INTRODUCTION: Hypopharyngeal perforation is a rare but potentially life-threatening complication that can follow cervical or aerodigestive procedures. Its deep anatomical location and often subtle symptoms make early recognition difficult, increasing the risk of mediastinitis and sepsis. Massive hematemesis as the initial manifestation is exceptionally uncommon and diagnostically challenging.
CASE SUMMARY: A 49-year-old man underwent elective C4-C5 anterior cervical discectomy for degenerative canal stenosis. Shortly after extubation, he developed massive hematemesis. Endoscopy revealed a per

BACKGROUND: To evaluate the efficacy and safety of gastric lavage (GL) in neonates with coffee-ground hematemesis due to swallowing maternal blood by postnatal day 1.
METHODS: This multicenter, randomized controlled trial included term neonates presenting coffee-ground hematemesis by postnatal day 1. Patients were randomly assigned to GL or non-GL groups. The primary outcome was the rate of feeding intolerance (FI) within 24 h. Secondary outcomes included onset of FI, residual gastric volume, vomiting episodes, weight changes, and bilirubin levels. Adverse events were monitored.
RESULTS: Eig

Some platelet alpha-granule contents were assessed in parallel with other markers of hemostatic imbalance in 50 patients with hepatosplenic schistosomiasis (15 patients with compensated hepatosplenomegaly, 15 patients with advanced hepatic fibrosis and ascites and 20 patients during an acute attack of hematemesis from ruptured esophageal varices). Platelet factor 4 (PF4), beta-thromboglobulin (beta-TG), fibronectin (FN), prothrombin fragment 1 + 2, thrombin-antithrombin (TAT) complexes, fibrin degradation products (FbDP) and D-dimer were assessed in schistosomal patients compared to controls (

INTRODUCTION: Scimitar syndrome, or congenital venolobar syndrome, is a rare congenital heart condition that typically involves partial anomalous pulmonary venous drainage (PAPVD) of the right lung, right lung hypoplasia, and dextroposition of the heart. The incidence is estimated to be between 1 and 3 cases per 100 000 live births. Most patients present with cardiopulmonary symptoms, though rare cases may involve gastrointestinal (GI) complications. We report a unique case of Scimitar syndrome in a 10-year-old boy presenting with hematemesis due to esophageal varices.

BACKGROUND: Hematemesis is a rare manifestation of a bronchial artery aneurysm (BAA), as bleeding from a ruptured BAA typically occurs into the bronchial tree, leading to hemoptysis rather than gastrointestinal bleeding.
CASE PRESENTATIONS: A 71-year-old male presented to the emergency department with syncope and hematemesis. Computed tomography angiography (CTA) revealed a ruptured bronchial artery aneurysm in the posterior mediastinum, with contrast extravasation into the lower esophagus. The patient underwent transcatheter arterial embolization (TAE) using coils, a mixture of N-butyl cyano

Esophageal varices are life-threatening complications in which the enlargement of the esophageal veins causes bleeding and reduces blood flow to the esophagus. They are complications caused by portal hypertension, renal failure, hepatic dysfunction, and infection. The leading cause of esophageal varices is cirrhosis, as patients with this disease are more susceptible to forming esophageal varices. Bleeding episodes occur due to the rupture of the blood vessels. We present the case of a 45-year-old male patient in the hospital with a history of chronic alcohol use and clinical symptoms of hemat

INTRODUCTION: The esophageal squamous papilloma (ESP) is a rare cause of dysphagia and hematemesis. The malignant potential of this lesion is uncertain; however, the malignant transformation and concurrent malignancies have been reported in the literature.
CASE DESCRIPTION: We report a case of esophageal squamous papilloma in a 43 years old female who had a background diagnosis of metastatic breast cancer and liposarcoma of the left knee. She presented with dysphagia. Upper gastrointestinal (GI) endoscopy showed a polypoid growth, and its biopsy confirmed the diagnosis. Meanwhile, she present

Aortic aneurysm is an uncommon in young patients. The purpose of this report is to describe a case of dissecting aortic aneurysm in a 32-year-old man with human immunodeficiency virus (HIV) infection. The presenting symptom was recurrent massive hematemesis. Diagnosis was suspected on the basis of clinical cardiovascular assessment and confirmed by ultrasonography and computed tomography. The clinical course was complicated by superior vena cava syndrome and kidney failure. Outcome in the absence of surgical treatment was fatal. Perusal of the literature indicates a rising incidence of large v

A case of fatal haematemesis due to erosion of a retro-oesophageal right subclavian artery by a nasogastric tube is reported. In view of this exceptional but extremely serious complication, no oesophageal tube should be used in patients known to have this abnormal anatomical arrangement. Systematic treatment of aberrant subclavian arteries should perhaps be considered when it can be performed during thoracic surgery.

The differential for neonatal hematoma sis ranges from benign etiologies to life-threatening emergencies. Neonatal gastric perforation is a rare cause of neonatal hematoma sis but is a deadly condition, requiring prompt diagnosis and treatment. The etiology is usually related to conditions predisposing to over distension of the stomach, such as positive pressure ventilation or distal obstruction, but in some cases cannot be determined. Patients generally present with abdominal distension and respiratory distress. We present a case of a 1-day old term baby girl who developed sudden onset hemato

Foreign body ingestion in adults is a clinically significant condition that may lead to serious complications, particularly in cases involving intentional ingestion of high-risk objects. We report the case of a 34-year-old woman who presented with a two-day history of hematemesis after intentionally ingesting two metallic foreign bodies approximately two weeks before admission. Imaging studies, including abdominal radiography and computed tomography, revealed elongated metallic objects measuring up to 6.8 cm within the stomach, associated with focal gastric wall thickening. Given the size of t

A gastric ulcer was detected in a 54-year-old man who underwent upper gastrointestinal endoscopy for hematemesis. An abdominal contrasted computed tomography scan detected a splenic artery aneurysm adjacent to the gastric wall. Endoscopic hemostasis was thought to be risky owing to possible rupture of the aneurysm. Rupture of a splenic artery pseudoaneurysm due to segmental arterial mediolysis (SAM) was diagnosed by abdominal angiography, and subsequently transcatheter arterial embolization was performed. In cases of upper gastrointestinal hemorrhage, hemostasis is often performed during the e

Esophageal variceal bleeding is a common lethal complication of cirrhosis. Endoscopic injection sclerotherapy (EIS) is one of the major endoscopic approaches for treating esophageal variceal bleeding. However, complications may occur after EIS, which mainly include retrosternal discomfort/pain, dysphagia, re-bleeding, esophageal ulcer, esophageal strictures, and esophageal perforation, etc. In this article, we reported a 36-year-old male who developed esophageal ulcer related bleeding after EIS. Currently, there is no consensus on the treatment strategy for esophageal ulcer-related bleeding af

Congenital duodenal obstruction, a common complication of Down syndrome, most often presents as neonatal bilious vomiting. We report the first premature infant with Down syndrome whose duodenal atresia presented with a massive hematemesis secondary to duodenitis. Since a plain radiograph of the abdomen is diagnostic in the case of duodenal obstruction, it should be included in the diagnostic workup of hematemesis in newborn infants.

Hepatoportal arteriovenous fistulae are a rare cause of portal hypertension, which can have significant clinical manifestations. They have multiple etiologies, one of which includes hepatic trauma. We present a case of hepatoportal fistula presenting with bleeding esophageal varices in an 83-year-old man. The exact cause of fistula in this case is not entirely clear; however, hepatic trauma was noted in the patients' history, some 52 years before presentation. We also present a literature review on this rare and interesting phenomenon.
