causative cytogenetic result in this syndrome. This suggests the importance of array comparative genomic hybridiz

The p63 gene in EEC and other syndromes.

Several autosomal dominantly inherited human syndromes have recently been shown to result from mutations in the p63 gene. These syndromes have various combinations of limb malformations fitting the split hand-split foot spectrum, orofacial clefting, and ectodermal dysplasia. The p63 syndrome family includes the EEC syndrome, AEC syndrome, ADULT syndrome, limb-mammary syndrome, and non-syndromic split hand/foot malformation. The pattern of heterozygous mutations is distinct for each of these syndromes. The functional effects on the p63 proteins also vary between syndromes. In all of these syndr

Prenatal diagnosis of ectrodactyly: the 'lobster claw' anomaly.

Ectrodactyly is a rare condition characterized by absence of digits, ranging from single absence of a finger to the split hand/foot deformity. This anomaly has been reported as sporadic and in association with a number of genetic and non-genetic syndromes, such as the autosomal dominant ectrodactyly and the ectrodactyly-ectodermal dysplasia-cleft palate (EEC) syndromes. In this report we present two cases of ectrodactyly prenatally diagnosed by ultrasonography during the second trimester of pregnancy. Routine examination of fetal hands and feet during second-trimester ultrasonography should ma

Polydactyly: a study of four generations of a Turkish family including an affected member with bilateral cleft lip and palate.

Polydactyly is one of the most common congenital deformities of the hands. It can occur as an isolated disorder, in association with other malformations of the hands or feet, or as part of a syndrome. It can occur sporadically but it can also be inherited with a mainly autosomal dominant inheritance. We present a Turkish family with affected members in four generations. Bilateral duplication of the second digit in both hands and feet with 24 digits in total was the most common pattern, but one affected member had 26 digits: seven on each hand and six on each foot. In addition, another affected

TP63-related disdorders broadly involve varying combinations of ectodermal dysplasia (sparse hair, hypohydrosis, tooth abnormalities, nail dysplasia), cleft lip/palate, acromelic malformation, split-hand/foot malformation/syndactyly, ankyloblepharon filiforme adnatum, lacrimal duct obstruction, hypopigmentation, and hypoplastic breasts and/or nipples. TP63-related disorders are associated with heterozygous pathogenic variants in TP63 and include seven overlapping phenotypes; Ankyloblepharon-ectodermal defects-cleft lip/palate syndrome (AEC), Ectrodactyly-ectodermal dysplasia-cleft lip/palate s

Reconstruction of a Monodactylous Hand with Microsurgical Free Foot-to-Hand Transfer in Split-Hand/Split-Foot Malformation with Tibial Aplasia.

Split-hand/split-foot malformation with long bone deficiency (SHLFD syndrome) is a rare congenital disorder, which may be sporadic or autosomal dominant with incomplete penetrance. When complete tibial aplasia is seen, the mainstay of treatment is amputation and lower limb prosthesis. This rare constellation of congenital differences presents an opportunity for microsurgical free tissue transfer using the principle of "spare parts" to improve the functionality of the hand. We present a rare case of split-hand/split-foot malformation with a monodactylous right hand and complete tibial aplasia,

[Carpal Extensor Tenosynovitis and Adult Sickle Cell Vaso-Occlusive Crisis].

If the acute and chronic osteo-articular complications of sickle cell anemia are well-known to the rheumatologist, certain forms including hand-foot syndrome are less so since they almost exclusively concern small children less than 4 years of age. We report here the observation of a hand-foot syndrome in a 36-year-old adult, homozygous sickle cell disease (SS), during a vaso-occlusive crisis, manifested by tenosynovitis of carpal extensors. After eliminating the other etiologies, notably infectious of palmar tenosynovitis, the vaso-occlusive origin will be confirmed by the rapidly favorable e

Skin Toxicities: Common Side Effect.

For skin toxicities, standards of care are based on established evidence-based practice. Cancer treatment-related skin toxicities are a frequent and distressing side effect of antineoplastic therapies, particularly chemotherapy and targeted therapies. Toxicities associated can appear as rashes, hand-foot skin reaction, hand-foot syndrome, and hair loss. As many as 90% of patients will experience some skin toxicity at some point in their therapy.

Paclitaxel-induced periarticular thenar eminence erythema with onycholysis: A case report.

Paclitaxel is a drug frequently used in the treatment of gynecological cancers. Its cutaneous side effects are fairly well documented. A subtype of hand-foot syndrome, periarticular erythema of the thenar eminences with onycholysis, is rarer. Here, we present a case of a woman treated with paclitaxel for recurrent ovarian cancer who developed periarticular thenar eminence erythema with onycholysis syndrome. Involvement presented as an erythematous rash on the top of the left hand progressing up the arm. A lesion was also present on the right lower limb and on the dorsal surface of the right fo

Cheiro-pedal syndrome following pontine infarction.

We report the case of a 64-year-old man with sudden onset of numbness in the right hand and foot. Neurological examinations were normal except for hypersthesia, and hyperalgesia of the right hand and foot. Brain MRI demonstrated a high signal intensity on T2-weighted image and a low signal intensity on T1-weighted image in the left tegmetum of the pons. He was diagnosed with pontine infarction presenting with cheiro-pedal syndrome (CPS). Damage in the sensory pathways can cause CPS. Difference in the threshold may explain the specific sensory pattern in this syndrome. Further examination of th

Digital Transfer for Hand Reconstruction in Cleft Hand and Foot Differences.

BACKGROUND: Digital transfer for hand reconstruction in children with cleft hand and foot differences present unique challenges with anomalous anatomy and rare opportunities to dramatically improve function of one- or two-digit hands. METHODS: Medical records were reviewed for patients with cleft hand and foot treated at two pediatric institutions between 1996 and 2018. Hospital records, clinical photographs, radiographs, and alginate molds were available on all patients. Patient characteristics, indications for transfer, associated syndromes, donor and recipient anatomy, and complications we
