Health insurance portability and coverage: Evidence from China's cross-regional instant reimbursement reform.

Equitable healthcare access for mobile, informally employed populations remains elusive in many developing contexts. We examine the impact of the first phase of China's cross-regional instant reimbursement (CRIR) reform-a province-level reform-on enrollment in health insurance and healthcare utilization. Informed by a theoretical model, we implement a triple-differences design leveraging CRIR's staggered rollout to identify causal effects. Results show that CRIR substantially increased local UEBMI enrollment (by 8%) and healthcare visits (by 14%), while reducing out-of-pocket spending (by 22%)

Impact of legislative change on waiting time for women accessing surgical abortion services in a rural hospital in the Northern Territory.

BACKGROUND: Women face challenges when accessing abortion, including varied legislation and reduced access to services in rural and remote settings. There are limited clinical guidelines in Australia and little information regarding the patient journey, particularly the timeframe between referral to abortion procedure. Legislation reform in the Northern Territory (NT) legalised early medical abortion (EMA) in primary health care, providing an opportunity to review service provision of elective surgical abortion prior to and after these changes. AIMS: To review the waiting time to access abort

The implementation process of decentralization of health services: a qualitative perspective from Sudan.

BACKGROUND: Health system reform initiatives have increasingly embraced decentralization as a key trend. The implementation process and its outcomes are influenced by a myriad of factors, including economic forces, political dynamics, and ideological factors. Comprehending and carefully examining the implementation phase of decentralization and its consequences to achieve desired outcomes is crucial. Notably, this phase is often considered the weakest aspect of policy reforms, especially in developing countries. Therefore, this study aimed to explore and analyze the implementation phase of dec

Possible objectives and resulting entitlements of essential health care packages.

The notion of a defined 'core package of essential health care services' has appeared in many different health reform proposals in the 1990s. This paper attempts to explore the possible objectives of the 'core package' component of health care reform. Two board applications are apparent: the use of essential packages to ration scarce public funds and the incorporation of a minimum benefit package into 'managed competition' type reforms, where they constitute a mandated minimum level of private insurance cover. Eight possible objectives for an essential benefit package are described: To protect

If we keep doing what we're doing we'll keep getting what we're getting: A need to rethink "academic" medicine.

PURPOSE: For generations there have been warnings of the need to reform medical education at all levels. Today the voices pushing reform are louder, the need is greater, and there is an urgency not seen before. Approaches that have worked in the past to train physicians are no longer as relevant today as demographics, disease patterns, human resources, practice behaviors, technology, and attention to costs demand new collaborative approaches to clinical practice. To prepare for this practice tomorrow's doctors will need a different type of educational model, a different type of learning, in di

Critical Analysis and Exploring Stigma, Legal Barriers, Healthcare Access, and Rights Advocacy Challenges of People Living With HIV in Tunisia.

Our study provides a critical analysis and synthesis of the existing literature, public health data, and policy documents related to HIV stigma, legal barriers, and health care access in Tunisia. It examines complex challenges faced by people living with HIV (PLWH) in Tunisia, focusing on those three areas of concern. We identify persistent stigma and discrimination as critical obstacles, compounded by restrictive legal frameworks and health care access challenges, including workforce shortages and insufficient training. We highlight promising policy and advocacy efforts for improving care and

Czechoslovakia's changing health care system.

Before World War II, Czechoslovakia was among the most developed European countries with an excellent health care system. After the Communist coup d'etat in 1948, the country was forced to adapt its existing health care system to the Soviet model. It was planned and managed by the government, financed by general tax money, operated in a highly centralized, bureaucratic fashion, and provided service at no direct charge at the time of service. In recent years, the health care system had been deteriorating as the health of the people had also been declining. Life expectancy, infant mortality rate

Victorian Mental Health Nurses' Experiences of Mental Health Reform: A Qualitative Descriptive Study.

