Management of pulmonary toxicity associated with immune checkpoint inhibitors.

Immunotherapy has become a standard of care in oncology, following the recent approvals of cytotoxic T-lymphocyte-associated protein-4 and programmed cell death-1 inhibitors in lung cancer, melanoma, renal cell carcinoma, Hodgkin's lymphoma, bladder, head and neck cancers. Besides their efficacy, these agents also generate specific immune-related adverse events. Due to the increasing prescription of immune-checkpoint inhibitors, the incidence of immune toxicity will continue to rise. The awareness of immune-related adverse events is key to ensuring both diagnosis and management of the possible

Immune checkpoint inhibitors and the development of granulomatous reactions.

Immune checkpoint inhibitors (ICPIs) have emerged as a frontline treatment for a growing list of malignancies. Disruption of the negative regulatory immune checkpoints by ICPIs has been associated with many immune-related adverse events. Granulomatous reactions, such as sarcoidosis-like reactions, granulomatous panniculitis, granuloma annulare, and granulomatous dermatitis, are uncommon but increasingly recognized immune-related adverse events seen in patients treated with ICPIs. The frequency and significance of these eruptions, including whether they portend responsiveness to treatment, rema

The immunological paradox: immune checkpoint inhibitors in liver transplantation.

The incorporation of immune checkpoint inhibitors (ICIs) into the treatment repertoire for hepatocellular carcinoma (HCC) and advanced biliary tract cancers has significantly transformed the field of oncology, offering remarkable survival advantages. However, the interaction of these powerful immunomodulators with liver transplantation (LT), the ultimate curative intervention for end-stage liver disease, presents a complex immunological paradox. While LT depends on the induction and maintenance of immune tolerance to suppress alloimmune responses and prevent allograft rejection, ICIs operate b

[Primary resistance mechanisms of immune checkpoint inhibitors in cancer].

In recent years, immune checkpoint inhibitor (ICI) therapy has achieved a breakthrough in cancer treatment. By targeting and blocking immune checkpoint molecules on T cells, ICIs release inhibited anti-tumor immune responses, demonstrating durable clinical efficacy and improved long-term survival across multiple cancer types. However, only a minority of cancer patients currently benefit from ICI treatment, with primary resistance observed in most malignancies. Clarifying the mechanisms underlying primary resistance to ICI therapy is conducive to the development of effective combination strateg

Antineoplastic Drugs: Research Progress of Immune Checkpoint CD47 Inhibitors.

Immunotherapy using immune checkpoint inhibitors represents a breakthrough in cancer treatment. The overexpressed CD47 on tumor surfaces functions as a 'don't eat me' signal by binding to signal regulatory protein alpha (SIRPα) on macrophages, facilitating immune escape. However, the advancement of targeted CD47 inhibitors is currently challenged by uncertainties in efficacy, hematological safety, and dosing optimization. This paper provides a comprehensive overview of the structural domains and physiological functions of CD47. We evaluate the current research status of various inhibition stra

Hematological immune-related adverse events associated with checkpoint inhibitors.

BACKGROUND: The use of immune checkpoint inhibitors (ICIs) has expanded substantially across tumor types, establishing them as a cornerstone of modern oncology. However, the frequency and severity of immune-related adverse events (irAEs) remain underestimated, particularly hematologic toxicities, which can be life-threatening and require distinct management approaches compared to cytotoxic or targeted therapies. METHODS: We performed literature reviews using PubMed to identify and analyze reported cases of the most frequent hematologic irAEs; autoimmune hemolytic anemia (AIHA), immune thrombo

[Perioperative Treatment for Gastric Cancer Using Immune Checkpoint Inhibitors].

