Wasting coexisting with underweight and stunting among children aged 6‒59 months hospitalised in Garissa County Referral Hospital, Kenya.

Management of undernourished children depends only on wasting yet it can coexist with underweight and/or stunting. Among children admitted to hospital with acute illness, we determined the proportion with wasting coexisting with underweight and/or stunting and their risk factors. A retrospective review of hospital records of children 6‒59 months old admitted at Garissa County referral hospital, Kenya, from January 2017 to December 2019 was conducted. Using World Health Organization 2006 growth standards, undernutrition were defined: wasting as Weight-for-height Z-score < -2, stunting Height-fo

Residence in urban or rural counties in relation to opioid overdose mortality among Kentucky hospitalizations before and during the COVID-19 pandemic.

BACKGROUND: At the beginning of the opioid overdose epidemic, overdose mortality rates were higher in urban than in rural areas. We examined the association between residence in an urban or rural county and subsequent opioid overdose mortality in Kentucky, a state highly impacted by the opioid epidemic, and whether this was modified by the COVID-19 pandemic. METHODS: We captured hospitalizations in Kentucky from 2016 to 2020, involving an opioid using ICD-10-CM codes T40.0-T40.4 and T40.6. Patient's county was classified as urban or rural based on the NCHS Urban-Rural Classification Scheme. M

Demographic and socioeconomic correlates of suicide deaths and nonfatal self-injury related hospital visits: An analysis of counties in New York State.

For many years, suicide rates in U.S. general and veteran populations have steadily increased, stimulating research into suicide and nonfatal self-injury (NFSI). However, little research has examined community correlates of suicide and NFSI. This study used county data from New York State to identify community correlates of veteran and general population suicide deaths and general population NFSI-related hospitalization. In bivariate analyses, both suicide and NFSI-related hospitalization were associated with measures of social disintegration (i.e., smaller population size, larger male and/or

HepB-boost: Implementation of free-of-charge health care worker hepatitis B testing and vaccination at Kilifi County Referral Hospital, Kenya.

OBJECTIVES: Health care workers (HCWs) are at high risk of hepatitis B virus (HBV) exposure due to contact with blood and bodily fluids. In Kenya, HCWs are rarely fully vaccinated against HBV. During 2024, Kilifi County Referral Hospital (KCRH) in Kenya implemented HCW HBV testing and vaccination. We assess this implementation, including acceptability, feasibility, and costs. METHODS: A technical working group was formed at KCRH, sensitization was undertaken, and vaccines procured in multi-dose vials. Clinics were run for 2 hours twice weekly over 6 months. Hepatitis B surface antigen testing

Endobronchial valve placement for persistent air leaks in patients with underlying lung disease: a case series from a resource-strapped county hospital.

Current guidelines recommend pleurodesis or video-assisted thoracoscopic surgery (VATS) for the management of persistent air leaks (PALs). However, for those with underlying lung disease, the interventions may not be feasible. Endobronchial valve (EBV) placement has shown early promise as a viable option in this patient population. The aim of our study is to evaluate the time to successful resolution of PALs following EBV placement. We hypothesize that its early implementation may reduce hospital costs, particularly in the setting of a public safety-net hospital. A single-center retrospective

Increased prevalence, ER visits, and hospitalizations in medicare systemic lupus erythematosus patients living in socially vulnerable counties: A cross-sectional study.

BACKGROUND: Systemic Lupus Erythematosus (SLE) disproportionately affects women, minorities, and individuals with low socioeconomic status. We hypothesized that counties with a higher percentage of disadvantaged individuals have a higher prevalence of SLE and increased acute hospital events, including emergency room (ER) visits and hospitalizations, among Medicare patients with SLE. METHODS: This cross-sectional study used the Centers for Disease Control and Prevention's Social Vulnerability Index (SVI) and Lupus Research Alliance's Lupus Index Medicare data. SLE was identified through Medica

[Does steroid therapy have an effect on the course of Löfgrenn syndrome? Catamnestic studies on 25 patients with Löfgren syndrome at the Zürich county hospital].

To investigate whether steroids influence the course and prognosis of acute sarcoidosis (Löfgren syndrome) 35 patients who developed this disease during the period 1960-1970 were examined and retrospectively divided into two groups, one of which had received steroids and the other not. At the onset of the disease all the patients had had typical symptoms of acute sarcoidosis: bilateral hilar lymphadenopathy, erythema nodosum, arthralgia, fever and elevated sedimentation rate. Biopsy of hilar lymphnodes had revealed non-caseating granulomas in all cases, whereas such granulomas were present in

Estimating the Burden of Clinically Significant Staphylococcus aureus Infections and Predictors for Hospitalization for Skin and Soft Tissue Infections, Fulton County, Georgia, 2017.

BACKGROUND: Incidence estimates of Staphylococcus aureus infections rarely include the full spectrum of clinically relevant disease from both community and healthcare settings. METHODS: We conducted a prospective study capturing all S aureus infections in Fulton County, Georgia, during 2017. Medical records of patients with any incident infection (clinical cultures growing S aureus from any site, without prior positive culture in previous 14 days) were reviewed. Estimates of disease incidence were calculated using age-, race-, and sex-specific population denominators accounting for weighted s

Tropical Cyclone Exposures and Risks of Emergency Medicare Hospital Admission for Cardiorespiratory Diseases in 175 Urban United States Counties, 1999-2010.

BACKGROUND: Although injuries experienced during hurricanes and other tropical cyclones have been relatively well-characterized through traditional surveillance, less is known about tropical cyclones' impacts on noninjury morbidity, which can be triggered through pathways that include psychosocial stress or interruption in medical treatment. METHODS: We investigated daily emergency Medicare hospitalizations (1999-2010) in 180 US counties, drawing on an existing cohort of high-population counties. We classified counties as exposed to tropical cyclones when storm-associated peak sustained winds

A case-crossover analysis of the relationship of air pollution with out-of-hospital sudden unexpected death in Wake County, North Carolina (2013-2015).

Out-of-hospital sudden unexpected deaths are non-accidental deaths that occur without obvious underlying causes and may account for 10% of natural deaths before age 65. Short-term exposure to ambient air pollution is associated with all-cause (non-accidental) and cause-specific (e.g., cardiovascular) mortality, and with immediate exposures often yielding the highest magnitude risk estimates. Few studies have focused on short-term exposure to air pollution and sudden unexpected deaths. Using the University of North Carolina Sudden Unexpected Death in North Carolina population, we examine associ

Perceived changes in health care personnel attitudes toward influenza vaccination since the COVID-19 pandemic: A Los Angeles County hospital survey.

Influenza vaccination among health care personnel has declined nationwide since the COVID-19 pandemic began in 2020. We surveyed acute care hospitals within Los Angeles County to examine worker attitudes regarding influenza vaccination and the impact of COVID-19. Comparing before and during the pandemic, we found increased safety and efficacy concerns, and decreased perception of disease severity and risk of influenza, suggesting that redundancy of established COVID-19 measures and pandemic fatigue may have contributed to vaccine refusal.
