Hand motor function of patients with early- and late-onset Alzheimer's disease and its relation to activities of daily living.

BACKGROUND: Alzheimer's disease (AD) is the most common cause of dementia, characterized by a progressive deterioration of cognitive capacity and the ability to carry out activities of daily living (ADL). Hand motor function may also be impaired, with slower and less automated movements during fine motor tasks. The severity of impairments may depend on task characteristics and the age of onset of AD. OBJECTIVE: This study investigated hand motor function of persons with dementia due to AD during handwriting and sequential hand movements, focusing on the impact of task complexity and age at on

Task specific grip force control in writer's cramp.

OBJECTIVE: Writer's cramp is defined as a task specific focal dystonia generating hypertonic muscle co-contractions during handwriting resulting in impaired writing performance and exaggerated finger force. However, little is known about the generalisation of grip force across tasks others than writing. The aim of the study was to directly compare regulation of grip forces during handwriting with force regulation in other fine-motor tasks in patients and control subjects. METHODS: Handwriting, lifting and cyclic movements of a grasped object were investigated in 21 patients and 14 controls. T

Evidence of graphomotor dysfunction in children with dyslexia: A combined behavioural and fMRI experiment.

AIM: Children with dyslexia (DYS) have a deficit in spelling (i.e., central processes of writing), and past experiments have suggested that they also frequently experience difficulties in handwriting (i.e., motor peripheral processes of writing) compared with typically developing children (TD). However, the presence of handwriting difficulties in dyslexia is controversial. This experiment aimed to better understand the writing difficulties in DYS children, investigating both the central and peripheral processes of writing and combining cognitive and neuroimaging data. METHOD: Participants wer

Is writer's cramp caused by a deficit of sensorimotor integration?

Writer's cramp is a highly specific movement disorder in which handwriting is impaired while most other manual skills are often unaffected. On the basis of abnormal findings in experiments measuring the control of grip forces, it has been suggested that writer's cramp is caused by a deficit of sensorimotor integration. The aim of our study was to determine whether there is a functional link between sensory deficits, abnormalities in the control of grip force, and handwriting disorders. We compared the grip force and handwriting performance of writer's cramp patients with that of control subjec

Albert Schweitzer: a patient with writer's cramp.

Albert Schweitzer (1875-1965) the world-famous philosopher, theologian, concert organist, musicologist, philanthropist and winner of the 1952 Nobel Peace Prize suffered throughout most of his life from severe and painful muscle cramps in his right upper extremity which were triggered exclusively by handwriting. They led to tonic finger flexion and wrist extension and produced slow and clumsy handwriting of a reduced character size. Other motor functions including Schweitzer's highly skilful and famous organ playing were not affected. Inheritance from his mother is likely. Schweitzer applied se

How to assess motor impairment in writer's cramp.

Writer's cramp is a task-specific hand dystonia affecting handwriting. Clinical scores such as the Arm Dystonia Disability Scale (ADDS) or Writer's Cramp Rating Scale (WCRS) as well as kinematic analysis of handwriting movements have been used to assess functional impairment in affected patients. In 21 patients with writer's cramp and healthy controls, we analyzed the kinematics of writing and cyclic drawing movements. We rated the severity of dystonia using the ADDS and WCRS and correlated the clinical scores with movement kinematics. Mean stroke frequency was significantly reduced in dystoni

Silas Weir Mitchell's essential tremor.

Silas Weir Mitchell (1829-1914) is recognized as an important American neurologist. Biographers refer in brief to a tremor. The objective of this review was to characterize Mitchell's tremor using handwriting samples, to examine handwriting samples of family members to determine whether this tremor was familial, and study Mitchell's allusions to tremor in personal, scientific, and fictional writings. Primary sources were the Papers of S. Weir Mitchell, College of Physicians of Philadelphia, and Mitchell's scientific and fictional writings. Mitchell's early handwriting was tremor-free yet, by 1

Effects of transcription ability and transcription mode on translation:: Evidence from written compositions, language bursts and pauses when students in grades 4 to 9, with and without persisting dyslexia or dysgraphia, compose by pen or by keyboard.

This study explored the effects of transcription on translation products and processes of adolescent students in grades 4 to 9 with and without persisting specific language disabilities in written language (SLDs-WL). To operationalize transcription ability (handwriting and spelling) and transcription mode (by pen on digital tablet or by standard US keyboard), diagnostic groups contrasting in patterns of transcription ability were compared while composing autobiographical (personal) narratives by handwriting or by keyboarding: Typically developing students (n=15), students with dyslexia (impair

Task-specific interhemispheric hypoconnectivity in writer's cramp - An EEG study.

