Clinical and literature insights into the frontotemporal dementia and motor neuron disease spectrum.

BACKGROUND: Frontotemporal dementia represents a heterogeneous group of neurodegenerative disorders primarily affecting the frontal and temporal lobes. The overlap between FTD and motor neuron disease is increasingly recognized, presenting a complex clinical syndrome characterized by progressive cognitive, behavioral, and motor decline. CASE PRESENTATION: We describe a 69-year-old patient with a 4-year history of excessive ambulation. Over the last year, behavioral changes including disorganized conduct, irritability, spitting, and cold water foot immersion developed. The patient experienced

Successful Management of Severe Exertional Heat Stroke with Endovascular Cooling After Failure of Standard Cooling Measures.

BACKGROUND: Exertional heat stroke (EHS) is a potentially life-threatening emergency requiring rapid reduction in core body temperature. Methods of cooling include cold water immersion, ice packs, cold water lavage, and chilled saline, among others. We report a case of EHS successfully cooled using an endovascular cooling device after traditional cooling methods failed to reduce core body temperature. CASE REPORT: A 24-year old soldier collapsed during a 12-mile foot march while training in southern Georgia. His initial rectal temperature was 43.1°C (109.6°F). External cooling measures (ice s

Beep tones attenuate pain following Pavlovian conditioning of an endogenous pain control mechanism.

Heterotopic noxious counter-stimulation (HNCS) is commonly used to study endogenous pain control systems. The resulting pain inhibition is primarily based on spinal cord-brainstem loops. Recently, functional imaging studies have shown that limbic structures like the anterior cingulate cortex and amygdala are also implicated. Since these structures are involved in learning processes, it is possible that the HNCS-induced pain inhibition may depend on specific cues from the environment that have been associated with pain reduction through associative learning. We investigated the influence of Pav

Koroška 8000 Himalayan expedition: digit responses to cold stress following ascent to Broadpeak (Pakistan, 8051 m).

PURPOSE: Cold-induced vasodilatation (CIVD) is a peripheral blood flow response, observed in both the hands and feet. Exercise has been shown to enhance the response, specifically by increasing mean skin temperatures (Tsk), in part due to the increased number of CIVD waves. In contrast, hypobaric hypoxia has been suggested to impair digit skin temperature responses, particularly during subsequent hand rewarming following the cold stimulus. This study examined the combined effect of exercise and hypobaric hypoxia on the CIVD response. We compared the CIVD responses in the digits of both the han

Cortical and spinal assessment - a comparative study using encephalography and the nociceptive withdrawal reflex.

BACKGROUND: Standardized objective methods to assess the analgesic effects of opioids, enable identification of underlying mechanisms of drug actions in the central nervous system. Opioids may exert their effect on both cortical and spinal levels. In this study actions of morphine at both levels were investigated, followed by analysis of a possible correlation between the cortical processing and spinal transmission. METHODS: The study was conducted after a double-blinded, two-way crossover design in thirty-nine healthy participants. Each participant received 30mg morphine or placebo as oral s

The impact of a cold pressor test on brachial artery handgrip exercise-induced flow-mediated dilation.

It is unknown how endothelial-dependent flow-mediated dilation (FMD) stimulated by a sustained, exercise-induced increase in shear stress (EX-FMD) is affected by a simultaneous sympathoexcitatory painful stimulus. The purpose of this study was to examine the impact of a cold pressor test (CPT) on brachial artery EX-FMD elicited by a handgrip exercise-induced increase in shear stress. Participants were healthy males (age 21±2 years) (n=28; 16 Experimental group, 12 Control). Brachial artery diameter and blood velocity were measured using echo and Doppler ultrasound, respectively. Shear stress w

Epidemiology of U.S. Army cold weather injuries, 1980-1999.

BACKGROUND: Cold weather injuries (CWI) are of great military concern due to their wide-ranging impact on military readiness. Previous short-term studies have identified CWI to be more prevalent in African-Americans, infantrymen, and lower-ranking soldiers. The purpose of this cross-sectional study was to determine the occurrence of CWI hospitalizations in the U.S. Army from 1980 to 1999, and to identify possible trends, high-risk groups and/or activities. METHODS: The U.S. Army Research Institute of Environmental Medicine Total Army Injury and Health Outcomes Database was searched for hospit

Identification of complex regional pain syndrome in the upper limb: Skin temperature asymmetry after cold pressor test.