Perioperative and adjuvant treatment for resectable gastric cancer was historically fragmented across regions. In East Asia, high-quality D2 gastrectomy followed by postoperative chemotherapy became the standard, whereas postoperative chemoradiotherapy was established in North America and perioperative chemotherapy was developed in Europe. Over the past 2 decades, surgical quality in Western countries has improved, particularly through wider adoption of D2 lymphadenectomy, and perioperative chemotherapy has evolved from ECF/ECX to FLOT. The global phase Ⅲ MATTERHORN trial marked a major turnin

Neurologic complications of immune checkpoint inhibitors: A comprehensive review.

Cancer is characterized by uncontrolled cell proliferation, often leading to invasion of adjacent tissues and onset of metastasis. Therapeutic options include surgical intervention, chemotherapy, radiotherapy, and targeted therapies, with immune checkpoint inhibitors (ICIs) emerging as a revolutionary approach. ICIs, like anti-CTLA-4 (e.g., ipilimumab) and anti-PD-1/PD-L1 (e.g., pembrolizumab and nivolumab), enhance antitumor immune responses by blocking inhibitory mechanisms within T-cells. Despite benefits, ICIs can elicit immune-related adverse effects (irAEs), manifested by dermatologic, g

Myocardial Injury Following Immune Checkpoint Inhibitors: An Australian Experience.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have revolutionised cancer treatment, becoming a cornerstone therapy for various malignancies. Despite their efficacy, there are concerns about their potential cardiotoxicity, particularly, subclinical myocardial injury. However, there remains a scarcity of local data on the precise incidence of such cardiac complications. This prospective pilot study aims to assess myocardial injury in patients undergoing ICI therapy without known cardiovascular disease, using biochemical and cardiac imaging markers of myocardial injury. METHOD: A total of 41 p

Myasthenia Gravis in Patients Treated With Immune Checkpoint Inhibitors.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have improved outcomes significantly for patients across multiple tumor types, and now are being used in combination with other therapies and in earlier settings where treatment intent is curative. Immune-related adverse events occur commonly and there are clear guidelines regarding management. Neurological toxicities such as myasthenia gravis (MG) with or without myositis are rare but are associated with high morbidity and mortality. METHODS: This single-centre study presents a series of patients treated with ICIs who subsequently developed imm

Pituitary and adrenal disorders induced by immune checkpoint inhibitors.

Over the past decade, the development of ICI (immune checkpoint inhibitors) has constituted a revolution in the treatment of many cancers, but with a specific toxicity profile including endocrine IRAEs (immune-related adverse events). As the indications for these molecules are constantly increasing due to their efficacy, it is important that endocrinologists and oncologists know how to detect, manage and monitor this type of toxicity. Many guidelines and recommendations have been proposed in the last few years for the management of endocrinopathies. French guidelines on immunotherapy-related e

Immune Checkpoint Inhibitors and Risk of Type 1 Diabetes.

OBJECTIVE: Type 1 diabetes mellitus (T1DM) is a rare, irreversible immune-related adverse event reported in patients receiving treatment with immune checkpoint inhibitors (ICI). However, clinical risk factors for ICI-induced T1DM (ICI-T1DM) and its impact on survival in patients remain unknown. RESEARCH DESIGN AND METHODS: We used Optum's Clinformatics Data Mart database for assessment of the incidence and characteristics of T1DM in a large de-identified cohort of patients treated with ICI between 2017 and 2020. We applied Fine-Gray and cause-specific hazard models to study associations betwe

Thyroid-related adverse events induced by immune checkpoint inhibitors.

Immune checkpoint inhibitors, namely anti-CTLA-4, anti-PD-1 and anti-PD-L1 monoclonal antibodies, have emerged in the last decade as a novel form of cancer treatment, promoting increased survival in patients. As they tamper with the immune response in order to destroy malignant cells, a new type of adverse reactions has emerged, known as immune-related adverse events (irAEs), which frequently target the endocrine system, especially the thyroid and hypophysis. Thyroid irAEs include hyperthyroidism, thyrotoxicosis, hypothyroidism and a possibly life-threatening condition known as the "thyroid st
