The spectrum of hospitalization-associated harm in the elderly.

Acutely ill patients are not infrequently referred to the hospital and admitted, when they could be diagnosed and managed in the ambulatory setting or by hospital-level care at home. Avoidable admissions are particularly regrettable when the wide spectrum of hospitalization-associated patient harm is considered. It includes acute discomfort to the patient due to multiple disturbing hospital stressors; an emotional trauma; the burden of multiple redundant tests begetting false-positive and incidental findings triggering further testing and cascades; highly prevalent adverse events and serious h

Transcranial Doppler After Successful Endovascular Revascularization and Hospitalization Outcomes.

BACKGROUND: Almost half of large-vessel occlusion strokes have unfavorable outcomes despite successful endovascular therapy. We aim to investigate whether postrevascularization cerebral hemodynamics, determined by transcranial Doppler (TCD), associate with hospitalization outcomes in this population. METHODS: The current observational cohort study analyzed 155 patients with successfully revascularized anterior circulation large-vessel occlusion stroke (mean age, 68.3±15.4 years; 55% women) who had TCD within 48 hours from endovascular therapy. TCD parameters (mean flow velocity, peak systolic

Managing tandem breastfeeding during acute hospitalization: A case study.

This case study presents a mother who continued tandem breastfeeding throughout the acute admission of her critically ill newborn. Her younger child, the subject of this case report was admitted to our hospital with septic shock and progressing bullous scalp lesions. The infant had streptococcal toxic shock syndrome and necrotizing fasciitis of the scalp due to streptococcus pyogenes. The infant required immediate life-sustaining interventions, followed by weekly surgical procedures which meant the infant had brief periods of intravenous nutrition surrounding the time of anesthesia administrat

Legionellosis-Associated Hospitalization in Spain from 2002 to 2021.

Legionellosis is a respiratory disease of bacterial and environmental origin that usually presents two distinct clinical entities, "Legionnaires' disease" (LD) and "Pontiac fever". LD is an important cause of hospital-acquired pneumonia (HAP). The objective of this study is to describe the epidemiology of legionellosis-associated hospitalization (L-AH) in Spain from 2002 to 2021 and the burden of hospitalization due to legionellosis. Discharge reports from the Minimum Basic Data Set (MBDS) were used to retrospectively analyze hospital discharge data with a diagnosis of legionellosis, based on

Detection of pneumococcus during hospitalization for SARS-CoV-2.

BACKGROUND: Infections with respiratory viruses [e.g. influenza and respiratory syncytial virus (RSV)] can increase the risk of severe pneumococcal infections. Likewise, pneumococcal coinfection is associated with poorer outcomes in viral respiratory infection. However, there are limited data describing the frequency of pneumococcus and SARS-CoV-2 coinfection and the role of coinfection in influencing COVID-19 severity. We, therefore, investigated the detection of pneumococcus in COVID-19 inpatients during the early pandemic period. METHODS: The study included patients aged 18 years and older

[SUICIDES DURING PSYCHIATRIC HOSPITALIZATION - CHARACTERISTICS AND WAYS OF COPING].

It is known as a circumstantial background to suicide that patients with psychiatric disorders admitted to a psychiatric hospital are at risk of suicide. Most of the reasons offered for suicide during hospitalization link them to the circumstances of the need for psychiatric hospitalization: the existence of a significant mental disorder and the condition of a crisis, that in many cases, cover a complete life span: psychological, social and medical. In Israel, the multi-year average number (1990-2013) of suicides during psychiatric hospitalization is about 13.6, which account for 3.4% of the t

Effects of atrial fibrillation on outcomes of influenza hospitalization.

BACKGROUND: There is little information available on AF and its association with outcomes in adult influenza hospitalizations. METHODS: The National Inpatient Sample was queried from years 2009-2018 to create a cohort of discharges containing an influenza diagnosis. AF was the primary exposure. Univariate and multivariate regression analysis was used to describe the association of AF with clinical and healthcare-resource outcomes. Finally, a doubly-robust analysis using average treatment effect on the treated (ATT) propensity score weighting was performed to verify the results of traditional

Hospitalization rates from radiotherapy complications in the United States.

