Unexpected Severe Cyanosis Following Ventricular Septal Defect Closure and Repair of Right Ventricular Outflow Tract Obstruction.

Ventricular septal defect (VSD) is a common congenital heart defect that often requires surgical closure. We present the case of a seven-month-old infant with a large perimembranous VSD, moderate pulmonary valve stenosis, and hypertrophied right ventricular muscle bundles who failed to wean from cardiopulmonary bypass due to severe cyanosis after VSD closure and muscle bundle resection. An intraoperative transesophageal echocardiogram was performed with an agitated saline test, and a large atrial-level right-to-left shunt was seen. Return to cardiopulmonary bypass revealed an unroofed coronary

Coronary bypass using bilateral internal mammary arteries in an achondroplast.

Coronary bypass grafting for ischemic heart disease in achondroplastic dwarfs is very rare. Shortage of veins and inadequate vein quality may cause difficulties during surgery. Only 2 cases of coronary bypass surgery in an achondroplastic dwarf, in which the left internal mammary artery and vein grafts were used, have been reported. We describe the case of a 55-year-old male achondroplastic dwarf who had triple-vessel coronary disease and underwent successful coronary bypass surgery using one saphenous vein graft and bilateral internal mammary artery grafts. The anatomic and surgical challenge

Reoperative cardiac surgery using a new femoral venous right atrial cannula.

A new ultra-thin but nonkinkable catheter was used for transfemoral venous right atrial drainage for cardiopulmonary bypass in 35 patients undergoing reoperations for coronary artery disease (CAD) (14), valve replacement (16), combined valve and CAD (4), and left ventricular aneurysm resection (1). Adequate flow rates were obtained with these cannula of 25, 27, and 29 French diameter in all patients. This technique provided excellent drainage of the heart allowing for a decompressed heart to dissect with no bleeding or damage to patent obstructed cardiopulmonary bypass grafts. There was no mor

Left ventricle after palliation of hypoplastic left heart syndrome: friend, fiend, or innocent bystander?

Hypoplasia of the left side of the heart is the most common cause of death from congenital heart disease in the first weeks of life. Once considered a surgically fatal disease, hypoplasia has been successfully palliated for more than 30 years. Although the palliative route is staged by an early differential bypass of the systemic outflow and the venous inflow to the right ventricle, the left ventricle remains anatomically and biologically influential throughout. Given the variation of the left ventricle, contemporary outcomes for different hypoplastic left heart subsets can vary both early aft

Left atrial infarction: a case report and review of the literature.

The majority of cardiac related deaths are due to ischemic heart disease, with the most common clinical scenario being severe coronary artery atherosclerosis resulting in left ventricular myocardial infarction. However, infarction of other cardiac chambers does occur, and often has specific clinical associations. We report a case of a 70-year-old man who suffered from left atrial infarction that resulted in a transmural rupture of his left atrium. The patient had a history of rheumatic heart disease, mitral valve stenosis, and severe atherosclerotic coronary artery disease. Four days before de

[Mitral Valve Surgery for a Patient with Mediastinal Shift after Left Pneumonectomy:Report of a Case].

A 70-year-old patient who survived about 40 years after left pneumonectomy for tuberculosis visited emergency hospital, because of dyspnea. She received suitable medical therapy for atirial fibrillation and severe mitral regurgitation and hesitated heart surgery because of anxiety for surgical risk. The computed-tomography showed mediastinal shift to left and right lung compensatory expansion. Respiratory function test after treatment of heart failure showed only mild restrictive disorder. And the blood-gas examination in room air was 101 mmHg of Pao2 and 37 mmHg of Paco2. The mitral valve rep

Cardiac function and myocardial performance of 24-hour-preserved asphyxiated canine hearts.

A method of 24-hour storage of asphyxiated canine hearts for orthotopic cardiac transplantation was studied to expand the geographical size of the donor pool. Left ventricular function of asphyxiated hearts preserved for 24 hours (group 1, n = 8) was compared with that of hearts donated on-site (group 2, n = 5). Group 1 donors were pretreated with verapamil hydrochloride, propranolol hydrochloride, and prostacyclin. The donor hearts were perfused with warm blood cardioplegia in situ after 10 minutes of asphyxiation and then perfused with cold crystalloid cardioplegia for 2 hours. The hearts we

Coronary subclavian steal syndrome: an extracoronary cause of acute coronary syndrome.

Significant atherosclerotic changes of the coronary arteries are the common cause of cardiac chest pain. We report the case of an 80-year-old woman suffering from unstable angina caused by extracoronary atherosclerosis. The patient had an extensive medical history with severe coronary heart disease and cardiac bypass surgery (LIMA to LAD, two venous bypass grafts). An urgent coronary angiography was performed. The angiogram displayed the already known three-vessel disease, the bypass grafts were in a good functional condition. Subsequently, a stenosis of the proximal segment of the left subcla

Aortic Valve Stenosis and Left Main Coronary Disease: Hybrid Approach.

