Preventable yet persistent: silicosis and gaps in public health policy - a narrative review.

Silicosis remains a critical, 100% preventable occupational lung disease that continues to persist as a global public health crisis in the 21st century. Despite decades of industrial safety knowledge, the disease is currently seeing a modern resurgence, particularly in the engineered stone industry, where high-intensity silica exposure leads to rapid, fatal disease variants. This review synthesizes current global and national evidence to highlight the staggering burden of silicosis, with a specific focus on India, where over 50 million workers are projected to be at risk by 2026. Epidemiologic

Informal PrEP use in Greece: The long, hard road to formal programmatic implementation.

OBJECTIVES: Pre-exposure prophylaxis (PrEP) with antiretroviral drugs (ARVs) is a highly effective HIV prevention strategy. Although the European Medicines Agency approved oral PrEP in 2016 and the World Health Organization (WHO) has since recommended simplified and person-centred delivery models, implementation remains inconsistent across Europe. In Greece, national PrEP guidelines were issued in 2022, but public sector access has not yet been established. METHODS: We report on informal PrEP use in 2024 among men who have sex with men (MSM) and transgender women (TGW) attending a private sex

Catastrophic and impoverishing effects of health expenditure: new evidence from the Western Balkans.

This paper investigates the effect of health-related expenditure on household welfare in Albania, Bosnia and Herzegovina, Montenegro, Serbia and Kosovo, all of which have undertaken major health sector reform. Two methodologies are used: (i) the incidence and intensity of 'catastrophic' health care expenditure, and (ii) the effect of out-of-pocket payments on poverty headcount and poverty gap measures. Data are drawn from the most recent Living Standards and Measurement Surveys, 2000-05. While our analyses are not without their limitations, and the lack of comparability across instruments prec

Community-based reports of morbidity, mortality, and health-seeking behaviours in four Monrovia communities during the West African Ebola epidemic.

The goal of this study was to assess morbidity, mortality, and health-seeking behaviours during the 2014 Ebola outbreak in Monrovia, Liberia. This study examined commonly reported symptoms of illness, pre-clinical diagnostic practices, typical healthcare-seeking strategies, and health resources available to populations, in order to identify salient needs and gaps in healthcare that would inform local emergency response efforts. Semi-structured interviews were conducted with household members in four Monrovia neighbourhoods. Researchers used a multi-stage cluster approach to recruit participant

Abortion law, policy and services in India: a critical review.

Despite 30 years of liberal legislation, the majority of women in India still lack access to safe abortion care. This paper critically reviews the history of abortion law and policy in India since the 1960s and research on abortion service delivery. Amendments in 2002 and 2003 to the 1971 Medical Termination of Pregnancy Act, including devolution of regulation of abortion services to the district level, punitive measures to deter provision of unsafe abortions, rationalisation of physical requirements for facilities to provide early abortion, and approval of medical abortion, have all aimed to

Stakeholder attitudes and perspectives on wildlife disease surveillance as a component of a One Health approach in Thailand.

Coordinated wildlife disease surveillance (WDS) can help professionals across disciplines effectively safeguard human, animal, and environmental health. The aims of this study were to understand how WDS in Thailand is utilized, valued, and can be improved within a One Health framework. An online questionnaire was distributed to 183 professionals (55.7% response rate) across Thailand working in wildlife, marine animal, livestock, domestic animal, zoo animal, environmental, and public health sectors. Twelve semi-structured interviews with key professionals were then performed. Three-quarters of

Pollution Emissions, Environmental Policy, and Marginal Abatement Costs.

Pollution emissions impose serious social negative externalities, especially in terms of public health. To reduce pollution emissions cost-effectively, the marginal abatement costs (MACs) of pollution emissions must be determined. Since the industrial sectors are the essential pillars of China's economic growth, as well as leading energy consumers and sulfur dioxide (SO₂) emitters, estimating MACs of SO₂ emissions at the industrial level can provide valuable information for all abatement efforts. This paper tries to address the critical and essential issue in pollution abatement: How do we det

The Moral Foundations of Health Sector Solidarity in Taiwan.

