An evaluation of paediatric tinnitus services in UK National Health Service audiology departments.

BACKGROUND: Whilst evidence indicates many children experience troublesome tinnitus, specialist services for children are far less established than those available for adults. To date, there is limited understanding of how paediatric tinnitus is managed in the UK, and to what extent current practice reflects what is recommended. This service evaluation aimed to 1) profile how tinnitus in children is managed in UK clinical practice, and assess to what extent care provided by services reflects advice included in the British Society of Audiology (BSA) Tinnitus in Children Practice Guidance, 2) co

Communicating about sexual concerns within cardiac health services: do service providers and service users agree?

OBJECTIVE: Sexual assessment and counselling is a recommended, yet underprovided and challenging, aspect of cardiac rehabilitation. We compared the views of cardiac healthcare providers and patients in terms of their experiences of communication about sexual issues. METHODS: Cardiac patients (n=382) completed telephone surveys and hospital cardiac rehabilitation staff (n=60) and general practitioners (n=61) returned postal questionnaires. RESULTS: Patients reported that sex was rarely discussed, yet nearly half of patients said they would have liked this opportunity. Most general practitione

New approaches to sexual health services in a rural health board area: involving service users and primary care professionals.

Research was undertaken to inform the future development of sexual health services for the population of a rural health board. Three separate surveys were carried out: (i) users of local and regional genitourinary medicine (GUM) services, (ii) users of local family planning clinics, and (iii) all local general practitioners (GPs) and practice nurses-to obtain views on current sexual health services and the optimum future configuration of services. Respondents to the GUM survey (n=137) attended GUM service for a range of sexual health reasons; confidentiality was the most important factor affec

Suicidal ideation, mental health services, and barriers to care among syringe services program clients who use opioids.

INTRODUCTION: People who use opioids (PWUO) experience a disproportionately high burden of suicidal risk. Syringe services programs (SSPs) are a critical point of contact for this population, providing low-threshold, harm-reduction services. However, gaps persist in understanding suicidal risk among individuals attending SSPs and the extent to which such risk is associated with mental health service utilization in this setting. This study aimed to assess the prevalence and specific risk factors for suicidal ideation (SI) in PWUO, as well as their use of and barriers to accessing mental health

RESULTS: Although 61% had a gastroenterologist, when experiencing active symptoms, only 29% felt they could call their gastroenterologist for an urgent appointment, and 42% could call their gastr

Preconception health policy, health promotion, and health services to achieve health in current and future generations: a narrative review.

Non-communicable diseases (NCDs) represent the leading cause of death and disability worldwide, and those NCDs contributing most burden - cardiometabolic illness, diabetes, cancer, chronic respiratory disease - can be largely prevented through improvements in health behaviours such as tobacco use, harmful use of alcohol, exposure to environmental hazards, unhealthy dietary habits, and physical inactivity. Despite concerted efforts at all levels of health care provision and policy, population-level health behaviour change still presents an ongoing challenge to primary care clinicians, public he

Health facility capacities for providing adolescent sexual and reproductive health services in Southwestern Uganda.

BACKGROUND: Provision of adolescent-friendly Health services (AFHS) at lower primary health care centres (PHCs) is a core strategy for improving access and health outcomes of adolescents. In this study we assessed the health facility-based prevalence of repeat adolescent pregnancies and the capacity of PHCs (Health centres (HC) level III and IV) to provide AFHS in Uganda. METHODS: We conducted a cross-sectional study between April and May 2023 involving 43 healthcare facilities (33 HC III and 10 HC IV) across 4 districts in Southwestern Uganda. Using pretested structured questionnaires, we in

Understanding Engagement With Digital Mental Health Technology in Mental Health Services: Multicenter Observational Study.

BACKGROUND: Digital technologies can substantially improve mental health care by facilitating measurement-based care through routine outcome monitoring. However, their effectiveness is constrained by the extent to which these technologies are used by services, clinicians, and clients. OBJECTIVE: This study aims to investigate engagement with the Innowell platform, a measurement-based digital mental health technology (DMHT), to gain insights into the individual- and service-level factors influencing engagement. METHODS: Participants were 2682 help-seeking clients from 12 Australian mental hea

Military veterans and civilians' mental health diagnoses: an analysis of secondary mental health services.

PURPOSE: Healthcare provision in the United Kingdom (UK) falls primarily to the National Health Service (NHS) which is free at the point of access. In the UK, there is currently no national marker to identify military veterans in electronic health records, nor a requirement to record it. This study aimed to compare the sociodemographic characteristics and recorded mental health diagnoses of a sample of veterans and civilians accessing secondary mental health services. METHODS: The Military Service Identification Tool, a machine learning computer tool, was employed to identify veterans and civ

Tracking progress towards universal health coverage for essential health services in China, 2008-2018.

INTRODUCTION: We comprehensively evaluate whether the Chinese Government's goal of ensuring Universal Health Coverage for essential health services has been achieved. METHODS: We used data from the 2008, 2013 and 2018 National Health Services Survey to report on the coverage of a range of Sustainable Development Goals (SDG) indicator 3.8.1. We created per capita household income deciles for urban and rural samples separately. We report time trends in coverage and the slope index (SII) and relative index (RII). RESULTS: Despite much lower levels of income and education, rural populations made

Exploring barriers and facilitators in primary oral health services: step towards Universal Health Coverage.

INTRODUCTION: Despite the Alma-Ata Declaration's emphasis on primary health care (PHC) to achieve universal health coverage (UHC), oral diseases remain prevalent and costly. In Iran, health reforms have expanded PHC, yet oral health services lag. This study explored barriers and facilitators of primary oral health service delivery in Tehran. METHODS: This qualitative study employed semi-structured, face-to-face interviews with dentists, heads of comprehensive health centers (CHCs), healthcare providers and visitors to the health centers in Tehran from November 2021 to January 2022. Predicting anxiety treatment outcome in community mental health services using linked health administrative data.

Anxiety disorders is ranked as the most common class of mental illness disorders globally, affecting hundreds of millions of people and significantly impacting daily life. Developing reliable predictive models for anxiety treatment outcomes holds immense potential to help guide the development of personalised care, optimise resource allocation and improve patient outcomes. This research investigates whether community mental health treatment for anxiety disorder is associated with reliable changes in Kessler psychological distress scale (K10) scores and whether pre-treatment K10 scores and past

Adapting Community Health Worker Care Models to Advance Mental Health Services Among LGBTQ Youth.

Lesbian, gay, bisexual, transgender, and queer (LGBTQ) youth of color experience high rates of mental health disorders, yet they experience challenges to accessing mental health services. Community health worker (CHW) models of care have potential to promote equitable mental health services among LGBTQ youth. Our aim was to understand how CHW models could be adapted to better support LGBTQ youth of color in accessing mental health services. Semi-structured qualitative interviews were conducted with LGBTQ youth of color (n = 16), caregivers of LGBTQ youth (n = 11), and CHWs (n = 15) in Massachu

Women's Health Care in the Deployed Setting 2013-2020: A Health Services Research Approach.

INTRODUCTION: With the management and oversight of MTFs moving under the authority of the Defense Health Agency, coupled with a careful examination of the composition of uniformed medical personnel, it is imperative to ensure that active duty servicewomen who are in deployed settings receive timely, appropriate, and quality health care. This study sought to examine the amount and types of gynecological and obstetric care provided in the deployed setting and to examine that data by the socioeconomic and demographic characteristics of the women receiving that care.
