Comparison of bone turnover suppression in atypical femoral fractures and osteoporotic hip fractures.

We aimed to compare the extent of bone turnover suppression between patients with atypical femoral fractures (AFFs) and osteoporotic hip fractures (typical femur fractures, TFFs) using a one-to-one matching strategy. A single-center retrospective comparison of females aged ≥ 60 years who underwent operative treatment for AFFs and TFFs between January 2010 and March 2021 was conducted. Demographic characteristics and clinical data including fracture site, past medical history, bone mineral density (BMD), bisphosphonate (BP) medication history, and serum bone turnover marker (BTM) levels were ex

Regional disparities in the risk of secondary fractures in patients with hip fractures.

PURPOSE: We aimed to examine the regional disparities in secondary fracture incidence among patients with hip fractures in South Korea. METHODS: This observational, retrospective, cohort study was conducted using data of 6,213 South Korean nationals from the National Health Insurance Service-National Sample Cohort (2004-2019). Secondary fractures included hip, wrist, humerus, spine, ankle, and pelvis fractures that occurred 6 months after hip fracture. The position value for relative composite index was used to identify medically vulnerable regions. Cox proportional hazards models were used f

Predictors of Intraoperative Fractures during Hemiarthroplasty for the Treatment of Fragility Hip Fractures.

BACKGROUND: The aim of our study was to determine the rate and preoperative predictors of intraoperative fracture (IOF) during hip hemiarthroplasty (HA) in patients who have sustained a fragility hip fracture injury. METHODS: We reviewed 626 patients who underwent HA at our institution using the National Hip Fracture Database. Various patient- and surgery-related data including demographic information, cement usage, surgeon grade, time to surgery, and operative duration were collected. The metaphyseal diaphyseal index and modified canal bone ratio were measured on preoperative radiographs. We

Treatment of unstable intertrochanteric hip fractures associated with spinal fractures. A treatment dilemma.

Treatment approaches to a patient with polytrauma comprising combined spinal and hip fractures need more careful planning than those for a patient with an isolated spinal or hip fracture. The patient in this case presentation had multiple burst fractures of the spine, but was neurologically intact. The spinal injuries were treated conservatively. The patient also had concomitant unstable intertrochanteric fractures of the hip, which were treated surgically. The presence of a thoracic or lumbar fracture, in addition to a hip or femur fracture, may require an altered therapeutic approach. We pre

Management of Failed Fixation of Extracapsular Hip Fractures.

Background Extracapsular hip fractures are routinely treated with fixation, and the majority heal without complication. The fixation fails in a minority of cases, typically either by 'cutting out' of the superior femoral head or through breakage of the metalwork following non-union. In such cases, if operative treatment is thought appropriate, there are two major treatment options: revision fixation of the fracture or joint replacement surgery. Methods Medline on OvidSP was searched using relevant medical-specific subject headings (MeSH) and keywords. The inclusion criteria were: study regardi

Postoperative delirium in geriatric patients with hip fractures.

Postoperative delirium (POD) is a frequent complication in geriatric patients with hip fractures, which is linked to poorer functional recovery, longer hospital stays, and higher short-and long-term mortality. Patients with increased age, preoperative cognitive impairment, comorbidities, perioperative polypharmacy, and delayed surgery are more prone to develop POD after hip fracture surgery. In this narrative review, we outlined the latest findings on postoperative delirium in geriatric patients with hip fractures, focusing on its pathophysiology, diagnosis, prevention, and treatment. Perioper

Effects of hospitalist co-management for hip fractures.

BACKGROUND: Early surgery is recommended for patients with hip fractures. Average time to surgery in Japan is 3.8 days. Such delay could be caused by the burden of preoperative assessment and management of geriatric comorbidities upon orthopedic surgeons. Hospitalist co-management has potential benefits in comprehensive perioperative care that could positively affect time to surgery in patients with hip fractures. We compare clinical outcomes of patients with hip fractures managed by orthopedic surgeon-led care (conventional group) vs. those of patients with hip fractures managed by hospitalis

Multidisciplinary rehabilitation for older people with hip fractures.

BACKGROUND: Hip fracture is a major cause of morbidity and mortality in older people, and its impact on society is substantial. After surgery, people require rehabilitation to help them recover. Multidisciplinary rehabilitation is where rehabilitation is delivered by a multidisciplinary team, supervised by a geriatrician, rehabilitation physician or other appropriate physician. This is an update of a Cochrane Review first published in 2009. OBJECTIVES: To assess the effects of multidisciplinary rehabilitation, in either inpatient or ambulatory care settings, for older people with hip fracture

Dynamic hip screw fixation of subtrochanteric femoral fractures.

