Cyclosporine in SJS/TEN management: a brief review.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are considered to be among the most severe dermatologic emergencies with high risk for morbidity and mortality if managed poorly. These disease processes usually are the result of a reaction to antipsychotic or antibiotic medications, though the complete list of potential causative drugs is extensive. Despite the life-threatening nature of these conditions, studies evaluating systemic immunomodulating agents that would be effective in halting the poor overall outcome are limited. Over the last several years, reports advocating

Response to rituximab and prednisolone for opsoclonus-myoclonus-ataxia syndrome in a child with ganglioneuroblastoma.

Opsoclonus-myoclonus-ataxia (OMA) syndrome is a rare neurobehavioral paraneoplastic disorder in children with neuroblastic tumors. The neurologic symptoms are generally treated with a number of immunosupressive and immunomodulating agents. A 4-year-old previously healthy male patient was admitted to the authors' center with progressive ataxia, gait disturbance, difficulty of speech, and opsoclonus. He had a diagnosis of ganglionueroblastoma at the thoracal paraspinal region. Following surgery, the patient received IVIG and prednisolone but his cerebellar symptoms progressed. Rituximab therapy

The role of lipids in vitiligo and schizophrenia.

The pathogenesis of vitiligo and schizophrenia has not been adequately clarified. We explore the role of lipids in these diseases. Both conditions have been associated with stress in several observations and studies. Research data indicate complex interactions between oxidative stress and metabolic syndrome-with lipid abnormalities being a significant component of the latter-in these diseases. The impaired membrane lipid homeostasis mechanism is related to the increased phospholipid remodeling caused by excessive oxidative stress in schizophrenia. We indicate that sphingomyelin is possibly inv

Unusual case report of thrombotic microangiopathy of the small bowel following liver transplantation, a possible immunosuppressant-induced disease with histological and ultrastructural findings.

Cyclosporin-A (CsA) and tacrolimus (FK-506) are immunomodulating agents used to prevent rejection in organ transplantation. They are both associated with several side effects, including nephrotoxicity and severe hypertension due to vascular injury, which often appears as a microvascular occlusive disorder (thrombotic microangiopathy, TMA). We report the first case of a microvascular occlusive disorder with the features of TMA in the small bowel of an orthotopic liver transplant (OLT) patient after immunosuppressive therapy with CsA and FK506. The patient presented with severe recurrent abdomin

High-dose intravenous immunoglobulins: an approach to treat severe immune-mediated and autoimmune diseases of the skin.

Adjuvant high-dose intravenous immunoglobulins (IVIgs) are being used increasingly in a range of immune-mediated and autoimmune diseases. Although numerous immunomodulatory mechanisms have been suggested, the exact mechanisms of action are poorly understood. The efficacy of IVIg in certain diseases has been proven in clinical trials, insofar as IVIg is approved as the therapy of choice for Kawasaki syndrome or idiopathic thrombocytopenic purpura. IVIg treatment has been shown to be safe, without the many drug-related adverse effects, including systemic immunosuppression, that are related to co

Recent strategies for the chemotherapy of visceral leishmaniasis.

Visceral leishmaniasis is a widespread and deadly disease. First-line drugs are antimonials, but amphotericin B and its lipid formulations B is used for treating visceral leishmaniasis that is unresponsive to antimony. New therapeutic approaches are being actively developed, including the following: use of drug carriers targeted specifically to the parasite location, thus reduce adverse effects of drug; use of immunomodulating drugs; evaluation of natural products; pharmacokinetic studies; and drug combinations. Recent clinical trials with paromomycin and miltefosine were successful and these

Update on therapy for refractory dermatomyositis and polymyositis.

Evaluation of new therapies for the inflammatory myopathies is complicated by the heterogeneity of these syndromes as well as by the lack of internationally accepted definitions of disease categories and assessments of disease activity and chronicity. This review covers our opinion of therapies and emphasizes the need for an early rehabilitation evaluation for these patients. Oral corticosteroids are the first line of therapy for the inflammatory myopathies, but because of their side effects and the existence of a subset of patients in whom disease is controlled only with high-dose corticoster

Effects of immunosuppressants on immune response to vaccine in inflammatory bowel disease.

OBJECTIVE: To evaluate the response rate to vaccination in different treatment groups (nonimmunosuppressants and immunosuppressants). DATA SOURCES: We completed an online systematic search using PubMed to identify all articles published in English between January 1990 and December 2013 assessing the effect of the response rate to vaccination in different treatment groups (with and without immunomodulators). The following terms were used: "inflammatory bowel disease (IBD)" OR "Crohn's disease" OR "ulcerative colitis" AND ("vaccination" OR "vaccine") AND ("corticosteroids" OR "mercaptopurine" O

Comparative risk of psoriatic arthritis in psoriasis patients on immunomodulators.

Background: Psoriatic arthritis (PsA) is a rapidly progressive arthritis that is difficult to prevent and treat. The comparative risk of PsA development in psoriasis patients prescribed PsA-approved immunomodulators is unknown. Methods: We assessed the 3-year risk of PsA in psoriasis patients on immunomodulators of IL-12, 17, 23, tumor necrosis factor alpha, JAK1, and JAK3. Using the TriNetX Research Network, we identified patients with psoriasis and the use of an immunomodulatory agent. The immunomodulatory agents assessed included adalimumab, infliximab, ixekizumab, secukinumab, tildrakizuma

Medicinal Plants and Lead Phytomolecules as Immunomodulators: An Updated Review.