Hospitalizations due to radiotherapy (RT) complications result in significant healthcare expenditures and adversely affect the quality of life of cancer patients. Using a nationally representative dataset, the objective of this study is to identify trends in the incidence of these hospitalizations, their causes, and the resulting financial burden. Data from the National Inpatient Sample was retrospectively analyzed from 2005 to 2016. RT complications were identified using ICD-9 and ICD-10 external cause-of-injury codes. The hospitalization rate was the primary endpoint, with cost and in-hospit

The Effects of Maintenance Electroconvulsive Therapy on Hospitalization Rates.

This study aimed to study the effect of maintenance electroconvulsive therapy (mECT) on hospitalization rates in patients who had been readmitted after acute courses of electroconvulsive therapy (ECT), and determine the most frequently used treatment schedules in mECT. Patients who had undergone mECT treatment for the last 5 years were retrospectively reviewed. Seventy patients were included in the study. The control group of 70 patients was selected from patients who received only acute ECT. Of the patients in the mECT group, 55.8% (39) were female, and 41.4% (29) were diagnosed with major de

Psychiatric hospitalization among youth at high risk for HIV.

Youth at-risk for HIV are also at-risk for mental health disorders and psychiatric hospitalization. Understanding the association between engagement in HIV prevention, concurrent risk behaviors, and psychiatric hospitalization may lead to improvements in integrated prevention and mental health treatment efforts. Youth at-risk for HIV, aged 14-24 years old, predominantly Black/African American and Latinx (75%) were recruited through youth-serving clinics and community sites in Los Angeles (n = 839) and New Orleans (n = 647). We compared youth with and without histories of psychiatric hospitaliz

[Involuntary hospitalization: A survey of liberty and custody judges].

The procedure of involuntary hospitalization in France has been recently modified by the law of 5 July 2011. Since that time, a liberty and custody judge has been appointed to guarantee the rights of psychiatric inpatients and to prevent abusive hospitalizations. Currently, for one involuntary hospitalization in ten a release is decided by the liberty and custody judge although psychiatrists consider that psychiatric care is necessary. In order to improve our understanding of the role of liberty and custody judges, and how they make their decisions, we conducted a qualitative survey of liberty

Changes in Pediatric Heart Transplant Hospitalization Costs Over Time.

BACKGROUND: Despite significant changes in the past decade for children undergoing heart transplantation, including the evolution of mechanical circulatory support and increasing patient complexity, costs and resource utilization have not been reassessed. We sought to use a novel linkage of clinical-registry and administrative data to examine changes in hospitalization costs over time in this population. METHODS: We identified all pediatric heart transplant recipients in a unique linked Pediatric Health Information System/Scientific Registry of Transplant Recipients data set (2002-2016). Hosp

Air Pollution and Hospitalization for Bronchiolitis among Young Children.

RATIONAL: Several studies have found higher risks for childhood respiratory illness, associated with exposure to particulate matter (PM) less than 10 μm in diameter (PM10) and PM2.5 and gaseous pollution. OBJECTIVES: We analyzed the association between air pollution and hospitalizations due to bronchiolitis, an obstructive pulmonary disorder, commonly caused by respiratory syncytial virus infant infection. METHODS: Data were obtained from a local tertiary medical center providing services for a population of 700,000 comprising two ethnic groups: predominantly urban Jews and rural Bedouin Ara

STRESS IN PEDIATRIC PATIENTS--THE EFFECT OF PROLONGED HOSPITALIZATION.

Long-term hospitalization emotionally impacts any patient, especially children, and is defined as a long period of time during which the patient is hospitalized and experiences isolation from his or her family, friends and home. Stressful situations trigger a nonspecific response that involves multiple physiological mechanisms. Currently, because of the complexity of these mechanisms, there are no laboratory markers that allow the quantification of the stress intensity felt by the patient. Laboratory determinations currently used in evaluating the response to stress are neuroendocrine, immunol
