A patient with sickle cell trait who presented with gross hematuria and was subsequently found to have renal papillary necrosis is presented. The hematuria resolved with conservative therapy consisting of bed rest and hydration with hypotonic intravenous fluids. The pathophysiology of renal abnormalities associated with sickle cell trait is described. The management of the primary clinical manifestations of this disorder, hematuria and papillary necrosis, are discussed.

We screened for increased osmotic fragility of erythrocytes in 1464 healthy German blood donors. The phenomenon of cell volume recovery following a hypo-osmotic stress mediated by intracellular osmolyte regulation is well known. In many, perhaps all, cell types, the osmolytes involved are usually inorganic ions and amino acids. The details of the regulatory mechanisms for the organic-type osmolytes are not well known. The history of intensive care in gastroenterology is briefly discussed. The insertion of intensive care units in the hospital scene is also examined and illustrative examples of emergency cases treated in recent years at the Rome United Hospitals gastroenterology division are presented. It is felt that centres tailored to hospital dimensions should be formed, though their cost is likely to prove a bar to their early institution. Dysnatremias are among the most common electrolyte abnormalities encountered in hospitalized patients. In most cases, a dysnatremia results from improper fluid management. Dysnatremias can occasionally result in death or permanent neurological damage, a tragic complication that is usually preventable.
