Acute acalculous cholecystitis is defined as acute inflammation of the gallbladder in the absence of gallstones. Patients are usually critically ill with atherosclerotic heart disease, recent trauma, burn injury, surgery, or hemodynamic instability. The presentation of acute acalculous cholecystitis may be insidious, characterized by unexplained fever, leukocytosis, hyperamylasemia, or abnormal aminotransferases, and patients often lack right upper quadrant tenderness. Diagnostic evaluation includes ultrasonography, computerized tomography, and cholescintigraphy. Given the high mortality of un

Acute pancreatitis due to pancreatic exocrine over-secretion induced by organophosphate poisoning has been previously reported but parotid gland involvement has not. This paper describes a case of acute organophosphate-induced parotitis in a patient with pre-existing sialolithiasis. The patient developed bilateral facial swelling in the pre-auricular area extending to the angle of the jaw and also developed elevated serum amylase on the second day of the poisoning. Serum lipase remained normal. Autopsy confirmed parotid gland inflammation and pre-existing ductal lithiasis. This case illustrate

Reported is the case of a 64-year-old female who was admitted to our clinic because of hyperamylasemia. Duodenal endoscopic examination revealed a tumor at the papilla of Vater. A biopsy specimens taken from the top of the tumor showed a well-differentiated adenocarcinoma. Under the diagnosis of cancer of the papilla of Vater, pancreaticoduodenectomy was performed. The first histological diagnosis was carcinoma in adenoma, but from the results of nucleus-gland ratio examined from 6 parts of the "adenoma", all of which were over 0.50, we finally concluded this case as de nove well-differentiate

Two cases of intrathoracic tumor, different in histology and accompanied by hyperamylasemia, were studied ultrastructurally, histochemically, and biochemically. The ultrastructure of the tumor cell cytoplasm showed many zymogen granules in case 1 and smaller cored granules in addition to zymogen granules in case 2. Both tumors contained not only a large amount of amylase, which was electrophoretically of saliva type with three components, but also significant amounts of immunoreactive ACTH and beta-MSH. Starch film and immunofluorescence showed that the tumor cells stored amylase. It was concl

BACKGROUND AND PURPOSE: The laparoscopic-endoscopic rendezvous procedure, which integrates laparoscopic cholecystectomy with intraoperative endoscopic retrograde cholangiopancreatography (ERCP), is an established therapeutic strategy for patients with cholelithiasis and concomitant choledocholithiasis. This study evaluated the impact of ureteral catheter-assisted antegrade guidewire intubation on the procedural success rate and duration of the rendezvous technique, with additional assessment of safety and clinical efficacy.
METHODS: Clinical data were retrospectively analyzed from patients wi

Several viral infections, including mumps, coxsackie B, and infectious mononucleosis, have been associated with acute pancreatitis; however, varicella infection has not previously been associated with this complication. A 29-year-old man was admitted to the hospital with severe abdominal pain and hyperamylasemia several days following the onset of a characteristic varicella skin rash. His son had been diagnosed with varicella one week earlier. The hospital course was complicated by the development of pseudocysts in the head and tail of the pancreas which caused partial obstruction of the commo

A man in his 60s with hyperamylasemia underwent contrast-enhanced computed tomography, which revealed masses in his pelvic cavity on the right side and in the left axilla. Hence, a detailed examination was performed. Upon performing Sonazoid® (perfluorobutane) contrast-enhanced ultrasound, it was discovered that the right-sided pelvic cavity mass exhibited centripetal contrast-enhancement right from the early stage. Subsequently, the contrast material disappeared from the center and was washed out in the postvascular phase. The mass was suspected to be caused by vascular malformations. The rig

BACKGROUND: The 2019 European Society of Gastrointestinal Endoscopy guidelines recommend advanced endoscopic techniques or laparoscopic surgery for the management of difficult common bile duct (CBD) stones (DCBDSs). A treatment approach that focuses on minimizing inflammation while preserving liver function may enhance decision-making and improve patient outcomes. This study aimed to compare clinical outcomes, inflammatory markers, and liver function tests (LFTs) in patients with cholelithiasis (CL) and concomitant DCBDSs who underwent either laparoscopic CBD exploration (LCBDE) followed by la

