PURPOSE: Spinal injections are increasingly used for back pain treatment. Vertebral osteomyelitis (VO) after spinal injection (SIVO) is rare, but patient characteristics and outcome have not been well characterized. The aim of this study was to assess patient characteristics of SIVO in comparison to patients with native vertebral osteomyelitis (NVO) and to determine predictors for 1-year survival.
METHODS: This is a single-center cohort study from a tertiary referral hospital. This is a retrospective analysis of Patients with VO who were prospectively enrolled into a spine registry from 2008

BACKGROUND: Therapeutic epidural spinal injections (ESIs) of steroids are one of the most common nonsurgical management modalities employed for alleviating pain due to chronic persistent lumbar spinal disease. However, it is well documented that they have significant risks and complications without any long-term efficacy. ESI may result in epidural empyema which may be difficult to diagnose with delays resulting in significant permanent neurological sequelae.
CASE DESCRIPTION: A 45-year-old female presented with a lumbar spinal epidural empyema after receiving ESI for low back and right leg p

BACKGROUND: Interlaminar cervical epidural injections are commonly performed in the practice of interventional pain medicine. Injury to the spinal cord following injection into the substance of the cord is a known complication of this procedure, but it has rarely been reported and illustrated in the literature.
OBJECTIVE: To describe and analyze a case where an inadvertent injection of contrast medium into the cervical spinal cord occurred. Highlighted is the dissonance between how the procedure was performed and published guidelines for its performance. Presented is the first published obliq

OBJECTIVE: To compare the long-term effect and safety of an epidural steroid injection in spinal stenosis patients, with or without local anesthetics.
METHODS: Twenty-nine patients diagnosed with spinal stenosis were included and randomly divided into two groups. Translaminar epidural and selective nerve root spinal injection procedures were performed using steroids mixed with local anesthetics or normal saline. The effects of spinal injection procedures were measured with visual analogue scale (VAS) and functional rate index (FRI). These measurements were performed before injection, at 1 mon

Extradural haematoma is a rare but known complication of neuraxial procedures. Although the literature quotes an incidence of 1:150,000 and 1:220,000 after epidural and spinal procedures, respectively, there is only a limited number of case reports described in pain practice. This case report describes an unusual occurrence of thoracic extradural haematoma following a lumbar epidural steroid injection. An entirely unique feature was its occurrence at a level distant to the site of injection, which has not been reported previously unless associated with catheterisation. We would like to highlig

Three millilitres of plain 0.5% bupivacaine were injected intrathecally at two different spinal interspaces (L2/3 and L4/5) and at two different speeds (15 and 30 sec) in four groups of ten patients. Injection at L2/3 over 15 sec produced a significantly higher mean maximum spread of analgesia (T6.4) when compared to injection at L4-5 over 15 sec (T10.3) (P < 0.05). Over the same interspace L2/3, injection over 15 sec also produced a higher level of spread as compared to the 30 sec group (p < 0.05). At 15 min there was a greater fall in blood pressure in the L2/3 15 sec group when compared to

The pharmacological effect of local anaesthetic solutions in mice including their distribution within the spinal cord, were assessed following intrathecal injection at the L5-L6 interspace. A solution of 5 microliters radioactively labelled lidocaine at a concentration of 50 mg/ml or bupivacaine at 7.5 mg/ml, was used. Both drugs blocked the motor function of the hindlegs in less than 1 min. This persisted for 15.1 +/- 0.8 min. (mean +/- S.E.M.) after lidocaine and 15.2 +/- 2.7 min. after bupivacaine. The highest spinal cord concentration was adjacent to the site of injection, even if traces w

BACKGROUND: Chronic axial spinal pain remains a leading cause of disability. Therapeutic interventional modalities for managing axial spinal pain of facet joint origin include intraarticular injections, facet joint nerve blocks, and radiofrequency neurotomy.Based on multiple randomized controlled trials (RCTs), systematic reviews, and clinical guidelines, the evidence supporting intraarticular facet joint injections is rated as Level III, with a weak to moderate recommendation for managing spinal facet joint pain.
OBJECTIVE: To evaluate intraarticular facet joint injections as a therapeutic o

Chornic spinal cord injury (SCI) will induce bladder urothelium dysfunction. This study investigated the therapeutic effects on urothelial dysfunction after repeated detrusor injections of onabotulinumtoxinA (BoNT-A) in SCI patients with neurogenic detrusor overactivity (NDO). Twenty chronic suprasacral SCI patients with NDO were enrolled. The patients received 300 U BoNT-A injection into the detrusor every six months. The urothelium was assessed by cystoscopic biopsy at baseline and six months after each BoNT-A treatment. Immunofluorescence staining for urothelial dysfunction, including E-cad

BACKGROUND: Lumbar canal stenosis is a common degenerative disorder that causes pain and functional alterations. Epidural steroid injections (ESIs) are often used to treat the symptoms of this condition; however, treatment protocols can vary significantly, and no standardized practice for the management of lumbar canal stenosis has been established.
OBJECTIVES: To identify the optimal method of administration for maximizing the effectiveness and duration of pain relief and functionality improvements provided by ESIs.
STUDY DESIGN: Systematic review and meta-analysis.

PURPOSE: Intrathecal injection of nusinersen is an approved treatment of spinal muscular atrophy (SMA). CT-guided injection is a method of nusinersen administration in patients with severe scoliosis, in whom standard lumbar puncture is not feasible. The injections are repeated every 4 months for life, and accumulated radiation doses absorbed by the patient can increase the risk of cancer. In this study, we present the results of CT-guided intrathecal nusinersen injections with an ultra-low radiation dose protocol.
METHODS: Eighteen patients (15 adults and three children) in whom standard lumb

Intramuscular injections of botulinum toxin block pre-synaptic cholinergic release at neuromuscular junctions producing a temporary paralysis of affected motor units. There is increasing evidence, however, that the effects are not restricted to the periphery and can alter the central excitability of the motoneurones at the spinal level. This includes increases in input resistance, decreases in rheobase currents for action potentials and prolongations of the post-spike after-hyperpolarization. The aim of our experiments was to investigate possible anatomical explanations for these changes. Unil

BACKGROUND: Cervical radiculopathy is most often related to foraminal stenosis due to osteoarthritic changes of the uncovertebral joints anteriorly or zygapophyseal joints posteriorly, rather than disc herniation.
OBJECTIVES: To describe anatomical specificities of the degenerated cervical spine.
METHODS: A critical narrative review was conducted. Articles were non-systematically selected and based on authors' expertise, self-knowledge, and reflective practice.
RESULTS: Vertebral bodies of the lower cervical spine are characterized by 2 lateral prismatic bony protuberances, the uncinate pro

Randomized controlled trials are considered the hallmark of evidence-based medicine. This conveys the idea that up-to-date evidence applied consistently in clinical practice, in combination with clinicians' individual expertise and patients own preference/expectations are enjoined to achieve the best possible outcome. Since its inception in 1990s, evidence-based medicine has evolved in conjunction with numerous changes in the healthcare environment. However, the benefits of evidence-based medicine have not materialized for spinal pain including surgical interventions. Consequently, the debate
