INTRODUCTION: Coronary artery aneurysm (CAA) rupture is a rare but life-threatening condition that typically presents as acute coronary syndrome or with hemodynamic collapse. Distinguishing whether severe trauma is the primary event or secondary to a preceding medical catastrophe poses a significant diagnostic challenge in the emergency setting.
CASE PRESENTATION: An 85-year-old woman, found crouched on a street, was struck by a truck and was transported to the emergency department. On arrival, she was unconscious but hemodynamically stable. Contrast-enhanced computed tomography revealed a ru

A 29-year-old male sustained a penetrating abdominothoracic injury after falling onto a sharpened wooden branch. The object entered through the right hemiscrotum, traversed the abdomen, and extended through the diaphragm into the left pleural space. On admission, he had hemorrhagic shock, hypoxemia, tachypnea, and hypotension. Computed tomography showed hemoperitoneum, bowel and mesenteric injuries, diaphragmatic rupture, left hemopneumothorax, pulmonary contusion, subcutaneous emphysema, and rib fractures. Emergency laparotomy and thoracoscopy included small bowel resections with anastomoses,

Traumatic esophageal-tracheal fistulas (ETFs) secondary to a stab wound are rare and often underdiagnosed. Chest computed tomography (CT) scan and opacification of the esophageal lumen help in diagnosis. Surgery is required for treatment. We here report the case of a patient who developed an ETF after being stabbed in the back. Admitted six hours after the incident, he complained of mid-thoracic pain. Physical examination revealed a right paravertebral wound with intrathoracic knife blade. Imagery showed the blade traversing the upper mediastinum, right hemopneumothorax, and mediastinal air bu

Diaphragmatic injuries, particularly on the right side, are a rare yet challenging clinical scenario, especially when associated with other injuries. We present the case of a 38-year-old male patient who sustained a fall from a significant height, resulting in blunt abdominal trauma, chest injuries, right-side diaphragmatic injury, a grade 4 liver injury, and fractures of the right ribs, right femur, and pelvis. The patient also suffered a lung laceration with hemopneumothorax. The clinical team managed these injuries through a video-assisted thoracoscopy, laparotomy, and primary repair of the

A 44-year-old man who drove a motorcycle experienced a collision with the side of another motorcycle. Because he had sustained a high-energy injury to the spinal cord, he was transferred to our hospital. His circulation was unstable, and received tracheal intubation in addition to thoracostomy for the hemothorax. Whole-body computed tomography (CT) revealed multiple fractures, right hemopneumothorax with pulmonary contusion, and minor liver injury. After infusing 5000 mL of lactated Ringer's solution and 10 units of blood, his circulation remained unstable. On a repeat CT examination, the left

Thirty patients with 33 vascular injuries from blunt trauma to the brachiocephalic branches of the aortic arch are reported. To our knowledge, this is the largest series reported to date of blunt injuries to these vessels. Mechanisms of injury included deceleration, traction, and crush. Half of the injured vessels were the innominate artery, and a quarter each were the common carotid and subclavian arteries. Common associated injuries were head injuries, hemopneumothorax, lung contusion, long bone fractures, and brachioplexus injuries. Widened mediastinum and extrapleural hematoma were common

Exposure of arch branches for repair following trauma can be difficult. Seven patients with blunt injuries and 5 with penetrating injuries underwent repair of the innominate, common carotid and subclavian arteries. Common associated injuries included head injury, hemopneumothorax, lung contusion, long bone fractures and brachial plexus injury. Widened mediastinum was seen in 5 patients. Six patients with innominate, right subclavian and right common carotid injuries were approached through sternotomy with extension into the right side of the neck. Posterolateral thoracotomy was used to reach t

A 66 years old male was admitted to our hospital after a serious car accident. The patient presented with severe shock after admission. After the examination, the patient was diagnosed with hemopneumothorax and myocardial contusion, accompanied by spleen rupture. After emergency surgery and a series of symptomatic treatments, the patient's condition gradually stabilized. One week later, the patient suddenly presented with severe shock. Massive hemothorax was found on the left side of the chest. Surgical exploration revealed cardiac rupture and accidental absence of congenital pericardium. Acco

BACKGROUND: Traditional lead systems for implantable cardioverter defibrillators (ICDs) require a thoracotomy for placement. Nonthoracotomy lead systems are available and are usually implanted by an electrophysiologist and a surgeon. The purpose of this study was to prospectively evaluate the safety and efficacy of ICD implantation with a nonthoracotomy lead system by electrophysiologists.
METHODS AND RESULTS: A consecutive series of 100 patients (mean age, 61 +/- 13 years, +/- SD) underwent ICD implantation with a nonthoracotomy lead system while intubated and under general anesthesia. Seven

A 31-year-old male sustained life-threatening chest contusions and recovered after damage control surgery. The patient was in an unwitnessed accident where his motorcycle was struck by a car. Upon admission, blood pressure was 69/58 mmHg, heart rate was 126 bpm, and oxygen saturation was 85%. Chest computed tomography revealed fractures to right ribs 1-9 and left ribs 1-7, lung contusions, multiple lung lacerations, and right hemopneumothorax. Upon presentation to the intensive care unit, hemostasis was achieved by suturing a deeply lacerated lung and applying gauze packing. The patient was pl

INTRODUCTION: Combined thoracoabdominal injuries are the most severe case in the medical practice. Bilateral injuries are rare case in patients and just few cases reported in literature. Diaphragm traumas and pneumothorax are defined as severe trauma leading to injuries of thoracic and abdominal organs. We are describing multiple injuries of abdominal tract and chest causing internal bleeding and hemothorax in patient. Surgical operation was carried out on the right side thoracotomy including revision of the pleural cavity and subphrenic space, closure of the wound, removal of the blood clot a

Urinary bladder and urethral injuries usually occur as part of multiple injuries in polytrauma patients. These injuries are easily overlooked because the initial evaluation is focused on other life-threatening injuries such as traumatic brain injury, hemopneumothorax or hemoperitoneum. Although the urinary bladder and urethral injuries are not life-threatening, they pose the risk of long-term morbidity which can be burdensome. These complications include urinary incontinence, voiding dysfunction, urethrocutaneous fistula, urethral stricture and erectile dysfunction. Computed tomography (CT) fi

We describe the aspiration of a traumatic haemopneumothorax and an unexpected anteroposterior chest radiograph finding after the procedure. Chest aspiration is now routine emergency management for spontaneous pneumothorax. There have been no previous documented reports of this clinical scenario of radiological deterioration with clinical improvement after aspiration of a haemopneumothorax.