All therapeutic interventions aimed at modulating the immune response to pathogens, self-antigens, carcinogens, or xenogeneic antigens are referred to as immunomodulation, which either prevents hyperactivation or restores the appropriate response of the immune system. Since antiquity, medicinal plants have been used as a source of immune-boosting medicines with fewer side effects. These medicinal plants, for example, Curcuma longa, Camellia sinensis, Artemisia annua, and Andrographis paniculata, have been used to treat immune system disorders such as multiple sclerosis, psoriasis, lupus, and o

Immunomodulatory actions of tonifying polysaccharides: pharmacological effects, mechanisms and therapeutic applications.

Tonifying polysaccharides, as a class of natural medicines, have attracted considerable interest due to their low toxicity, high safety profile, and excellent stability. Investigations have highlighted the significant impact of polysaccharides derived from Chinese medicinal herbs on the modulation of immune responses, significantly enhancing the functionality of immune organs such as the spleen, thymus, bone marrow, and intestines, stimulating immune cell proliferation and activation of T and B lymphocytes, macrophages, natural killer cells, and dendritic cells, and regulating the secretion an

Potential of using lactic acid bacteria for therapy and immunomodulation in man.

There is in 1993 no proven medical indication of lactic acid bacteria (LAB) for therapy or immunomodulation in man. However, within the bulk of publications, rigorous trials have now opened rational fields of research on beneficial effects of LAB. These include lactose digestion, cholesterol metabolism, diarrheal disorders, prophylaxis of intestinal or urogenital infections, immunomodulation or even oral vaccination. We try here to analyse these studies, considering LAB as pharmacological agents, and conclude that pursuit of research could be promising for ecological therapy of mucosal disease

Immunomodulatory drugs: a promising clinical ally for cancer immunotherapy.

While immunomodulatory imide drugs (IMiDs) have been authorised for treatment of haematological cancers for over two decades, the appreciation of their ability to stimulate antitumour T cell and natural killer (NK) cell responses is relatively recent. Clinical trial data increasingly show that targeted immunotherapies, such as antibodies, T cells, and vaccines, improve outcomes when delivered in combination with the IMiD derivatives lenalidomide or pomalidomide. Here, we review these clinical data to highlight the relevance of IMiDs in combinatorial immunotherapy for both haematological and so

Immunosuppressive therapy does not increase operative morbidity in patients with Crohn's disease.

GOALS: The aim of this study was to examine the impact of immunosuppressive therapy on the morbidity of intestinal surgery in patients with Crohn's disease. BACKGROUND: An increasing number of immunomodulating agents are being used in the treatment of Crohn's disease. The effect of these medications on surgical morbidity is controversial. STUDY: We performed a retrospective review of our prospectively maintained database of patients with Crohn's disease who underwent intestinal surgery between June 1999 and May 2010. The effect of perioperative immunomodulation on postoperative outcomes, spe

Preparing for the next pandemic: Simulation-based deep reinforcement learning to discover and test multimodal control of systemic inflammation using repurposed immunomodulatory agents.

BACKGROUND: Preparation to address the critical gap in a future pandemic between non-pharmacological measures and the deployment of new drugs/vaccines requires addressing two factors: 1) finding virus/pathogen-agnostic pathophysiological targets to mitigate disease severity and 2) finding a more rational approach to repurposing existing drugs. It is increasingly recognized that acute viral disease severity is heavily driven by the immune response to the infection ("cytokine storm" or "cytokine release syndrome"). There exist numerous clinically available biologics that suppress various pro-inf

[Hemophagocytic lymphohistiocytosis: diagnostic problems in pediatrics].

Hemophagocytic lymphohistiocytosis (HLH), also known as hemophagocytic syndrome (HS), is a life-threatening hyperinflammatory condition caused by uncontrolled proliferation of activated lymphocytes and histiocytes producing excessively proinflammatory cytokines. HLH can occur in all age groups. The most common syndromes are: prolonged fever, hepatosplenomegaly, unspecific neurological symptoms, pancytopenia, hemophagocytosis. Characteristic biochemical markers include high triglycerides, ferritin and decreased fibrinogen. There are two forms of HLH, primary (genetic) and acquired (secondary to

Severe vernal keratoconjunctivitis successfully treated with subcutaneous omalizumab.

A 12-year-old boy with severe mixed limbal and palpebral vernal keratoconjunctivitis experienced persistent ocular symptoms despite treatment with topical corticosteroids or cyclosporine. Signs and symptoms resolved completely with monthly subcutaneous omalizumab, an immunomodulating biologic agent. To our knowledge, this is the first report of its use as a monotherapy agent to treat vernal keratoconjunctivitis.

Treatment of idiopathic inflammatory myopathies.

PURPOSE OF REVIEW: This article reviews the results of recent therapeutic trials in dermatomyositis, polymyositis, and inclusion body myositis and suggests an approach to treating patients with inflammatory myopathy. RECENT FINDINGS: We reviewed 10 double-blind, placebo-controlled therapeutic trials in patients with inflammatory myopathy. Only one, using intravenous immunoglobulin in refractory dermatomyositis, indicated benefit. A brief trial of azathioprine in polymyositis and eight studies using various treatments in inclusion body myositis did not show benefit. SUMMARY: There have been n