The case of a 36-year-old man with a villous adenoma of the duodenum is presented. His history included jaundice, pruritus, and weight loss. Physical examination was otherwise unremarkable. Laboratory data included anemia, guaiac-positive stool, hyperamylasemia, and abnormal liver function tests. Biliary ductile dilation was seen on ultrasonography; a filling defect in the duodenum was seen on CT scan, and endoscopic biopsy revealed villous adenoma with dysplasia. Pancreatoduodenectomy was performed and recovery was uneventful. Histopathology confirmed moderate dysplasia. Review of the literat

This report describes a case of pancreatic panniculitis secondary to acute pancreatitis caused by biliary stricture following multiple biliary surgeries. The aim is to highlight the importance of recognizing atypical presentations. The patient, a 46-year-old woman, presented with erythematous macules and nodules on the extremities accompanied by polyarthralgia. She had a history of multiple biliary surgeries over the past decade. Histopathological examination of the skin lesions revealed no significant epidermal abnormalities but demonstrated perivascular neutrophilic infiltration in the dermi

Pancreatic intraepithelial neoplasia (PanIN) is one of the most important issues for the early detection of pancreatic ductal adenocarcinoma. In particular, PanIN-3 is recognized as a precancerous lesion, e.g., carcinoma in situ, high-grade dysplasia, and severe dysplasia. We report a rare, completely resected case of PanIN-3 in the main pancreatic duct (MPD) detected from localized pancreatitis. A 63-year-old man developed upper abdominal pain with hyperamylasemia. He underwent distal pancreatectomy soon after recovery because an abnormal narrow segment, suggesting PanIN, was identified in th

We treated a 49 year-old woman with acute pancreatitis, in whom there was an accessory pancreatic duct which opened into a duodenal duplication cyst. Epigastric pain associated with vomiting and fever were present. Laboratory data showed leukocytosis and hyperamylasemia. An upper G-I series revealed a stricture at the pyloric region. At operation a spherical mass of 6 cm in diameter was present between the greater curvature of the pyloric region and the head of the pancreas. The cyst was removed by hemigastrectomy and partial resection of the head of the pancreas. The accessory pancreatic duct

A 23-year-old Japanese man was admitted to our hospital because of dry cough and lacrimal gland swelling. Laboratory findings showed hypergammaglobulinemia (4859 mg/dl), hypocomplementemia (CH50 13 U/ml), and hyperamylasemia. CT revealed a marked swelling of the bilateral lacrimal glands, diffuse patchy infiltration in the bilateral lung fields, and enlargement of the whole pancreas. Gallium citrate scintigraphy showed abnormal accumulation in the bilateral lacrimal glands, submandibular glands, and both lung fields. Biopsy specimens from the salivary gland revealed dense lymphoplasmacytic inf

Double balloon enteroscopy (DBE) is a new technique, first published and introduced into clinical practice in 2001 by Yamamoto, the inventor of this outstanding method. DBE allows complete visualization, biopsy and treatment of the small bowel. Nowadays, we have some experience of this method for evaluation of the complication rate. Severe complications are described in 1%-1.7% of patients. Acute pancreatitis is a rare complication of the investigation. The incidence of acute pancreatitis after diagnostic DBE is 0.3% in most studies. More than 50 cases of acute pancreatitis have been described

Cystic dystrophy in heterotopic pancreas (CDHP) is characterized by the presence of cystic formations surrounded by inflammation and scarring. It usually involves the duodenal wall and can be responsible for strictures and pain. The diagnosis of this disorder was previously based on pancreatoduodenectomy specimens removed for a suspected pancreatic tumor. Six cases were observed in young men (mean age 40 years) between 1989 and 1993. Computed tomography (CT) and endoscopic ultrasonography (EUS) features allowed definitive preoperative diagnosis of CDHP. After surgical resection of the tissue-b

Heterotopic pancreas (HP) is a relatively uncommon entity that is defined as pancreatic tissue without a true anatomical or vascular connection to the pancreas. HP does not cause symptoms in most cases but can occasionally produce various manifestations, including nausea, vomiting, abdominal pain, and even heterotopic pancreatitis. Here, we report an unusual case in which heterotopic pancreatitis complicated by the formation of a pseudocyst that caused gastric outlet obstruction was diagnosed based on serum hyperamylasemia and findings from endoscopic ultrasonography (EUS)-guided fine needle a