The state of Victoria, Australia is currently in a period of mental health reform, spurred on by a recent damning Royal Commission and updates to legislation. New legislation promotes the choices and rights of consumers in the mental health system, including recommendations to eliminate seclusion by 2030. While much of this reform has been met with enthusiasm by consumer groups and nurses alike, there remains uncertainty. The aim of this paper is to describe the experiences and perceptions of mental health nurses on mental health reform in the State of Victoria, Australia. Adopting a qualitati

Basaglia: the psychiatrist as intellectual, theoretician, and activist for a dialogic citizen psychiatry.

PURPOSE: Franco Basaglia (1924–1980) was a psychiatrist who played an important role in transforming mental health care in Italy during the 1960s and 1970s. Current mental health debates focus on community care, recovery, and co-production. Against this background, the paper describes theoretical references used by Basaglia and colleagues to psychiatric phenomenology, Jean-Paul Sartre, and Michel Foucault, and the role of psychiatrists as professionals and intellectuals, with a view to their relevance to current mental health care issues. METHODS: The paper is based on a non-systematic search

Charting the path to the implementation of universal health coverage policy in Nigeria through the lens of Delphi methodology.

BACKGROUND: Expanding access to equitable health insurance is an important lever towards the overall strategy for achieving universal health coverage. In Nigeria, health insurance coverage is low with a renewed government action on increasing access to and coverage of high-quality healthcare services to citizens, particularly for the vulnerable and poor population. Therefore, our study co-creates the priorities for expanding health insurance in Nigeria, focusing on key policy reforms, public advocacy, and innovative financing strategies to ensure broader and more equitable coverage for the pop

INTRODUCTION:  Health system reform in Morocco aims to achieve universal health coverage (UHC), primarily through the expansion of health insurance. However, financial coverage does not automatically guarantee physical access to services. This study examines the spatial efficiency of this reform by comparing the geography of financially solvent demand (coverage rate) with that of healthcare supply (infrastructure density). METHOD:  Based on official data from the Ministry of Health (2023) and the Census (2024), this mixed quantitative approach combines inequality analysis us

Promoting access to family medicine in Québec, Canada: Analysis of bill 20, enacted in November 2015.

Primary care can potentially make an important contribution to improving health system performance. However, Canada does not fare as well as other developed countries in terms of timely access to primary health care services. In November 2015, Bill 20 was introduced in the province of Québec. The goal of Bill 20 was to optimize the utilisation of medical and financial resources to improve access to primary care. Bill 20 states the obligations of general practitioners to register a minimum number of patients, ensure the continuity of care of that population, and practice a minimum number of hou

Spain: Health System Review.

This review of the Spanish health system analyses recent developments in health organization and governance, financing, health care provision, recent reforms and health system performance. Overall health status continues to improve in Spain, which presents the highest life expectancy in the European Union - although some socioeconomic inequalities in health persist and risk factors such as overweight, tobacco and alcohol consumption and illegal drug use remain a concern. The Spanish national health system (SNS) provides universal coverage, and it is mainly funded by taxes. Health competences a

Charting A Path Forward For Medicare Advantage: Expert Consensus On Policy Priorities And Trade-Offs.

More than half of Medicare beneficiaries are enrolled in Medicare Advantage (MA), yet concerns persist about overpayments to MA plans, inadequate plan oversight, and limits on beneficiaries' access to care. To identify MA reform options, we conducted a two-round e-Delphi panel with thirty-nine experts, including researchers, industry representatives, patient advocates, and former policy makers. Panelists rated fifteen policy reform ideas on their impact on beneficiaries, the federal government, and MA plans. Experts agreed that six policy ideas would benefit beneficiaries, three would benefit

Concussion and Chronic Traumatic Encephalopathy Deaths: Coroners' Inquests as a Catalyst for Public Health Reforms.

Deaths of participants in sport from the effects of concussive injuries and from chronic traumatic encephalopathy (CTE) raise confronting social issues and challenges for tort law. An uncertainty that often needs to be addressed in such cases is proof of the causes of the former athlete's symptomatology, especially when they may be multifactorial, some or all of which were not directly related to sport. Accounts from the person prior to their death and from family members can be vital sources of such information. Coroners' analyses of evidence in concussion-related deaths constitute an importa