BACKGROUND: Skin temperature asymmetry (SkTA) may assist in early identification of complex regional pain syndrome (CRPS), but previous work has been limited by methodological shortcomings including failure to account for the cutaneous nerve distribution where temperature is measured and reliance on laboratory equipment not clinically available. Pilot work suggested that a cold pressor test (CPT) provided a consistent thermoregulatory stress and might increase sensitivity/specificity of SkTA measurements generated reliably by handheld infrared (IR) thermometers. AIMS: This study investigated

Evaluating the Impact of Optokinetic Stimulation on Weight Balance and Muscle Strength in Healthy Individuals in Virtual Reality During Squats.

Background Asymmetry in lower limb muscle strength and weight-bearing imbalance, commonly observed among athletes, individuals with unilateral impairments, and stroke survivors, require targeted interventions to correct left-right imbalances. Such corrections are essential for enhancing athletic performance, minimizing injury risks in athletes, and improving gait functionality and daily activity efficiency in patients. This study explored, in healthy participants, the potential of combining squat training with optokinetic stimulation (OKS) in an immersive environment to improve left-right bala

[Stonefish stings: difficult analgesia and notable risk of complications].

INTRODUCTION: Marine activities and water sports in tropical countries entail some dangers. In our emergency department, stonefish (Synancae verrucosa) stings are second only to those by sea urchins among the 400 consultations a year for wounds by marine animals. METHOD: We retrospectively collected data for all emergency department visits for stonefish stings over a 5-year period from 2001 through 2005. RESULTS: The study included 57 patients (42 men; mean age 31.2+/-15.9 years, range 3-63 years) at 61 consultations. The reason for the emergency department visit was pain, often described as

Low-Cost Spray-Patterned Triboelectric Textiles for Wearable Interaction and Energy Harvesting.

Smart textile interfaces hold promise for battery-free wearable interaction, yet their adoption is limited by complex fabrication and insufficient on-body evaluation. We present TriboTex, a low-cost spray-patterning workflow that forms nylon-Cu-nylon triboelectric stacks on cotton textiles using laser-cut PET stencils and commercially available materials. The core consumables cost approximately USD 0.003/cm2, and sensor geometry can be rapidly iterated by modifying only the digital stencil. Controlled characterization across nine devices from three fabrication batches showed a peak open-circui

Effects of diabetes mellitus on step length and minimum toe clearance adaptation.

BACKGROUND: Adaptive gait involves the ability to adjust the leading foot in response to the requirement of dynamic environments during walking. Accurate adjustments of the minimum toe clearance (MTC) height and step length can prevent older people from falling when walking and responding to hazards. Although older people with diabetes fall more frequently than healthy older adults, no previous studies have quantified their adaptive gait abilities. This study aimed to investigate the effects of diabetes mellitus on step length and MTC height adjustments using a non-immersive virtual-reality sy

The effect of local mild cold exposure on pulse transit time.

Pulse transit time (PTT) is a simple, noninvasive technique which shows great promise in the continuous monitoring of blood pressure and the assessment of arterial stiffness, and has potential applications in wearable healthcare devices. Usually, PTT is measured from the electrocardiogram (ECG) R-wave to a characteristic point on the peripheral pulse by photoplethysmography. However, peripheral blood circulation is sensitive to local temperature variation. This study investigated the effect of mild cold exposure on PTT by immersing one finger into cold water. The photoplethysmographic (PPG) si

Discrepancies in CDE and Measured Phaco Tip Energy: Comparison of Energy Produced in Longitudinal and Torsional Ultrasound Using Calorimetry.

PURPOSE: This study aims to investigate the differences between the heat energy produced during cataract surgery and Cumulative dissipated energy (CDE). CDE is often used as a medium for understanding the energy delivered to the eye during cataract surgery. However, the actual energy produced at the tip level of the tip is not well understood. We propose that a discrepancy may exist between the CDE reported by the surgical machine and the actual energy delivered to the eye. METHODS: About 50 mL of distilled water were degassed and placed in an isobaric calorimeter. Using the Alcon Centurion a
