The development of tuberculosis (TB) is dictated by a complicated interplay between antimicrobial defense and host-mediated immunopathology. Neutrophils are known to exhibit striking functional plasticity with phenotypes associated with both bacterial containment and tissue damage. However, the mechanisms that connect the granuloma microenvironment to neutrophil fate remain poorly understood. This review aims to fill this gap by integrating emerging evidence that defines immunometabolic adaptation as a key determinant of neutrophil plasticity in TB. The core of this analysis delineates how the

The 2022 global mpox outbreak and subsequent viral evolution have exposed critical limitations in current therapeutic strategies, which predominantly rely on repurposed smallpox drugs targeting a narrow range of viral proteins. This review provides a timely and comprehensive evaluation of the mpox antiviral landscape, spanning from established clinical agents to cutting-edge preclinical candidates. We critically analyse the 'clinical translation gap' observed in recent clinical trials (e.g., STOMP and PALM007), attributing the limited efficacy in mild cases to host immunopathological responses

PURPOSE OF REVIEW: To summarize recent advances in drug-resistant tuberculosis (DR-TB) treatment for people with HIV (PWH), including drug-drug interactions, investigational medications and host-directed therapy, as well as emerging evidence on novel treatment regimens, post-TB complications, and DR-TB medication resistance among PWH.
RECENT FINDINGS: Treatment for DR-TB has evolved to shorter, all-oral regimens with reduced drug-drug interactions. However, emerging dolutegravir resistance may necessitate protease inhibitor-based ART regimens resulting in interactions that complicate DR-TB ma

Antimicrobial resistance in ESKAPE pathogens is primarily attributed to resistance genes, yet persistent infections despite appropriate therapy implicate immune evasion as an independent driver of treatment failure. Although immune-evasion mechanisms have been extensively characterized in individual pathogens, their shared architecture across the ESKAPE group remains insufficiently integrated. This review synthesizes current evidence to show that phylogenetically diverse ESKAPE pathogens have convergently evolved conserved strategies to evade host immunity under comparable selective pressures.

Salmonella infections remain a priority concern in both developed and developing regions due to antimicrobial resistance and the lack of proper diagnosis. While substantial progress has been made in deciphering the pathogenesis of this ancient pathogen, emerging insights into host-pathogen interactions, particularly those mediated by post-translational modifications (PTMs) orchestrated by bacterial effectors, offer promising avenues for identifying novel drug targets and advancing host-directed therapies. The rise in antimicrobial resistance among priority Salmonella serovars, particularly in

Tuberculous meningitis (TBM) is the most severe complication of tuberculosis (TB) and is associated with high rates of disability and mortality. Mycobacterium tuberculosis (M. tb), the infectious agent of TB, disseminates from the respiratory epithelium, breaks through the blood-brain barrier, and establishes a primary infection in the meninges. Microglia are the core of the immune network in the central nervous system (CNS) and interact with glial cells and neurons to fight against harmful pathogens and maintain homeostasis in the brain through pleiotropic functions. However, M. tb directly i

Infectious diseases are posing threats to the world. Although several types of antibiotics and antivirals have been created to treat the diseases, emerging/re-emerging infectious diseases, as well as those caused by pathogens with multidrug resistance, remain to be significant challenges. As a new therapeutic approach, "host-directed therapy" that enhances immune responses of host cells has been proposed. Nevertheless, the agents used in this strategy often lead to a side effect of hyperinflammation, posing a challenge in developing safe and effective drugs. In this review, I suggest boosting

Classical antiviral therapy targets viral functions, mostly viral enzymes or receptors. Successful examples include precursor herpesvirus drugs, antiretroviral drugs that target reverse transcriptase and protease, influenza virus directed compounds as well as more recent direct antiviral agents (DAA) applied in the treatment of hepatitis C virus (HCV). However, from early times, the possibility of targeting the host cell to contain the infection has frequently re-emerged as an alternative and complementary antiviral strategy. Advantages of this approach include an increased threshold to the em

Tuberculosis (TB) treatment regimens are lengthy, causing non-adherence to treatment. Inadequate treatment can lead to relapse and the development of drug resistance TB. Furthermore, patients often exhibit residual lung damage even after cure, increasing the risk for relapse and development of other chronic respiratory illnesses. Host-directed therapeutics are emerging as an attractive means to augment the success of TB treatment. In this study, we used C3HeB/FeJ mice as an experimental model to investigate the potential role of rapamycin, a mammalian target of rapamycin inhibitor, as an adjun

Host-directed-therapy strategies are warranted to fight tuberculosis. Here we assess the safety and immunogenicity of adjunctive vaccination with the H56:IC31 candidate and cyclooxygenase-2-inhibitor treatment (etoricoxib) in pulmonary and extra-pulmonary tuberculosis patients in a randomized open-label phase I/II clinical trial (TBCOX2, NCT02503839). A total of 222 patients were screened, 51 enrolled and randomized; 13 in the etoricoxib-group, 14 in the H56:IC31-group, 12 in the etoricoxib+H56:IC31-group and 12 controls. Three Serious Adverse Events were reported in the etoricoxib-groups; two

