Hypokalemic nephropathy after pelvic pouch procedure and protective loop ileostomy.

Proctocolectomy with ileal pouch-anal anastomosis and temporary ileostomy has been established as a curative operation in severe ulcerative colitis during the last 2 decades. Electrolyte imbalances during the first postoperative weeks until ileostomy closure have been reported previously. Here we report about a 70-year-old male patient with a 38 year-history of severe ulcerative colitis who developed slowly progressive renal failure after proctocolectomy with ileal pouch-anal anastomosis and temporary ileostomy. He was referred to our centre with a serum creatinine of 818 micromol/L, hypokalem

T-tube ileostomy for meconium ileus: four decades of experience.

BACKGROUND/PURPOSE: The T-tube ileostomy was first used at Texas Children's Hospital in 1959. The purpose of this study is to update the experience since the initial report of this technique in 1981. METHODS: A database of 448 patients with cystic fibrosis (CF) seen in the authors' institution was used to identify 83 patients (18.5%) who presented with meconium ileus. The clinic and hospital charts of these patients were reviewed retrospectively to identify patients who had undergone placement of a T-tube ileostomy. RESULTS: Surgery was performed in 60 of 83 patients for complications of mec

Chronic hepatic encephalopathy following portacaval shunt: management by loop ileostomy.

A 69 year old woman developed intractable episodic hepatic encephalopathy 12 years after an end-to-side portacaval shunt for variceal haemorrhage. Medical management was ineffective in preventing repeated episodes of encephalopathy and caused incapacitating faecal incontinence. A loop ileostomy was created with minimal morbidity and was effective in preventing further episodes of encephalopathy over a follow-up period of 33 months. The patient returned to a normal diet and was able to be discharged home from institutional care. Loop ileostomy is an alternative in the management of patients wit

Fecal calprotectin from ileostomy output in patients with Crohn's disease.

BACKGROUND: Fecal calprotectin (FC) is a reliable biomarker widely used for assessing disease activity and postoperative monitoring in patients with Crohn's disease (CD); however, its efficacy in patients with an ileostomy is poorly understood. Our study evaluated whether FC from the ileostomy output can be used to predict postoperative small bowel inflammation in patients with CD. METHODS: Data from patients with CD and an ileostomy who had undergone FC measurement between January 1, 2015, and December 30, 2022, were analyzed retrospectively. Patients were enrolled in the study if they had u

Flexible endoscopy is enough diagnostic prior to loop ileostomy reversal.

PURPOSE: This study investigates whether contrast enema (CE) and flexible endoscopy (FE) should be performed routinely after low anterior resection (LAR) before ileostomy reversal. Additionally, the impact of previous anastomotic leakage (AL) on diagnostic test accuracy (DTA) was assessed. METHODS: This is a retrospective analysis of prospectively collected tertiary care data of two centers. Consecutive rectal cancer patients undergoing LAR with loop ileostomy formation were included. Before ileostomy reversal, all patients were assessed by CE and FE. DTA of FE and CE for asymptomatic AL in p

Percutaneous transgastric endoscopic tube ileostomy in a porcine survival model.

AIM: To introduce natural orifice transgastric endoscopic surgery (NOTES) tube ileostomy using pelvis-directed submucosal tunneling endoscopic gastrostomy and endoscopic tube ileostomy. METHODS: Six live pigs (three each in the non-survival and survival groups) were used. A double-channeled therapeutic endoscope was introduced perorally into the stomach. A gastrostomy was made using a 2-cm transversal mucosal incision following the creation of a 5-cm longitudinal pelvis-directed submucosal tunnel. The pneumoperitoneum was established via the endoscope. In the initial three operations of the s

Defunctioning cannula ileostomy after lower anterior resection of rectal cancer.

BACKGROUND AND OBJECTIVE: Most surgeons suggest the use of fecal diversion in patients undergoing low anterior resections of rectal tumors at high risk for anastomotic leakage. We describe an exploratory study to evaluate the efficacy and safety of a new diversion method called a spontaneously closing cannula ileostomy, which was designed to protect rectal anastomoses in patients at high risk for anastomotic leakage. The outcomes of patients treated with cannula ileostomy were compared to those of patients treated with loop ileostomy. MAIN OUTCOME MEASURES: Outcomes included the rates of anas

Endoscopy-assisted minimally invasive loop ileostomy after previous restorative proctocolectomy.

BACKGROUND: Complications after proctocolectomy with ileal pouch-anal anastomosis may impair pouch function. Loop ileostomy can be performed to allow recovery of function before intestinal continuity is restored. To minimize operative trauma and to form the stoma with as little damage to the abdominal wall as possible, the invasiveness of the procedure should be minimized as far as possible. OBJECTIVE: To minimize the extent of surgical invasion, we developed a technical modification of loop ileostomy in which lower endoscopy is used to assist the procedure. PROCEDURE: The procedure requires

Laparoscopic loop ileostomy reversal: reducing morbidity while improving functional outcomes.

