Effect of Cilastatin on Cisplatin-Induced Nephrotoxicity in Patients Undergoing Hyperthermic Intraperitoneal Chemotherapy.

Cisplatin is one of the most widely used chemotherapeutic agents in oncology, although its nephrotoxicity limits application and dosage. We present the results of a clinical study on prophylaxis of cisplatin-induced nephrotoxicity in patients with peritoneal carcinomatosis undergoing cytoreduction and hyperthermic intraperitoneal intraoperative chemotherapy (HIPEC-cisplatin). Prophylaxis was with imipenem/cilastatin. Cilastatin is a selective inhibitor of renal dehydropeptidase I in the proximal renal tubule cells that can reduce the nephrotoxicity of cisplatin. Unfortunately, cilastatin is no

Delayed genital necrosis after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy with Mitomycin-C.

INTRODUCTION: Genital necrosis (GN) is a rare complication of cytoreductive surgery with hyperthermic intraoperative chemotherapy (CRS/HIPEC) which can be confused with necrotizing fasciitis. We present an analysis of GN after CRS/HIPEC to define its natural history. METHODS: We identified patients with GN after CRS/HIPEC at two peritoneal surface malignancy institutions. Patient demographic, surgical, and postoperative data were extracted from prospective databases. RESULTS: Of 1597 CRS/HIPECs performed, 13 patients (0.8%) had GN. The median age was 57 years (IQR: 49-64) and 77% (n = 10) we

Role of prophylactic hyperthermic intraperitoneal chemotherapy in patients with locally advanced gastric cancer.

BACKGROUND: Gastric cancer is the second most common malignant tumor in China, ranking third among all malignant tumor mortality rates. Hyperthermic intraperitoneal chemotherapy (HIPEC) has been shown to increase significantly the effectiveness of intraperitoneal chemotherapeutic drugs, prolong the action time of these drugs on intraperitoneal tumor cells, and enhance their diffusion in tumor tissues. HIPEC may be one of the best choices for the eradication of residual cancer cells in the abdominal cavity. AIM: The aim of this study was to study the role of preventive HIPEC after radical gast

Sodium thiosulfate protects from renal impairement following hyperthermic intraperitoneal chemotherapy (HIPEC) with Cisplatin.

BACKGROUND: Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC) has been shown to provide benefits in the management of peritoneal metastasis. Cisplatin (CDDP) is one of the most frequently used drugs for peritoneal infusion. A major restriction is that CDDP causes renal toxicity and acute renal failure, sometimes leading to chronic renal failure. The aim of the present study was to assess the impact of sodium thiosulfate (ST) in preventing renal impairment (RI) following HIPEC with CDDP. METHODS: This prospective study assessed the RI rates for all patients

Incidence and predictors of incisional hernia after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

Background and objectives: The incidence of incisional hernia (IH) after cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is largely unknown. Methods: We conducted a retrospective study to identify patients who underwent CRS/HIPEC from 2001 to 2016. Patients were followed postoperatively for a minimum of two years. The primary outcome was the occurrence of an IH identified either on CT scan or physical examination. Univariate and multivariable logistic regression models were used to test associations with IH. Results: We identified 155 patients who underwent CRS

Readmissions After Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy: a US HIPEC Collaborative Study.

BACKGROUND: Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) results in significant morbidity and readmissions. Previous studies have been limited by single-institution design or lack of tumor details in the database used. METHODS: The 12-institution US HIPEC Collaborative Database was queried between 1999 and 2017. Preoperative and intraoperative patient and tumor details were analyzed for associations with readmissions. RESULTS: A total of 2017 of 2372 cases were included in the analysis. The 30-day readmission rate was 15.9% (n = 321). Common indications for

[Complications of hyperthermic intraperitoneal chemotherapy: about a case of acute respiratory distress syndrome].

For about twenty years, extensive cytoreduction surgery associated to hyperthermic intraperitoneal chemotherapy (HIPEC) has been used to treat rare peritoneal tumors (mesothelioma, pseudomyxoma) and some digestive and gynecological neoplasias infiltrating the peritoneum. This heavy technique is associated to significant morbi-mortality but allows to increase lifetime and to improve quality of life of patients with these tumours. Complications of this technique are principally surgical, but hyperthermic intraperitoneal chemotherapy can result in toxic effects, especially for kidney and bone mar

Hepatocellular Carcinoma Peritoneal Metastasis: Role of Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy (HIPEC).

