This article describes the intricate challenges of bringing mental health services to isolated, guarded urban HIV-positive Native Americans suffering from chronic trauma-related illnesses and imbalances, depression, anxiety, substance abuse, thought disorders and trauma-based characterological disorders. It explores the integration of art therapy, Bowen Family Systems Therapy and in-home therapy in the Family & Child Guidance Clinic's attempt to provide support to a population that has profound distrust for "services and treatment," and no historical context for psychotherapy. Changing the par

We report 2 cases where accidents from inline skating resulted in significant genital injury to female children. When genital trauma occurs in children, it frequently raises the concern that sexual abuse might have caused the injury. Historical information provided by the child and others must be compared with the injuries to understand whether an injury may be the result of abuse. We characterize the nature of the genital injuries of our cases as being caused by an accidental "rapid-splits" type mechanism while children were using inline skates. We conclude that the reported accidental mechan

We present the case of a four-year-old child evaluated for core features of autism spectrum disorder, whose family context included a history of second-trimester perinatal loss 14 months prior to conception and severe maternal motor vehicle trauma at 18 weeks' gestation. This background coincided with parental hypervigilance, anticipatory grief, and complex caregiving dynamics, while early infancy was characterized by severe regulatory disturbances and sensory sensitivities that interacted with parental stress responses. Management utilized a dual approach combining neurodiversity-affirming de

Throughout history the consequences of psychological trauma and characteristic symptoms have involved clinical presentations that have had different names. Since the inclusion of the category of Posttraumatic Stress Disorder (PTSD) in the third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-III) with the symptomatic triad of re-experiencing the traumatic event, avoidance behaviors, and hypervigilance, this entity has been a source of controversy. Indeed, some authors have denied its existence, even considering it a diagnostic invention. In this article we review, fro

In a community-based sample of urban American Indian and Alaska Native adults, 25 lesbian, gay, bisexual, and two-spirit participants were compared with 154 heterosexual participants with respect to sociodemographic characteristics, Native, cultural participation, trauma, physical and mental health, and substance use. Compared with their heterosexual counterparts, two-spirit participants reported higher rates of childhood physical abuse and more historical trauma in their families, higher levels of psychological symptoms, and more mental health service utilization. Two-spirit participants repo

Uroperitoneum (UP) was diagnosed in 26 cats. Trauma was the most common cause (84.6%), including blunt abdominal trauma (59.1%), urethral catheterization (31.8%), and bladder expression (9.1%). The bladder was the most frequent site of urine leakage following blunt abdominal trauma (84.6%), while the urethra was the most common site following catheterization (71.4%). Common historical complaints were anuria (53.8%) and vomiting (50%). On physical examination, the bladders were palpable in nine (69%) of 13 cases; four of the nine had ruptured bladders. The ability to urinate did not exclude a d

Minimally invasive procedures are being increasingly proposed for trauma. Injuries to the chest wall and/or lung have historically been managed by drainage with a large bore thoracostomy tube, while cardiac injuries have mandated sternotomy. These treatments are associated with significant patient discomfort. Percutaneous placement of small 'pigtail' catheters was initially designed for drainage of simple pericardial fluid. Their use subsequently expanded to drainage of the pleural cavity. The role of pigtail catheters for primary treatment of traumatic pneumothorax and hemopneumothorax has in

BACKGROUND: After the beginning of the COVID-19 pandemic, there was a 53% increase in overdose deaths in the U.S., with the highest rates among American Indian/Alaska Natives (AI/AN) individuals compared with all racial/ethnic groups in the U.S. Historical trauma has been linked to substance use among AI/AN individuals. First cited in the 1990s, historical trauma is an intergenerational response to the loss of AI/AN communities' land, peoples, and culture secondary to colonization in North America. Two-Eyed Seeing is the gift of multiple perspectives treasured by many Indigenous peoples, which

BACKGROUND: Hemorrhage is a leading cause of preventable trauma death, particularly in rural settings where prolonged transport times delay resuscitation. Low-titer O+ whole blood (LTOWB) is increasingly favored for prehospital resuscitation due to its combined oxygen-carrying and hemostatic properties. However, logistical challenges and concerns regarding waste and cost have limited its implementation in rural trauma systems. This study evaluates the feasibility and clinical utility of a prehospital LTOWB pilot program at a rural Level I trauma center.
METHODS: We conducted a prospective, si

