Timely follow-up among multicultural women with abnormal mammograms.

OBJECTIVE: To examine factors influencing time from screening to final diagnosis among multicultural women with abnormal mammograms using the precede-proceed model. METHODS: Staff of 58 clinics and a sample of 436 women served by these clinics were interviewed and their medical records examined. RESULTS: Longer duration from screening to diagnosis was associated with speaking Spanish and having clinic staff make appointments. Ease of access to health care, provision of early morning screening services and higher levels of patient anxiety shortened the time to diagnosis. CONCLUSION: The prec

Suicide in Older Adult Men Is Not Related to a Personal History of Participation in Football.

Introduction: It is reasonable to estimate that tens of millions of men in the United States played high school football. There is societal concern that participation in football confers risk for later-in-life mental health problems. The purpose of this study is to examine whether there is an association between a personal history of playing high school football and death by suicide. Methods: The subjects were obtained from the Lieber Institute for Brain Development (LIBD) brain donation program in collaboration with the Office of the Medical Examiner at Western Michigan University Homer Stryk

Cutaneous leishmaniasis in British troops following jungle training in Belize: Cumulative incidence and potential risk practices.

BACKGROUND: British soldiers undergoing jungle training in Belize typically experience a relatively low risk of developing cutaneous leishmaniasis. However, an uncharacteristically large outbreak of cutaneous leishmaniasis occurred in 2022. This study aimed to determine the cumulative incidence of the disease and highlight potential shortcomings in personal protective measures to mitigate exposure to sand fly vector bites. METHODS: A retrospective analysis was conducted on medical records of cutaneous leishmaniasis cases between 2005 and 2022, as well as on questionnaire responses regarding p

Through the Narrative Looking Glass: Commentary on "Impact of Electronic Health Records on Information Practices in Mental Health Contexts: Scoping Review".

The authors of "Impact of Electronic Health Records on Information Practices in Mental Health Contexts: Scoping Review" have effectively brought to our attention the failure of the electronic health record (EHR) to represent the human context. Because mental health or behavioral disorders (and functional status in general) emerge from an interaction between the individual's characteristics and the social context, it is essentially a failure to represent the human context. The assessment and treatment of these disorders must reflect how the person lives, their degree of social connectedness, th

The Lifecycle of Electronic Health Record Data in HIV-Related Big Data Studies: Qualitative Study of Bias Instances and Potential Opportunities for Minimization.

BACKGROUND: Electronic health record (EHR) data are widely used in public health research, including in HIV-related studies, but are limited by potential bias due to incomplete and inaccurate information, lack of generalizability, and lack of representativeness. OBJECTIVE: This study explores how workflow processes among HIV health care providers (HCPs), data scientists, and state health department professionals may potentially introduce or minimize bias within EHR data. METHODS: One focus group with 3 health department professionals working in HIV surveillance and 16 in-depth interviews (ie

Internal Medicine Residents' Experience Performing Routine Assessment of What Matters Most to Patients Upon Hospital Admission.

PROBLEM: Failure to elicit patients' values, goals, and priorities can result in missed opportunities to provide patient-centered care. Little is known about resident physicians' direct experience of eliciting patients' values, goals, and priorities and integrating them into routine hospital care. INTERVENTION: In 2017, we asked resident physicians on general internal medicine wards rotations to elicit and document a "Personal History" from patients upon hospital admission, in addition to a traditional social history. We defined a Personal History as documenting "what matters most to the pati

Identifying and addressing health problems of school children through the Omaha System: a nurse-led intervention study in suburban Istanbul.

BACKGROUND: Health problems such as anxiety, depression, obesity, and dental caries are increasingly common among school-aged children. Early detection and timely interventions are essential to promote healthy childhoods and reduce future healthcare burdens. Especially in Turkey, few studies have addressed schoolchildren’s health problems holistically, particularly using the Omaha System. This study aimed to identify the physiological, psychosocial, health-related behavioral, and environmental health problems of migrant and non-migrant schoolchildren in suburban Istanbul, to implement person-c

A geospatial dataset of PPCPs occurrence in aquatic environments across China over the past two decades.

Pharmaceuticals and personal care products (PPCPs) are increasingly recognized as persistent and bioaccumulative contaminants in aquatic environments. To enhance understanding of their spatial distribution and prevalence across China, we have developed a comprehensive geospatial database containing 8,128 records of 250 PPCPs from 385 sampling sites nationwide. The dataset is organized at four geographic scales (i.e., provincial, prefectural, county, and township or finer), with each record detailing the sampling time, location, and measured concentrations. Importantly, this database expands on

The Swedish medical birth register during five decades: documentation of the content and quality of the register.

Pregnancy-related factors are important for short- and long-term health in mothers and offspring. The nationwide population-based Swedish Medical Birth Register (MBR) was established in 1973. The present study describes the content and quality of the MBR, using original MBR data, Swedish-language and international publications based on the MBR.The MBR includes around 98% of all births in Sweden. From 1982 onwards, the MBR is based on prospectively recorded information in standardized antenatal, obstetric, and neonatal records. When the mother and infant are discharged from hospital, this infor

Electronic patient messages to promote colorectal cancer screening: a randomized controlled trial.

BACKGROUND: Colorectal cancer is a leading cause of cancer mortality, yet effective screening tests are often underused. Electronic patient messages and personalized risk assessments delivered via an electronic personal health record could increase screening rates. METHODS: We conducted a randomized controlled trial in 14 ambulatory health centers involving 1103 patients ranging in age from 50 to 75 years with an active electronic personal health record who were overdue for colorectal cancer screening. Patients were randomly assigned to receive a single electronic message highlighting overdue

Developing an operational definition of housing instability and homelessness in Veterans Health Administration's medical records.

The main objective of this study was to examine how homelessness and housing instability is captured across data sources in the Veterans Health Administration (VHA). Data from 2021 were extracted from three data repositories, including the Corporate Data Warehouse (CDW), the Homeless Operations Management System (HOMES), and the Homeless Management Information System (HMIS). Using these three data sources, we identified the number of homeless and unstably housed veterans across a variety of indicators. The results showed that the use of diagnostic codes and clinic stop codes identified a large

Ethnic Minorities' Experiences of Cardiac Rehabilitation: A Scoping Review.

Cardiac rehabilitation (CR) can improve cardiovascular risk factors, decrease cardiac mortality, and promote healthy lifestyle behaviours. However, services remain underutilized by groups of ethnic minorities. The purpose of the study was to identify patients' personal CR experiences to identify the differences CR makes towards minorities' lifestyle. An initial electronic search was performed in 2021 for papers ranging from 2008-2020 across specific databases, including PubMed, EMBASE, APA PsycINFO, CINAHL (Cumulative Index to Nursing and Allied Health Literature), and Medline.