We describe a technique of combined transcatheter aortic valve replacement (TAVR), off-pump single coronary artery bypass, and percutaneous coronary intervention (PCI) in a high-risk patient presenting with unstable angina and severe heart failure. This patient had documented moderate to severe aortic stenosis, left ventricular dysfunction, and a heavily calcified ascending aorta. A robotic-assisted left internal thoracic artery harvesting was aborted owing to inability to tolerate single-lung ventilation. A median sternotomy was done, then successful off-pump single-vessel bypass, PCI, and TA

[Two successful surgical cases of total left anomalous pulmonary venous connection with intact atrial septum--a report on their operative procedures and approaches].

Partial anomalous pulmonary venous connection (PAPVC) is often associated with other congenital heart diseases. Atrial septal defect, particularly, appears in about ninety percent of patients with PAPVC. Successful repairs were made in two patients of total left PAPVC with intact atrial septum. In one patient, left thoracotomy was made, and the vertical vein was anastomosed to the left atrial appendage. Cardiopulmonary bypass was not used in this case. In the other patient, median sternotomy was made, and the vertical vein was anastomosed to the posterior wall of the left atrium under cardiopu

Neuroprotective Drugs in Infants With Severe Congenital Heart Disease: A Systematic Review.

Background: Perinatal and perioperative brain injury is a fundamental problem in infants with severe congenital heart disease undergoing neonatal cardiac surgery with cardiopulmonary bypass. An impaired neuromotor and neurocognitive development is encountered and associated with a reduction in quality of life. New neuroprotective drugs during surgery are described to reduce brain injury and improve neurodevelopmental outcome. Therefore, our aim was to provide a systematic review and best-evidence synthesis on the effects of neuroprotective drugs on brain injury and neurodevelopmental outcome i

The risks of reoperative coronary artery bypass in chronic ischemic cardiomyopathy: results of the CABG Patch Trial.

BACKGROUND: Coronary artery bypass grafting in patients with chronic ischemic cardiomyopathy is increasing in frequency, as is reoperative bypass. However, there exist very little data on early outcomes of patients with severe left ventricular dysfunction undergoing reoperation. Hence, we tested the following hypotheses: (1) in the presence of severe left ventricular dysfunction, repeat coronary bypass carries a higher surgical mortality than the primary operation and (2) among reoperative patients with left ventricular dysfunction, the surgical mortality is higher in those with the lowest pre

[Coronary artery bypass grafting under beating heart without cardiopulmonary bypass].

Recently coronary artery bypass grafting under beating heart without cardiopulmonary bypass has been widely accepted for patients with associated risks of standard bypass surgery. Between October 1995 and October 1997, we performed this procedure on 40 (11 reoperative procedures). There were 26 single-vessel (9 reoperative procedures), 12 double-vessel (2 reoperative procedures) and 2 triple-vessel bypass procedures. Major risk factors and concomitant disorders were as follows: chronic hemodialysis, 3; poor renal function, 5; poor lung function, 6; reoperative procedure, 11; elder patient (age

Off-pump coronary artery bypass grafting for a left main lesion due to cardiac allograft vasculopathy in Japan: first report of a case.

Cardiac allograft vasculopathy (CAV) is a major cause of mortality after transplantation. We treated a 44-year-old female with off-pump coronary artery bypass grafting (OPCAB) 4 years after heart transplantation. Annual examinations, including coronary angiography and intravenous ultrasound (IVUS), revealed a severe lesion in the left main trunk. The left internal mammary artery was successfully anastomosed to the left anterior descending artery in an off-pump manner. To ensure that patients have a good long-term outcome after heart transplantation, routine examinations, including IVUS, are cr

[Surgical treatment of left ventricular-right atrial communication].

Four cases of congenital left ventricular-right atrial communication were presented. Correct diagnoses were made in two cases before operation by left ventriculography. The other two cases were misdiagnosed as ASD or VSD by right heart catheterization and ventriculography. After surgical repair under cardiopulmonary bypass, they were all cured and discharged. During the follow-up period, one patient with residual leak was reoperated upon and died on the sixteenth postoperative day from arrhythmia and heart failure. The other three patients secured complete restitution.

[Anesthesic management in telerobotic totally endoscopic coronary artery bypass grafting].

OBJECTIVE: To investigate the strategies for anesthetic management for totally endoscopic coronary artery bypass grafting with the da Vinci S surgical system. METHODS: Between January 2007 and May 2009, 16 patients underwent off-pump coronary artery bypass grafting via a totally endoscopic approach. After anesthetic induction, a left-sided double-lumen endotracheal tube was positioned to allow single right-lung ventilation during intra-operative procedure. The left internal mammary artery graft was harvested and anastomosed with the anterior descending branch with robotic assistance. The hemo

Congenital right pulmonary artery-to-left atrial fistula.

A 23-year-old man presented with cyanosis since birth. Precordial echocardiography failed to detect any structural heart abnormality except for enlarged left atrium. Pulmonary angiography revealed a type I right pulmonary artery-to-left atrial fistula. Simple ligation of the fistula without cardiopulmonary bypass was performed successfully, and the cyanosis disappeared postoperatively.