This paper discusses the main features of Siberian identity formed throughout the historical development of Siberia under the influence of social, economic, geographical, climatic, and other factors. Siberian cultural identity is closely connected with the mythology and ancient religion of the indigenous peoples of Siberia - shamanism, whose rituals, images, symbols, and motifs are often manifested in the clients' dreams. Following an in-depth study of Siberian history and culture, I formulate a complex of homelessness rooted in a deep collective trauma that left its imprint on people's psyche

BACKGROUND: The San Diego Paramedic Rapid Sequence Intubation (RSI) Trial documented an increase in mortality after paramedic RSI, with hyperventilation identified as a contributing factor in a small subgroup analysis. Here we explore factors affecting outcome in the entire cohort of patients undergoing paramedic RSI to confirm previous findings. This also represents a synthesis of findings from previous analyses
METHODS: Adult trauma patients with severe head injury (Glasgow Coma Scale score, 3-8) who could not be intubated without RSI were prospectively enrolled in the trial. This analysis

The impact of the COVID-19 virus disproportionately affected U.S. Indigenous peoples, who experienced the highest infection and death rates in comparison with non-Indigenous peoples. In this article, we use the framework of historical oppression, resilience, and transcendence (FHORT) to understand how Southeastern Indigenous peoples in the United States navigated hardships associated with the COVID-19 global pandemic. This culturally congruent framework contextualizes imbalances found at individual, family, and community ecological levels, illustrating a direct correlation to sociopolitical, h

The remnants of colonialism manifesting as structural violence, racism, and oppression continue to plague our society as evidenced by the persistence of health inequities, particularly for minority populations in the United States. As a profession bound by moral and ethical mandates, nursing must resist and deconstruct oppression in all its forms. Nurses, informed by critical race theory, intersectionality, and historical trauma, can become formidable allies with marginalized populations in the fight for social justice and health equity.

This systematic review explores how trauma is conceptualized and measured in the context of community violence, with an emphasis on the limitations of dominant frameworks and the need for more critical, structural approaches. These approaches situate trauma within conditions of structural violence that shape chronic community violence and uniquely impact the lives of community members. While traditional trauma frameworks often focus on individual pathology, this review examined prevailing trauma models, specifically those centered around post-traumatic stress disorder (PTSD), and highlighted t

In much of contemporary culture, "trauma" signifies not so much terrible experience as a particular context for understanding and responding to a terrible experience. In therapy, in the media, and in international interventions, the traumatized are seen not simply as people who suffer and so are deserving of concern and aid; they are seen also as people who suffer for us, who are given special dispensation. They are treated with awe if they tell a certain kind of trauma story, and are ignored or vilified if they tell another. Trauma has become not simply a story of pain and its treatment, but

Post-traumatic stress disorder (PTSD) is a psychiatric disorder that causes debilitating functional impairment in patients. Observations from survivors of traumatic historical events solidify that this disease is not only associated with personal experiences but can also be inherited from familial traumas. Over the past decades, researchers have focused on epigenetic inheritance to understand how responses to adverse experiences can be passed down to future generations. This review aims to present recent findings on epigenetic markers related to PTSD and research in the intergenerational inher

Culturally relevant stressors and protective factors are vital to understanding and effectively supporting Native American/Alaska Native (NA/AN) college students' mental health and well-being. This study examined the theorized pathways among historical loss, well-being, psychological distress, and the proposed cultural buffer of ethnic identity in the indigenist stress-coping model (ISCM). Cross-sectional data were collected via online survey and analyzed using structural equation modeling. Participants were a national sample of 242 NA/AN college students. Participants were predominantly women

The emergency presentations of 85 patients diagnosed by computerized axial tomography (CT) as having subdural hematomas (SDH) were retrospectively reviewed to identify factors contributing to an early or delayed diagnosis. Sixty-one of 85 patients (72%) were diagnosed within 24 hours of presentation. Forty-five of 57 (79%) with definite history or signs of cranial trauma were diagnosed within 24 hours, while only 16 of 28 (57%) without historical or physical evidence of trauma were diagnosed promptly. Focal neurologic signs were present in 32 of 57 patients (56%) with head trauma and 24 of the
