Midlife in crisis? Life course pathways to health at midlife across U.S. cohorts.

OBJECTIVES: Recent trends of increasing midlife morbidity and mortality and widening educational disparities in the United States are concerning. This study documented the age pattern and educational disparities in health in the National Longitudinal Study of Adolescent to Adult Health (Add Health) cohort as they enter midlife, and situates the cohort relative to preceding, similar, and following cohorts in the National Health and Nutrition Examination Survey (NHANES). METHODS: Using data from Add Health and NHANES on 6 birth cohorts observed from adolescence to middle age, we characterized a

Prevalence, trends and inequalities of childhood undernutrition according to a Composite Index of Anthropometric Failure (CIAF): a systematic review and meta-analysis.

BACKGROUND: The Composite Index of Anthropometric Failure (CIAF) and its extensions-the Composite Index of Severe Anthropometric Failure (CISAF) and extended CIAF (eCIAF)-capture overlapping forms of child undernutrition but remain under-reported at global and regional levels. We aimed to estimate the prevalence of CIAF, CISAF and eCIAF among children under 5 years and assess inequalities by sex, residence, household wealth and maternal education. METHODS: We systematically searched major databases, including Embase, MEDLINE (PubMed), Scopus, CINAHL, ProQuest, Global Index Medicus and the Coc

Declining trends but persistent disparities in cervical cancer mortality in the United States, 1999-2023.

BACKGROUND: Cervical cancer is largely preventable through effective screening and human papillomavirus (HPV) vaccination; however, mortality disparities remain a persistent public health challenge in the United States. This study aimed to examine long-term trends and socio-demographic and geographic inequalities in cervical cancer mortality. METHODS: We conducted a population-based study of cervical cancer mortality among U.S. women aged ≥25 years from 1999 to 2023 using national mortality data from the CDC WONDER database. Age-adjusted mortality rates (AAMRs) were calculated using the 2000

Racial disparities in multiple myeloma: biological heterogeneity, treatment access, and prognostic implications.

Unlike the well-recognized two-fold higher incidence rate and an earlier age of onset of MM among African Americans (AA) compared to European Americans (EA) in the United States, findings on racial disparities in survival outcomes of MM are inconclusive, ranging from worse survival, to no difference, to better overall survival in AA patients than EA patients. The clinical outcomes for patients with MM depend on a complex interplay of factors including age, disease severity, cytogenetics, treatment modalities, biological features of the responsible clone, and non-biological factors such as trea

Bridging the Gap of Mental Health Inequalities in the Transgender Population: The Role of Nursing Education.

The transgender population experience mental health disparities as a result of numerous environmental, sociocultural and personal factors. Health care-related discrimination plays an unfortunate role in these disparities. The institutional and informational erasure of transgender individuals in health care actively contributes to this discrimination. The lack of preparation of health care professionals to care for this population is a significant factor. There is an absence of education on transgender topics in nursing school and a lack of nursing research on this population. An awareness thro

The concept of environmental injustice in allergy and asthma.

PURPOSE OF REVIEW: This review examines how environmental injustice contributes to disparities in allergic diseases, including asthma, allergic rhinitis, and atopic dermatitis. It focuses on the role of social, geographic, and economic inequities in shaping disease prevalence, severity, and access to care among underrepresented racial and ethnic communities. RECENT FINDINGS: Recent studies emphasize the persistent exposure of low-income and racially marginalized populations to environmental hazards such as air pollution, substandard housing, and climate-related changes, factors that are linke

Understanding Racial Disparities in Prostate Cancer: A Multifaceted Approach.

Prostate cancer (PCa) remains a significant public health challenge in the United States, disproportionately affecting African American (AA) men, who face higher incidence rates, more aggressive disease, and elevated mortality compared to Caucasian American (CA) men. This review explores the multifactorial underpinnings of these disparities, integrating genomic, socioeconomic, environmental, and systemic contributors. Genomic analyses reveal that AA men harbor distinct molecular alterations, including higher frequencies of FOXA1, BRAF, and CHD1 mutations, as well as DNA damage repair defects,

Health disparities in food allergy.

Rates of food allergy are rising worldwide, with a disproportionate amount of disease burden found in patients of Black, Asian, and Latinx descent. Recent guidelines have recommended early introduction of allergens as early as 4-months-old to mitigate the development of food allergy. However, non-White children continue to have significantly poorer outcomes with higher rates of food allergy-related emergency department visits for anaphylaxis without having equivalent access to epinephrine auto-injectors. With only half of allergists accepting Medicaid in the United States, and only one-third o

Addressing Health Disparities: How Having a More Diverse Biomedical Workforce Can Contribute to Addressing Health Disparities in Communities that Are Often Underrepresented in the Healthcare System.

Health disparities that are seen in underserved and underrepresented communities are a pressing issue in healthcare. These disparities are embedded into our society through structural inequalities that lead to poorer health outcomes in those from minoritised communities. In its place as the heart of modern healthcare, the biomedical science workforce has the potential to play a crucial role in mitigating these disparities by fostering greater cultural competence, improving patient outcomes and driving innovative solutions. This study reviewed the current literature on the impact of diversity w

Racial and Socioeconomic Health Disparities in Peripheral Artery Disease.

