Pneumonia in the immunocompromised host: the role of bronchoscopy and newer diagnostic techniques.

The microbiology laboratory plays an essential role in the laboratory diagnosis of pneumonia in the immunocompromised host. Both the diversity of underlying or predisposing host conditions that increase risk for pneumonia and the variety of microbial agents that may be etiologically responsible make the laboratory's role in the diagnostic process a challenging one. In addition, the laboratory has a number of diagnostic tools available for detection of etiologic agents including conventional stains and cultures as well as newer tests relying on antigen detection and molecular techniques. Cryptococcal meningitis in the immunocompromised host: intracranial hypertension and other complications.

Cryptococcosis as a complication of the immunocompromised host has dramatically increased in frequency since the start of the AIDS epidemic. This trend has heightened awareness of the complications of cryptococcal meningitis; of these, intracranial hypertension is common, severe, and life-threatening, as exemplified by three cases in our institutions presented here in detail. An aggressive approach to management of this complication has not been the standard of care, but neurosurgical interventional studies combined with physiologic observations suggest early intervention may reduce the devast

Coinfection of the immunocompromised but not the immunocompetent host by multiple human cytomegalovirus strains.

Coinfection by multiple human cytomegalovirus (HCMV) strains was investigated in immunocompetent individuals and AIDS patients. Thirty HCMV maternal and fetal or newborn isolate pairs from 9 cases of congenital HCMV infection as well as 36 HCMV isolates and 2 PCR-HCMV-positive CSF samples from 13 AIDS patients were tested by restriction fragment length polymorphism analysis of multiple genome regions. Results from the group of congenital infections showed that: i) all the 9 women with primary HCMV infection presumably harboured a single HCMV strain; ii) all strains were genetically unrelated;

Pneumocystis carinii pneumonia. Review of 32 cases in immunocompromised hosts.

The records of patients in whom Pneumocystis carinii pneumonia (PCP) was diagnosed between January 1989 and December 1991 were reviewed. Thirty-two patients--all immunocompromised--were included in the study: 41% were HIV-positive and 59% HIV-negative. In 23 patients (72%) concomitant pathogens were isolated, most frequently Cytomegalovirus. Presenting symptoms included fever (97%), cough (75%) and dyspnea (63%). All HIV-infected patients had a T4-lymphocyte count below 200/mm3 (or 20%). The majority of patients (80%) treated with trimethoprim-sulfamethoxazole experienced adverse events which

Herpes zoster ophthalmicus and the immunocompromised host: a case report and review.

BACKGROUND: Herpes zoster is the secondary form of varicella zoster virus disease, caused by the reactivation of the dormant virus in the sensory ganglia. It manifests as a unilateral cutaneous dermatitis with a prodromal fever and malaise. Herpes zoster ophthalmicus occurs when the ophthalmic division of the trigeminal nerve becomes afflicted. CASE REPORTS: This case represents a middle-aged female with breast carcinoma. Cancer radiotherapy and chemotherapy created an immunosuppressed state, which allowed the development of herpes zoster ophthalmicus. Most patients who manifest zoster are im

[Two cases of invasive pulmonary aspergillosis in non-immunocompromised hosts].

Invasive pulmonary aspergillosis generally occurs in immunocompromised hosts such as patients with leukemia, and other malignancies, who are receiving anti-cancer chemotherapy. In this report, two non-immunocompromised patients who developed invasive pulmonary aspergillosis are presented. Case 1: A 63-year-old man complained of productive cough and fever. He received antibiotic therapy from his personal physician. This symptoms did not respond, however, and dyspnea developed. He was then transferred to our hospital, about one month after the onset. The chest X-ray showed a meniscus shadow sugg

Dysgonic fermenter-3: a bacterium associated with diarrhea in immunocompromised hosts.

We describe two patients with chronic diarrhea associated with dysgonic fermenter-3 (DF-3) infection. One patient had common variable hypogammaglobulinemia and the other hand chronic idiopathic neutropenia and human immunodeficiency virus infection. Specific stool culture techniques were necessary to isolate DF-3. The organism was sensitive to clindamycin, tetracycline, and trimethoprim-sulfamethoxazole. Antibiotic therapy eradicated the organism and the diarrhea resolved in both patients. DF-3 is a little-recognized organism associated with diarrhea in the immunocompromised patient. It should

Opportunistic mycoses in the immunocompromised host: experience at a cancer center and review.