INTRODUCTION: A subtrochanteric proximal femur fracture occurs in the 5 cm of bone immediately distal to the lesser trochanter. UK national guidelines advise that adults with subtrochanteric fractures should be treated with an intramedullary nail (IMN). This study aims to compare peri-operative outcome measures of patients with subtrochanteric fractures treated with either an IMN or a dynamic hip screw (DHS) construct. MATERIALS AND METHODS: We retrospectively reviewed subtrochanteric fractures presenting at our institution over 4.5 years (October 2014-May 2019), classifying them into two tre

[Stress hip fractures in men in military training].

INTRODUCTION: Stress fractures are common injuries in the military population, they begin with the imposition of repetitive and excessive effort on the bone. This leads to the acceleration of bone remodeling and production of microfractures. The incidence, during their basic combat training, is 0.8 to 5.1% for men compared to 1.1 to 18% among women. We present a series of hip stress fractures in young male military men with progressive training of 8 hours a day over a period of 14 weeks. RESULTS: Mean age 19.72 years, 4 transcervical and 7 basicervical fractures, 6 displaced and 5 no, 54.5 %

Holistic Management of Older Patients With Hip Fractures.

As the silver tsunami hits the world, older patients with hip fractures are expected to increase to 6.3 million by the year 2050, of which the majority will occur in Asia. The estimated global cost of hip fractures in the year 2050 is estimated to reach U.S. $130 billion. Hence, in addition to implementation of prevention strategies, it is important to develop an optimal model of care for older patients with hip fracture to minimize the huge medical and socioeconomic burden, especially in rapidly aging nations. This review summarizes the complications of hip fractures, importance of comprehens

Complications of implant removal after healed hip fractures.

INTRODUCTION: Osteosynthesis of pertrochanteric fractures is one of the most frequently performed procedures in orthopaedic trauma care. Auto-dynamization during fracture healing can lead to lateralization of the screw or blade. This can cause debilitating complaints, which sometimes necessitate the removal of the screw or blade. After removal of the implant, we observed spontaneous femoral neck fractures in five patients. This article presents an evaluation of these cases with reference to literature. MATERIALS AND METHODS: Five cases of spontaneous femoral neck fracture (SFNF) after cephalo

Complications After Pediatric Hip Fractures: Evaluation and Management.

Pediatric proximal femur fractures are high-energy injuries with predictable and morbid complications. Osteonecrosis of the femoral head is the most common complication with identified risk factors including fracture type, patient's age, degree of displacement, timing to reduction, and stability of fixation. Additional complications include nonunion, coxa vara, and premature physeal arrest. The mainstay of treatment for traumatic pediatric osteonecrosis is hip preservation with total hip arthroplasty being reserved as a salvage procedure. An anatomic fracture reduction and a biomechanically st

Hypovitaminosis D in patients with osteoporotic hip fractures.

BACKGROUND: Inadequate serum vitamin D levels are associated with secondary hyperparathyroidism, increased bone turnover, bone loss and increased fracture risk. Vitamin D is well recognized to be suboptimal in older patients when compared to age-matched controls. There are no published studies on the prevalence of hypovitaminosis D in Indian population with fragility fractures around the hip associated with osteoporosis and comminution at the fracture site. AIM: To investigate the prevalence of hypovitaminosis D in patients admitted with osteoporotic hip fractures and associated fracture site

There are no specific guidelines for treating Parkinson's disease patients who present with a hip fracture. Here we present a large cohort of patients with Parkinson's disease who suffered hip fractures. Our aim was to assess for differences between a Parkinson's disease population and a non-Parkinson's disease population with hip fractures and make recommendations on management guidelines. We performed a comprehensive analysis of prospectively collected data on all patients with hip fracture who were admitted into our department over a period of 29 years. In total 9225 patients with hip fract

Fall scenarios In causing older women's hip fractures.

OBJECTIVE: Falls and fall-related injuries among older women constitute a major public health problem with huge costs for the society and personal suffering. The aim of this study was to describe and illustrate how a number of circumstances, conceptualized as a scenario, that were related to the individual, the environment, and the ongoing occupation contributed to a fall that led to a hip fracture among women. The sample included 48 women over 55 years old. METHODS: Interviews were conducted during home visits and the analysis provided a descriptive picture of circumstances in the shape of a