OBJECTIVE: Writer's cramp (WC) is a focal task-specific dystonia characterized by abnormal posturing of the hand muscles during handwriting, but not during other tasks that involve the same set of muscles and objects such as sharpening a pencil. Our objective was to investigate the pathophysiology underlying the task specificity of this disorder using EEG. We hypothesized that premotor-parietal connectivity will be lower in WC patients specifically during handwriting and motor imagery of handwriting. METHODS: We recruited 15 WC patients and 15 healthy controls. EEG was recorded while particip

Motor functions of higher education students with dysgraphia.

This study examines the motor skills and motor-related daily functions of higher education students with and without dysgraphia, and their contribution in predicting handwriting performance. The sample included 82 higher education students aged 20-35 years old. Thirty-four were students without any known developmental disorder (NDD) and 48 students had dysgraphia. We individually administered a test battery evaluating handwriting performance, fine-motor skills, and visual-motor spatial-organization skills. Students also filled out a questionnaire relating to their fine- and gross-motor-related

Micrographia in Parkinson's Disease: Automatic Recognition through Artificial Intelligence.

BACKGROUND: Parkinson's disease (PD) leads to handwriting abnormalities primarily characterized by micrographia. Whether micrographia manifests early in PD, worsens throughout the disease, and lastly responds to L-Dopa is still under scientific debate. OBJECTIVES: We investigated the onset, progression and L-Dopa responsiveness of micrographia in PD, by applying a non-invasive and cheap tool of artificial intelligence- (AI)-based pen-and-paper handwriting analysis. METHODS: Fifty-seven PD undergoing chronic L-Dopa treatment were enrolled, including 30 early-stage (H&Y ≤ 2) and 27 mid-advance

DysDiTect: Dyslexia Identification Using CNN-Positional-LSTM-Attention Modeling with Chinese Dictation Task.

Handwriting difficulty is a defining feature of Chinese developmental dyslexia (DD) due to the complex structure and dense information contained within compound characters. Despite previous attempts to use deep neural network models to extract handwriting features, the temporal property of writing characters in sequential order during dictation tasks has been neglected. By combining transfer learning of convolutional neural network (CNN) and positional encoding with the temporal-sequential encoding of long short-term memory (LSTM) and attention mechanism, we trained and tested the model with h

In Silico Decoding of Parkinson's: Speech & Writing Analysis.

Background: Parkinson's disease (PD) has transitioned from a rare condition in 1817 to the fastest-growing neurological disorder globally. The significant increase in cases from 2.5 million in 1990 to 6.1 million in 2016, coupled with predictions of a further doubling by 2040, underscores an impending healthcare challenge. This escalation aligns with global demographic shifts, including rising life expectancy and a growing global population. The economic impact, notably in the U.S., reached $51.9 billion in 2017, with projections suggesting a 46% increase by 2037, emphasizing the substantial s

Brief report: macrographia in high-functioning adults with autism spectrum disorder.

The initial description of Asperger syndrome commented on the poor handwriting and motor coordination difficulties of individuals with this condition. Early descriptions of autism do not remark upon such difficulties. Recent evidence, however, suggests that individuals with both conditions have a similar motor control impairment. Handwriting has not been formally assessed in this context. Our study compared handwriting size between individuals with autism spectrum disorder and age- and IQ-matched control subjects. Macrographia was observed among subjects with autism spectrum disorder which rem

Cumulative lead exposure in community-dwelling adults and fine motor function: comparing standard and novel tasks in the VA normative aging study.

BACKGROUND AND AIMS: Lead exposure in children and occupationally exposed adults has been associated with reduced visuomotor and fine motor function. However, associations in environmentally exposed adults remain relatively unexplored. To address this, we examined the association between cumulative lead exposure-as measured by lead in bone-and performance on the grooved pegboard (GP) manual dexterity task, as well as on handwriting tasks using a novel assessment approach, among men in the VA Normative Aging Study (NAS). METHODS: GP testing was done with 362 NAS participants, and handwriting a

Assessment of children's writing features: A pilot method study of pen-grip kinetics and writing surface pressure.

The writing process is a complex task involving dexterous manipulation of the writing instrument by the hand digits and biomechanical ergonomic factors that contribute to handwriting efficiency and productivity. We describe a pilot study using an instrumented writing apparatus - a sensor (pen) and a digitized writing surface (tablet) - to measure the pen-grip kinetics (digit forces) and the pen pressure applied to the tablet during a writing task. Eight elementary school students with no handwriting difficulties copied a short story. The mean digit forces on the pen were compared with the mean