The field of opportunistic mycoses in the patient with cancer is rapidly changing. Not only are fungal infections increasing in frequency in this patient population, but these infections are occurring earlier during the course of cytotoxic chemotherapy, and newer fungi are increasingly recognized as potentially lethal pathogens. Candidiasis remains the most commonly encountered infection. The spectrum of disease includes candidemia and acute and chronic disseminated candidiasis. Pulmonary aspergillosis and disseminated aspergillosis are common and remain relatively resistant to therapy. Dissem

Lymphoproliferative diseases in immunocompromised hosts: the role of Epstein-Barr virus.

Epstein-Barr virus (EBV) is a ubiquitous transforming virus of the herpes group showing tropism for B lymphocytes. Primary infection in normal hosts results in a transient lymphoproliferative disorder, acute infectious mononucleosis (IM), that is restricted by cytotoxic and suppressive lymphocytes. However, in the immunodeficient host, EBV-induced lymphoproliferation may behave in a biologically malignant fashion. Patients with primary immunodeficiencies and those with immune incompetence resulting from suppressive therapy in allograft transplantation or infection with human immunodeficiency v

Combination therapy and monotherapy in the treatment of severe infection in the immunocompromised host.

Profoundly granulocytopenic patients in whom fever develops are likely to have gram-negative septicemia, which may rapidly be fatal. These patients require early empiric therapy with a synergistic bactericidal combination of antibiotics that should be chosen on the basis of in vitro studies, animal models, and results of volunteer and clinical trials. From in vitro studies, it is apparent that the degree of synergy between an aminoglycoside and a beta-lactam is determined mainly by the aminoglycoside. Amikacin is the most synergistic of the aminoglycosides. A combination of an aminoglycoside s

An unusual central nervous system infection in a young immunocompromised host.

A 13-year-old girl with a ten-year history of lymphoblastic leukemia and several central nervous system (CNS) relapses developed a bone marrow relapse and accelerated CNS leukemia. Following treatment with CNS radiation and intravenous chemotherapy, she developed fever, pancytopenia, headache, and vomiting. Her neurological function deteriorated and she died on the 20th hospital day. Multiple CSF examinations failed to disclose either leukemic cells or organisms. Blood cultures obtained from a Broviac catheter yielded Micrococcus species. Postmortem examination showed meningoependymitis with i

Intra-Host Evolution During Relapsing Parvovirus B19 Infection in Immunocompromised Patients.

BACKGROUND: Parvovirus B19 (B19V) can cause severe relapsing episodes of pure red cell aplasia in immunocompromised individuals, which are commonly treated with intravenous immunoglobulins (IVIGs). Few data are available on B19V intra-host evolution and the role of humoral immune selection. Here, we report the dynamics of genomic mutations and subsequent protein changes during relapsing infection. METHODS: Longitudinal plasma samples from immunocompromised patients with relapsing B19V infection in the period 2011-2019 were analyzed using whole-genome sequencing to evaluate intra-host evolutio

Role of the gut microbiome in shaping drug response in immunocompromised hosts.

BACKGROUND: There is an increasing amount of evidence on microbiome-drug interactions in several clinical settings, including in immunocompromised patients. The gut microbiome has been shown to directly and indirectly influence drug efficacy and toxicity, offering high potential for clinical translation. OBJECTIVES: This narrative review aims to provide an up-to-date overview of the relationship between gut microbes and drugs, with a focus on immunochemotherapy in immunocompromised hosts, including oncological and transplant patients. SOURCES: We searched PubMed to identify relevant literatu

Campylobacter jejuni Bacteremia in an Immunocompromised Host: A Rare Case Report.

Campylobacter jejuni is a common cause of bacterial gastroenteritis and typically produces a self-limited illness in immunocompetent hosts. Bacteremia is rare and is usually described in patients with immunosuppression or significant comorbidities, with risk for severe systemic complications if diagnosis is delayed. We report a case of Campylobacter jejuni bacteremia in a 41-year-old man with type 2 diabetes mellitus, coronary artery disease, hypertension, hyperlipidemia, and obstructive sleep apnea who presented with fever, chills, and profuse watery diarrhea after consuming convenience-store

Hemichorea-Hemiballismus As Presentation of Toxoplasma gondii Encephalitis in an Immunocompromised Host.

Toxoplasma gondii encephalitis is a common but serious opportunistic infection in people living with human immunodeficiency virus (PLHIV), particularly those with advanced disease and low CD4 counts. Its classical manifestations include fever, altered mental status, seizures and focal neurological signs. Movement disorders are a less common consequence, with hemichorea-hemiballismus (HCHB) representing a rare but striking manifestation. We describe a 36-year-old man with advanced HIV disease who had defaulted on treatment. He presented with bizarre, involuntary movements of his left upper and