INTRODUCTION: Loop ileostomy reduces the morbidity associated with pelvic sepsis. However, its reversal carries a 10% to 30% complication rate. We present our technique for laparoscopic ileostomy closure. METHODS: We conducted a retrospective chart review of subjects undergoing laparoscopic-assisted loop ileostomy closure between 2006 and 2009. Operating time, length of hospital stay, return of bowel function, and complication rates were assessed. RESULTS: There were 24 (13 males) patients. Average age was 63 with a BMI of 25.9. Eighteen (75%) had a planned loop ileostomy, and 6 (25%) were e

Distal ileostomy drip feedings in neonates with short bowel syndrome.

When a newborn infant presents with high intestinal atresia, the proximal segment of the bowel is usually grossly distended and atonic. The anastomosis of this segment to the smaller and unused distal segment will usually result in little or no propulsion of contents distally. Many techniques have been employed to correct this problem. A common surgical approach is immediate end-to-end anastomosis, followed by parenteral alimentation until return of normal function. This can take many weeks, and requires special attention to fluid loss and complications associated with parenteral alimentation.

Analysis of diverting ileostomy for colorectal cancer surgery: stenosis and T4 invasion are risk factors of reoperation due to anastomotic leakage, even when ileostomy is performed.

PURPOSE: In colorectal cancer (CRC), diverting ileostomy prevents the occurrence and severity of anastomotic leakage (AL) during surgery. However, an ileostomy cannot prevent reoperation due to severe AL in some cases, and an approach other than ileostomy may be required. This study identified the risk factors of AL and reoperation due to AL in patients with diverting ileostomies. METHODS: Patients diagnosed with CRC who underwent resection surgery accompanied by diverting ileostomy between January 2015 and December 2023 were included. We analyzed the risk factors for AL and reoperation due t

Association between the protrusion of ileostomies and peristomal dermatitis: an observational study.

OBJECTIVE: This is an observational study to explore possible changes in end-ileostomy stoma protrusion through years of modified surgical methods. MATERIALS AND METHODS: Patients with permanent end-ileostomy were eligible for inclusion. Patients were recruited from two outpatient clinics in South-East Norway and from the Norwegian society for patients with ostomy (NORILCO). Stoma protrusion was measured in upright and supine positions by study nurse (outpatient cohort) or by the patient (NORILCO cohort). Clinical information was obtained through patients' questionnaire replies. For analyses,

Pharmacological Strategies for High-Output Ileostomies in Children: A Narrative Review.

AIM: Nutritional measures often suffice for managing high-output ileostomy (HOI) in paediatric patients, but pharmacological treatment may be required to control ostomy output. This paper reviews the literature on the pharmacological management of paediatric HOI and provides recommendations. METHODS: PubMed and Embase were searched for relevant articles up to 22 May 2024. Relevant articles were reviewed and recommendations were formulated. RESULTS: No pharmacological studies involving paediatric ileostomy patients were identified, apart from two case reports on octreotide. The formulated rec

Evaluating a novel intestinal tampon for adults with ileostomy: pilot study.

INTRODUCTION: Approximately 1 million people live with ileostomies and rely on stoma bags in their daily lives. They do not have access to alternative products. To address alternatives, InterPoc™, an absorbent intestinal tampon, has been developed. However, its safety and efficacy in humans have not yet been assessed. AIM: This study aimed to evaluate the safety and efficiency of InterPoc by assessing pain, vital parameters, mucosal lesions and tampon insertion/removal. METHOD: Eight adults with ileostomies were enrolled in a pilot safety study. InterPoc was inserted into the stoma and staye

Diverting ileostomy for treatment of ileoanal pouch dysfunction: a technical note.

BACKGROUND: The ileal pouch-anal anastomosis (IPAA) is a restorative procedure performed after proctocolectomy to improve quality of life in patients with colorectal conditions like ulcerative colitis, familial adenomatous polyposis, and selected cases of Crohn's disease and Lynch syndrome. However, severe pouch dysfunction can occur, often necessitating further surgical intervention. OBJECTIVE: This technical note aims to describe the operative approach and perioperative management for diverting ileostomy as a treatment for dysfunctional ileoanal pouches. METHODS: Indications for the proced

Peritoneal Dialysis in an Extremely Low Birth Weight Neonate with Ileostomy.

BACKGROUND: Acute kidney injury (AKI) is a common complication in sick, extremely low birth weight (ELBW) neonates. Peritoneal dialysis (PD) is the treatment modality but is seldom attempted in this patient population. We present a 40-day-old ELBW neonate previously operated for necrotizing enterocolitis with ileostomy who developed AKI. Peritoneal dialysis was started by a modifying intercostal drain. RESULTS: Doing a simple procedure and using a few modifications helped the baby to come out of acute kidney injury. CONCLUSION: Peritoneal dialysis can be technically quite challenging in ELBW
