This retrospective case review details the use of noncrosslinked intact porcine-derived acellular dermal matrix (PADM) for incisional herniorrhaphy in patients with infected synthetic mesh. A consecutive series of adult patients underwent single-stage ventral herniorrhaphy involving removal of infected synthetic mesh and repair with PADM by a single surgeon (2009 to 2011). The aim of this study is to show our experience with umbilical hernia herniorrhaphy and laparoscopic cholecystectomy, both in the same act. During last 10 years we operated 89 patients with cholecystitis and pre-existing umbilical hernia. Incidental discovery of vesical hernias during herniorrhaphy is quite common. In such cases, patients are usually asymptomatic since the hernia portion is small and easily repairable. On the other hand, vesical solid protrusion to the scrotum is quite unusual, and is generally found associated to obstructive urinary symptoms. Management involves basically the correction of any associated obstructive conditions, correction of the vesical hernia and herniorrhaphy.

Inguinal herniation of the bladder is relatively common, reported in as many as 4% of inguinal hernias. In the majority of those cases, it is a sliding hernia that is noted at time of herniorrhaphy. Complete herniation of the bladder or "scrotal cystocele" is very rare and normally managed with herniorrhaphy. This case report presents a case of massive inguinal scrotal herniation of the urinary bladder successfully managed with trans-scrotal drainage via neocystostomy tube.