People's supportive attitudes towards the publicly funded health system are crucial for ensuring the system's sustainability through crises and maintaining its core functions of funding and providing health services for all. In the literature, this supportive attitude is considered a form of health sector solidarity. Existing explanations for health sector solidarity focus on individuals' feedback to the health system-such as satisfaction and performance evaluation-but pay scant attention to the moral judgements underlying this solidaristic attitude. This study aims to investigate the moral fo

Academic-Nonprofit Partnership for Public Health: Document Analysis and Systematization of the Implementation of a Multijurisdictional Long COVID Surveillance Platform.

CONTEXT: Health information exchanges (HIEs) are generally underutilized as data sources for public health surveillance, potentially decreasing the ability of public health practitioners to leverage the rich, real-time, clinical, and public health data therewith contained. OBJECTIVES: To systematize the process of implementing an academic-nonprofit partnership (ANPP) designed to leverage multijurisdictional HIE data for public health surveillance of Long COVID by Systematizing the ANPP's. A new approach to gathering pharmaceutical market data to support policy implementation and access to medicines: as demonstrated by malaria medicines in Zambia.

BACKGROUND: The steady supply of quality, affordable medicines is a pillar of a functioning health system. In addition to the public sector, the private, mission and not-for-profit sectors often serve a large part of the population in Africa. However, while there is generally systematic recording of public sector supply of medicines, detailed, systematic and reliable national market data including these non-public sectors are not commonly available in most countries in Africa. Understanding the total market is a missing part of the access puzzle: without this information, policy makers and hea

Harnessing private sector strategies for family planning to deliver the Dual Prevention Pill, the first multipurpose prevention technology with pre-exposure prophylaxis, in an expanding HIV prevention landscape.

INTRODUCTION: The Dual Prevention Pill (DPP) combines oral pre-exposure prophylaxis (PrEP) with oral contraception (OC) to prevent HIV and pregnancy. Noting the significant role played by the private sector in delivering family planning (FP) services in countries with high HIV burden, high level of private sector OC uptake, and the recent growth in self-care and technology-based private sector channels, we undertook qualitative research in Kenya, South Africa and Zimbabwe to prioritize private sector service delivery approaches for the introduction of the DPP. METHODS: Between March 2022 and

Patterns of antibiotic use for acute febrile illness in resource-limited settings: a multicenter study in DR Congo, Kenya and Uganda.

INTRODUCTION: In East Africa and beyond, uncertainty regarding the underlying aetiology of febrile illnesses may result in inappropriate treatment. In our study, we aimed at enhancing our understanding of antibiotic use patterns, prescribing practices and factors influencing antibiotic use in this region by exploring the pathways of care for acute febrile illnesses in three countries in East Africa. METHODS: Between October 2021 and February 2024, acute febrile patients were enrolled in six health facilities in the eastern Democratic Republic of the Congo (DRC), Kenya, and Uganda. Our cross-s

Outreach-based clinical pharmacist prescribing input into the healthcare of people experiencing homelessness: a qualitative investigation.

BACKGROUND: Severely and multiply disadvantaged members of the homeless population are disproportionately vulnerable to exceptionally high levels of multi-morbidity and premature death. Given widespread calls for the development of interventions that might improve the uptake and effectiveness of healthcare for this population, this study investigated patient and other stakeholder perspectives regarding an outreach service, delivered by prescribing pharmacists in collaboration with a local voluntary sector provider, within homelessness services and on the street in Glasgow (UK). METHODS: The q

Using rubber stamps and mobile phones to help understand and change antibiotic prescribing behaviour in private sector primary healthcare clinics in Kenya.

BACKGROUND: Antibiotic use in primary care can drive antimicrobial resistance (AMR) in the community. However, our understanding of antibiotic prescribing in low- and middle-income countries (LMICs) stems mostly from hospital-based studies or prescription/sales records, with little information available on routine primary care practices. We used an innovative, paper-to-digital documentation approach to deliver routine data and understand antibiotic use for common infections in low-resource primary healthcare clinics (PHCs). METHODS: Rubber stamps were introduced in nine private sector PHCs se

Livelihood implications of dietary shifts to animal-source food alternatives: a scoping review.

Although dietary shifts from animal-source foods (ASFs) to alternatives (Alt-ASFs) could considerably affect livelihoods in food supply chains, the effects will likely vary by the type of Alt-ASFs. In this Review we identified 45 studies, including descriptive analysis of the present state of the sector and modelling studies of future trends. Key findings indicate that cell-based foods would likely reduce overall employment opportunities, while favouring high-skilled workers in high-income countries, although job quality might improve. Shifts towards plant-based foods might have small net empl