INTRODUCTION: Peritoneal dissemination of hepatocellular carcinoma (HCC) is a rare presentation with an incidence of 2-6%. The most common cause of peritoneal deposits is a ruptured HCC that results in tumor spillage into the peritoneal cavity. The overall incidence of spontaneous ruptures of HCC ranges from 5 to 15% and carries a high mortality rate of up to 50%. Other factors influencing peritoneal dissemination are the lymph node metastasis and the direct diaphragmatic invasion and there is no significant association with past history of FNAB, or percutaneous RFA or ethanol injection and lu

Hepatocellular carcinoma with peritoneal metastasis treated with cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy.

Peritoneal carcinomatosis from hepatocellular carcinoma is well regarded as a poorly treatable malignant disease with rapid decline. Over the past decade, new modalities that combine cytoreductive surgery with perioperative hyperthermic intraperitoneal chemotherapy have been shown to be aggressive local-regional treatments with improved survival. We present a case of a 67-year-old non-cirrhotic woman with peritoneal metastases from hepatocellular caracinoma who was treated with cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy. Consideration remains individualized, but it ca

Iterative procedures combining cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for isolated peritoneal recurrence.

PURPOSE: The aim of this study was to analyse feasibility, morbidity and outcome of repeat complete cytoreductive surgery (CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC). CRS combined with HIPEC is becoming the gold standard treatment for resectable peritoneal carcinomatosis in highly selected patients. As yet treatment of isolated peritoneal recurrence with iterative CRS and HIPEC has not been thoroughly explored. MATERIALS AND METHODS: We selected 16 patients presenting isolated peritoneal recurrence who had undergone iterative CRS and HIPEC from a dataset of 322 CRS associated

Quality of life after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy: an Asian perspective.

BACKGROUND: Data on quality of life (QOL) after cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS + HIPEC) is scarce in the Asian population. This study assesses QOL outcomes after CRS and HIPEC in an Asian cancer center. METHODS: Patients who completed CRS + HIPEC 6-18 months ago (27 patients) were enrolled in the study. QOL was measured via the European Organization for Research and Treatment of Cancer QLQ-C30 questionnaires. The scores were compared with a group of 393 disease-free cancer patients, not on active treatment, who had ECOG scores of either 0 or 1. The 1

Successful palliation of malignant ascites from peritoneal mesothelioma by laparoscopic intraperitoneal hyperthermic chemotherapy.

A variety of options have been proposed to treat malignant ascites but most of them have failed to reach a significant impact in terms of palliation. Laparoscopic hyperthermic intraperitoneal chemotherapy (LHIPEC) could represent a good therapeutic tool for patients in whom medical therapies have failed and peritoneovenous shunting is contraindicated. Here we present a case of a 49-year-old woman with malignant ascites secondary to peritoneal spreading of a right pleural mesothelioma. After failure of medical therapy, the patient underwent LHIPEC with Cisplatin 25 mg/m/L and Doxorubicin 7 mg/m

Multiple Recurarization Episodes After Sugammadex Reversal in Hyperthermic Intraperitoneal Chemotherapy: A Case Report.

Sugammadex reverses rocuronium-induced neuromuscular blockade via 1:1 molar encapsulation. Recurarization is rare when sugammadex is dosed as recommended. We report a 53-year-old woman undergoing hyperthermic intraperitoneal chemotherapy (HIPEC) who developed three sequential recurarization episodes despite an initial sugammadex dose (5.4 mg/kg), ultimately requiring 1400 mg cumulatively. HIPEC-associated hyperthermia and a prolonged rocuronium infusion likely promoted peripheral-to-central redistribution of rocuronium and overwhelmed free sugammadex in plasma. In patients with large cumulativ

Perioperative hemoglobin trajectories and transient hemodilution during cytoreductive surgery with hyperthermic intraperitoneal chemotherapy.

Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (HIPEC) for peritoneal malignancies involves heated intraperitoneal cisplatin perfusion and liberal intravenous hydration, both of which may affect perioperative hemoglobin (Hb) concentrations. This study aimed to characterize Hb trajectories during HIPEC and determine whether these patterns differ between laparoscopic and open procedures. We retrospectively analyzed 186 HIPEC procedures performed between 2018 and 2023, including 107 laparoscopic and 79 laparotomy cases. Hb concentrations were assessed before HIPEC, from HIPE

Perfusion Instability During Hyperthermic Intraperitoneal Chemotherapy: The Utility of a Problem-solving Flowchart.

BACKGROUND/AIM: During hyperthermic intra-peritoneal chemotherapy (HIPEC), perfusion instability (PI) is defined as the inability to maintain a proper perfusion flow without impairment of the target temperature. The management and resolution of this adverse event is underreported and poorly investigated. The study aimed to evaluate the incidence of PI during closed cytoreductive surgery (CRS)-HIPEC and how a problem-solving approach might limit the effects of this adverse event. PATIENTS AND METHODS: A retrospective analysis of patients who underwent CRS-HIPEC at our Institution was performed
