Chin-sternum-heart syndrome type of injury observed in a pedestrian victim of car traffic accident.

We report an autopsy case of a pedestrian victim of car traffic accident with the chin-sternum-heart syndrome type of injury. A drunken man who lay on the road was run over by a car. He died immediately at the scene. The autopsy findings were as follows: large scalp lacerations, abrasions in the chin and the sternal region, a transverse fracture of the sternum, ruptures of the heart, ruptures of the ascending and descending aortae, rupture and hemorrhage of the nuchal muscle, ring fracture of the base of the skull, subarachnoid hemorrhage at the base of the brain, multiple rib fractures, anter

Sudden death due to spontaneous rupture of the right atrium in the setting of chronic lung disease: A case report and review.

Sudden death from haemopericardium as a result of a right atrial rupture is uncommon, most particularly when this occurs spontaneously without any prior trauma or evidence of atrial wall pathology. The deceased was a 32-year-old man. At lunchtime his symptoms of unease, giddiness and unconsciousness began and within 45 minutes he arrived at the hospital. His vitals could not be recorded in the emergency department, and after CPR, he was pronounced dead. At autopsy, an isolated right atrial rupture, without any disease of the heart wall, was discovered. The right atrium has the weakest wall and

A rare cause of dyspnea: sudden rupture of aortic valsalva sinus aneurysm.

Aneurysm of the sinus of Valsalva is an uncommon cardiac abnormality; however, the most common complication is rupture into the right heart chambers or rarely towards the left chambers. A ruptured aneurysm typically leads to an aortocardiac shunt and progressively worsening heart failure. We report a case of a 21-year-old male who suffered an aneurysm of the sinus of Valsalva rupture into the right atrium who underwent successful surgical repair.

Left Ventricular Outflow Tract Rupture in a Case of Pediatric Bicuspid Aortic Valve Endocarditis.

BACKGROUND: Most pediatric infectious endocarditis (IE) cases occur in children with congenital heart disease. Acute left ventricular outflow tract rupture is a rare complication of bacterial IE. CASE SUMMARY: An 8-year-old boy with a bicuspid aortic valve and no surgical history was diagnosed with IE. He was found to have an acute contained rupture of the left ventricular outflow tract that required emergent surgical intervention. DISCUSSION: This case highlights the importance of maintaining a high index of suspicion for cardiac rupture in children with IE when there is persistence or deve

[Ventricular Free Wall Rupture].

Ventricular free wall rupture is an infrequent complication after acute myocardial infarction. However, it carries a high mortality rate. More than moderate amount of pericardial effusion (≥10 mm) is associated with an increased risk of free wall rupture. Surgery is superior to conservative treatment. Recently sutureless techniques, avoiding cardiopulmonary bypass, demonstrated relatively good survival rates.

Cardiac laceration following non-penetrating chest trauma in dog and cat.

Cardiac laceration with non-penetrating chest trauma is reported as a common cause of death in human following rapid deceleration in high-speed vehicular accident. In contrast, in veterinary medicine, traumatic rupture of heart and great-vessel structures appears to be an uncommon cause of death. Here we report three cases of cardiac laceration following non-penetrating chest trauma in a one cat and two dogs. In two of these cases, necropsy revealed a rupture of the heart associated with fractures of the ribs and lung contusion; only one case did not exhibit any external chest injury but revea

Ruptured Right Sinus of Valsalva Aneurysm Caused by Suspected Takayasu's Arteritis.

A sinus of Valsalva aneurysm (SVA) is a rare aortic anomaly. The most common complication is a rupture into the right ventricle and atrium. An SVA rupture into the left ventricle is a rare event. A 42-year-old man visited an outpatient clinic due to worsening exertional dyspnea. A loud to-and-fro heart murmur was detected, and echocardiography revealed a right SVA that had ruptured into the left ventricle, inducing acute heart failure. Computed tomography imaging allowed us to determine that the right SVA had been caused by asymptomatic Takayasu's arteritis. The patient was treated with predni

Aortic Complex Rupture After Transcatheter Aortic Valve Implantation.

Aortic complex rupture is one of the most critical complications associated with transcatheter aortic valve implantation (TAVI). Its incidence is rare, and its mechanism varies by case; therefore, it is difficult to identify the predictors of complex rupture. Herein, we report a clinical case series of aortic complex rupture. Within our cohort, the frequency of complex rupture was 0.8% (4/497 consecutive patients) with an in-hospital mortality of 0. Among these four patients with complex rupture, two underwent emergent thoracotomy and surgical hemostasis without a heart-lung machine and surgic

Clinical experiences of surgical repair for mitral regurgitation secondary to papillary muscle rupture complicating acute myocardial infarction.

Mitral regurgitation secondary to ischemic heart disease carries a significant mortality even after emergency open heart surgery. From 1993 to 1997, four patients were operated on for ischemic mitral regurgitation secondary to papillary muscle rupture. These patients were between 58 and 69 years of age and all were in class III or IV of the New York Heart Association Classification. The responsible infarction area was located in the lateral wall in 2 patients, and inferior in others. The interval between the onset of acute myocardial infarction and the appearance of mitral regurgitation was fr

Cardiac rupture in acute myocardial infarction: a reassessment.

