Oncogenic hypophosphataemic osteomalacia is a rare condition. The causative tumour is often difficult to locate. Primary tumours have been reported in the head and neck, skeleton, and soft tissue. Octreotide scanning was used in this case and detected a mesenchymal tumour in the pubic symphysis.

BACKGROUND: Somatostatin receptors have been found on a variety of neuroendocrine tumours, such as carcinoids and paragangliomas, as well as on most pancreatic endocrine and breast tumours. Somatostatin receptor scintigraphy with a radionuclide-labelled somatostatin analogue, [111Indium- diethylenetriaminopenta-acetic acid]octreotide, is a sensitive and specific technique for visualizing in vivo the presence of somatostatin receptors on various tumours.

Arterio-arterial embolism of carotid origin is one major cause of cerebral ischemia. To evaluate the role of blood platelets in this process, In-111 labeled platelet scintigraphy was performed in a series of 250 patients. Of this number, one patient with an 80% carotid stenosis and a normal intracerebral angiogram suffered a transient ischemic attack (TIA) several hours after the injection of radiolabeled platelets. We report a case of massive cerebrospinal fluid (CSF) leakage where the tracer injected intra-thecally for radionuclide cisternography was later visualized in the bowel as well as the nasopharynx. We discuss the potential implications of this finding in patients with CSF leaks. A brief review of the diagnosis of CSF leaks is included.

Indium-III leukocytes are useful in localizing infection and abscess with excellent sensitivity and specificity. In this case report, heterologous Indium-III leukocytes provided the same diagnostic information as autologous leukocytes without adverse effects to the patient. A potential application of donor Indium-III leukocytes in the nonleukopenic patient is discussed.

Indium-111-labeled leukocyte scintigraphy was performed on a 44-yr-old man to exclude an occult abscess. Four- and twenty-four-hour images of the abdomen revealed splenic photopenia except for a rim of activity medially. A subsequent computed tomography (CT) study demonstrated necrosis or hemorrhage of the spleen except for a medial rim. Exploratory laparotomy demonstrated necrotizing vasculitis with granuloma formation consistent with Wegener's granulomatosis and a rim of viable splenic tissue corresponding to the radionuclide and CT studies.

Repeated scintigraphy was performed for 6 days after the injection of autologous 111In-labeled platelets in a patient showing signs of recurrent occult gastrointestinal bleeding. A non-invasive method of imaging and assessing inflammatory bowel disease is described. 111Indium labelled leucocyte scans were performed on 33 patients with a wide variety of inflammatory bowel diseases and 25 control patients. All patients with moderate or severe inflammatory bowel disease had positive scans with localisation of abnormal activity corresponding to the sites assessed to be diseased by radiology in either small or large bowel. Localization of the placenta by radionuclide imaging techniques is useful in assessing the cause of vaginal bleeding late in pregnancy. Placenta previa is the most threatening cause of late gestational bleeding, and the acceptably high accuracy of nuclear placentography in detecting this condition is well-established. Nevertheless, B-mode ultrasonography is a preferable method because it delivers no ionizing radiation and is capable of identifying the cervix in virtually all patients. The latter method will eventually replace nuclear placentography.

To evaluate the clinical usefulness of immunoscintigraphy with OC 125 in ovarian carcinoma, we studied 25 patients suspected of having ovarian carcinoma or in follow-up after surgery. Fourteen patients were studied with 131I-OC 125 F(ab')2 and 11 with 111In-OC 125 F(ab')2. No differences were observed with the use of either radionuclide.