Macrophages are versatile innate immune cells that play a crucial role in immune responses and tissue repair. However, their plasticity and central role in immunity also make them prime targets for viral manipulation. Viruses have evolved sophisticated mechanisms to modulate macrophage functions, disrupting cytokine secretion and phagocytosis to evade immune clearance, establish infection and promote persistence. For instance, some viruses drive excessive cytokine secretion, resulting in hyperinflammation and tissue damage, while other viruses suppress cytokine production and impair phagocytic

Chikungunya virus (CHIKV) is a mosquito-borne alphavirus causing acute fever, rashes, and even acute or chronic arthralgia/arthritis. With its expanding global distribution, CHIKV represents a significant public health threat, yet no specific antivirals are approved. The lifecycle of CHIKV is highly dependent on interactions with host factors that mediate viral entry, genome replication, protein translation, assembly, and release. A deeper understanding of these host factors provides new insights into CHIKV pathogenesis and offers strategies to overcome drug resistance associated with high vir

Helicobacter pylori is the most common cause of chronic gastric mucosal infections worldwide. The current global H. pylori infection rate is estimated at 50%. Untreated H. pylori colonization may lead to gastritis, peptic ulcers, and malignancies such as mucosa-associated lymphoid tissue (MALT) lymphoma and gastric cancer. H. pylori grows in the gastric mucosa by modulating host immunity, and can trigger a strong host immune response. The interaction between H. pylori and host immune defenses is complex, and the bacterium induces a gastric mucosal immunosuppressive microenvironment through mul

PURPOSE: Hypervirulent Klebsiella pneumoniae (hvKp) evades antibiotics and drives inflammation by surviving within macrophages. This study explored metformin (MET), a common AMP-activated protein kinase (AMPK) activator, as a host-directed therapy against hvKp.
PATIENTS AND METHODS: Using hvKp strain NTUH-K2044-infected THP-1 macrophages, assays measured intracellular bacterial survival, autophagy flux, phagocytosis, and cytokine expression. AMPK-autophagy involvement was tested with agonists/inhibitors and siRNA. A murine liver abscess model assessed MET's efficacy on bacterial burden and im

Tuberculosis caused by Mycobacterium tuberculosis (Mtb), results in significant disease and death worldwide. Host-directed therapy, including conventional drugs, is a promising antituberculosis strategy that shows synergistic antibacterial effects when combined with antituberculosis drugs. Here, the mycobactericidal effect of 3 antidiabetic drugs was examined. Of these, only troglitazone (Trog) enhanced the antimycobacterial effect in vitro and in vivo. This was due to Trog-mediated autophagy activation. Moreover, a knock-down experiment revealed that Trog activated autophagy and exhibited ant

Global increases in the prevalence of antimicrobial resistance highlight the urgent need for novel strategies to combat infectious diseases. Recent studies suggest that host metabolic pathways play a key role in host control of intracellular bacterial pathogens. In this study we explored the potential of targeting host metabolic pathways for innovative host-directed therapy (HDT) against intracellular bacterial infections. Through gene expression profiling in human macrophages, pyruvate metabolism was identified as potential key pathway involved in Salmonella enterica serovar Typhimurium (Stm)

Chronic graft-versus-host disease (cGvHD) can affect about 70% of people who undergo allogeneic hematopoietic stem cell transplantation (allo-HSCT). This condition is characterized by a complex immune dysregulation that leads to persistent inflammation and fibrosis. These complications can lead to long-term health issues that significantly affect the quality of life after HSCT. Conventional therapies such as corticosteroids and broad immunosuppressants are partially effective and have significant toxicity. They also fail to reverse established fibrotic damage. Advances in targeted pharmacologi

PURPOSE OF REVIEW: Tuberculous meningitis (TBM) is associated with significant mortality and morbidity yet is difficult to diagnose and treat. We reviewed original research published in the last 2 years, since 1 January 2018, which we considered to have a major impact in advancing diagnosis, treatment and understanding of the pathophysiology of TBM meningitis in children and adults.
RECENT FINDINGS: Studies have sought to identify a high sensitivity diagnostic test for TBM, with new data on modified Ziehl--Neelsen staining, urinary and cerebrospinal fluid (CSF) lipoarabinomannan and GeneXpert

OBJECTIVES: Klebsiella pneumoniae tops the WHO priority pathogen list, and the convergence of carbapenem resistance with hypervirulence is exhausting pathogen-directed therapy. Host-directed therapy (HDT) acts on host targets and is therefore largely indifferent to carbapenemases. The mechanisms by which K. pneumoniae remodels the host immune microenvironment are now well characterised, but their therapeutic corollary has not been assembled. We aimed to map each evasion node onto host-directed interventions and grade the supporting evidence.
METHODS: Four evasion mechanisms were mapped: suppr

Chronic hepatitis C virus (HCV) infection remains a critical global public health burden affecting hundreds of millions of people worldwide. The emergence of antiviral resistance persistently undermines the long-term clinical efficacy of current direct-acting antiviral (DAA) regimens, creating an urgent need for therapeutics with different mechanisms of action. Host-directed multi-target therapy offers a promising avenue to overcome DAA resistance, yet dual inhibitors targeting HCV-essential host kinase phosphatidylinositol 4-kinase IIIβ (PI4KIIIβ) and epigenetic regulator histone deacetylases