Cardiac rupture as a complication of acute myocardial infarction (AMI) has been described as occurring infrequently. Because of the recent dramatic decrease in autopsy rates, the authors believe that current studies do not accurately represent the frequency of this catastrophic complication. Autopsy protocols and archived histologic slides of patients with AMI were retrospectively reviewed to determine whether the frequency of cardiac rupture, as a complication of AMI, is altered when a non-hospital-based patient cohort after autopsy is evaluated. This review yielded 153 cases of 41 women and

Postinfarctional rupture of the interventricular septum.

During a nine year period, 36 patients have been treated at the Mid-America Heart Institute for postinfarctional rupture of the interventricular septum. Twenty-two (61%) of the patients were male and the average age was 68.9 years. Coronary angiography revealed that 48% of the patients had single vessel coronary artery disease. Eight patients, under medical treatment, stabilized with congestive heart failure. One of these patients was operated upon early and six patients initially managed medically underwent delayed operation 1-6 months after septal rupture without operative mortality. Twenty-

The ability of traditional vital signs and shock index to identify ruptured ectopic pregnancy.

OBJECTIVE: This study evaluated the correlation between vital signs and hemoperitoneum and the association between abnormal vital signs and tubal rupture. STUDY DESIGN: With the use of a retrospective case-control design, the initial heart rate, systolic blood pressure, and heart rate/systolic blood pressure were correlated with respect to degree of hemoperitoneum; predictive values were calculated. RESULTS: Fifty-two patients were studied (25 ruptured pregnancies and 27 unruptured ectopic pregnancies). Correlation coefficients were heart rate (r=0.50; 95% CI, 0.26-0.68), systolic blood pres

Managing blunt cardiac injury.

Blunt cardiac injury (BCI) encompasses a spectrum of pathologies ranging from clinically silent, transient arrhythmias to deadly cardiac wall rupture. Of diagnosed BCIs, cardiac contusion is most common. Suggestive symptoms may be unrelated to BCI, while some injuries may be clinically asymptomatic. Cardiac rupture is the most devastating complication of BCI. Most patients who sustain rupture of a heart chamber do not reach the emergency department alive. The incidence of BCI following blunt thoracic trauma remains variable and no gold standard exists to either diagnose cardiac injury or provi

A case of right sinus of valsalva rupture with dissection into interventricular septum causing left ventricular outflow tract obstruction.

Sinus of Valsalva aneurysm (SVA) is an uncommon anomaly of the aorta. Rupture of SVA often precipitates dramatic clinical complications, including heart failures. Right SVAs are the most common type, and when they rupture, they usually rupture into the right ventricle or right atrium. Rupture into left ventricle or interventricular septum is rare. Herein, we report a case of right SVA rupture with dissection into interventricular septum, which produced significant left ventricular outflow tract obstruction and aortic regurgitation. The case was successfully treated by surgical operation.

Ruptured Sinus of Valsalva Aneurysm in Apert Syndrome: Case report.

BACKGROUND: Sinus of Valsalva aneurysm (SOVA) is a rare anomaly of the aorta that can be congenital or acquired. It can be associated with syndromes such as Marfan syndrome and Ehlers-Danlos syndrome. However, to our knowledge, it has never been described in a patient with Apert syndrome. Although it often presents as an incidental finding on imaging, SOVA is associated with the risk of serious complications, including rupture. A possible connection between the conditions might be the FGFR2 gene mutation in Apert syndrome and the influence of a mutation in fibroblast growth factor 2 (FGF2) on

[Rupture of the interventricular septum in acute myocardial infarct: diagnosis by Doppler color code. Clinical case].

Doppler Color Flow Mapping represents the most recent step in the evolution of Doppler techniques that display blood flow signals superimposed on real time, two dimensional echocardiographic images. The following case illustrates the utility of this technique in the diagnosis of a ventricular septal rupture complicating the course of an inferior wall acute myocardial re-infarction. An abnormal blood flow pattern from the left to right ventricle through the ventricular septum was promptly visualized. Early operative intervention may improve the prognosis of acute infarct-related ventricular sep

The value of three-dimensional echocardiography in the diagnosis and management of ruptured sinus of Valsalva aneurysm: a case report.

BACKGROUND: A sinus of Valsalva aneurysm (SVA) is a rare cardiac anomaly. Most SVA's rupture into right heart chambers and can be classified using the modified Sakakibara classification according to the site of rupture. Transoesophageal echocardiography (TOE) is a useful diagnostic tool and aides in treatment planning in patients with congenital anomalies in emergency situations. Three-dimensional TOE (3D-TOE) provides additional value over standard TOE. CASE SUMMARY: A 38-year-old man with a reported history of ventricular septal defect (VSD) presented to the emergency department complaining