Peripheral artery disease (PAD) is a progressive atherosclerotic disease that causes lower extremity arterial stenosis or occlusion. Patients with PAD are at increased risk of myocardial infarction, stroke, limitations in ambulation, and amputation. Despite the advances in medicine and technology, the outcomes from PAD, including critical limb-threatening ischemia, acute limb ischemia amputation, and mortality, remain increased among specific racial and ethnic groups that have been historically marginalized in America, including Black, Hispanic, and American Indian individuals in the United St

The Urban Dental Index: a Method for Measuring and Mapping Dental Health Disparities across Urban Areas.

The aim of this study was to create an Urban Dental Index (UDI) for Perth, Western Australia, adapting a method utilised by the World Health Organisation. Dental health indicators were derived from the 2011 census, standardised on a (0,1) interval, amalgamated using a geometric mean, and mapped to identify dental health inequity. The validity of the UDI was tested by comparison with oral cellulitis data. Dental health disparities were examined using a ratio of the mean of the highest to lowest decile and slope of the eight middle deciles. The robustness of the UDI was tested using indicator co

Rural-urban disparities in population practising open defecation across 47 African countries: a secondary analysis using the WHO health equity assessment toolkit.

Access to safe sanitation is a human right, yet millions in Africa practice open defecation, risking disease and environmental harm. Despite global efforts, rural-urban disparities remain, especially in low-income countries. This study examines these disparities to inform policies for equitable sanitation access. To assess rural-urban disparities in open defecation across 47 African countries using WHO and HEAT data, identifying patterns for equitable sanitation policies and SDG 6 progress. This study assessed disparities using WHO and HEAT data, focusing on open defecation prevalence, defined

Health Inequities in Pancreatic Disorders in the Black/African American Community.

Pancreatic diseases, such as acute pancreatitis (AP), chronic pancreatitis (CP), and pancreatic ductal adenocarcinoma (PDAC), disproportionately affect Black/African American (AA) communities in the United States, leading to high incidence, prevalence, and mortality rates. This paper outlines disparities in pancreatic diseases among AAs and explores contributing factors beyond individual biology and behavior, emphasizing the role of social determinants of health (SDoH), including poor access to healthcare, lack of inclusion in research studies, and other crucial systemic and structural factors

Chronic respiratory disease in Indigenous peoples: a framework to address inequity and strengthen respiratory health and health care globally.

Indigenous peoples around the world bear a disproportionate burden of chronic respiratory diseases, which are associated with increased risks of morbidity and mortality. Despite the imperative to address global inequity, research focused on strengthening respiratory health in Indigenous peoples is lacking, particularly in low-income and middle-income countries. Drivers of the increased rates and severity of chronic respiratory diseases in Indigenous peoples include a high prevalence of risk factors (eg, prematurity, low birthweight, poor nutrition, air pollution, high burden of infections, and

Cross-country inequalities and frontier analysis in global burden of cervical cancer: cross-sectional studies.

OBJECTIVE: This research evaluated global health disparities of cervical cancer, and forecasted trends from 2022 to 2040. METHODS: Cross-country inequalities [Slope Index of Inequality (SII) and the Concentration Index] are used to measure inequality. Frontier analysis evaluates the medical quality by calculating the difference between actual values and frontier values. Average annual percentage changes (AAPCs) were computed to discern trends from 2022 to 2040. RESULTS: The global SII for the age-standardized death rate (ASDR) of cervical cancer was -9.14 per 100,000 in 1990 and -9.62 in 202

Inequalities in healthy life expectancy by social class and area type: England, 2001-03.

BACKGROUND: Healthy life expectancy (HLE) is an important indicator which combines longevity with health status. This article examines inequalities in HLE by socio-economic position in England, and between Local Authorities (LAs) in the deprived 'Spearhead group' and other LAs. METHODS: Census and vital event data available from the ONS Longitudinal Study were used to calculate estimates of HLE based on general health status for each Registrar General's Social Class (RGSC) in 2001-03, in England as a whole and within the 'Spearhead group' and non-Spearhead LAs. RESULTS: A predominantly linea

How to Talk with Patients about Incarceration and Health.

The United States has the highest incarceration rate of any nation in the world-more than 700 people per 100,000. For this reason alone, clinicians practicing in the US should be aware of the numerous ways in which incarceration adversely affects the health of individuals, their families, and communities. While we clinicians are taught how to discuss ways that culture, religion, or sexuality can affect health outcomes, we are not instructed on how to talk about incarceration history with patients when it might be affecting their health, as highlighted in the case scenario. Here I present a "st

A Review of Disparities in Cardiac Rehabilitation: EVIDENCE, DRIVERS, AND SOLUTIONS.

PURPOSE: Cardiac rehabilitation (CR) has been shown to improve functional status, quality of life, and recurrent cardiovascular disease (CVD) events. Despite its demonstrated compelling benefits and guideline recommendation, CR is underutilized, and there are significant disparities in CR utilization particularly by race, ethnicity, sex, and socioeconomic status. The purpose of this review is to summarize the evidence and drivers of these disparities and recommend potential solutions. METHODS: In this review, key studies documenting disparities in CR referrals, enrollment, and completion are
