Pulmonary delivery of substances in small animal models is often useful for experimental testing of various vaccine and drug candidates. One of the most challenging elements to such protocols is the efficient disposition of test materials in the lungs of mice. Herein we detail a means to deliver dry powders of an inhalant live-attenuated Mycobacterium bovis Bacille Calmette-Guerin (BCG) vaccine against Mycobacterium tuberculosis to the lungs of mice. The direct delivery methodology is quick, safe, and allows for repeated pulmonary insufflation of substances if boosting is desired. This model s

BACKGROUND: Laparoscopic techniques are being increasingly used for retroperitoneal surgery. However, hemodynamic and ventilatory efforts of retroperitoneal carbon dioxide (CO2) insufflation have not been studied. We hypothesized that differences in absorptive surface, anatomy, and compartment compliance could result in different hemodynamic and ventilatory effects between retroperitoneal and intraperitoneal insufflation.
METHODS: Pigs (n = 7) were anesthetized and stabilized. The peritoneal cavity was incrementally insufflated with CO2 to a maximum pressure of 25 cm H2O and the gas released.

The investigations were carried out on 59 cows from Holstein half-breed, establishing that 8 cows suffered salpinx obstruction (5 cases with unilateral obstruction and 3 cases with bilateral obstruction). The authors are using an apparatus made by themselves, for insufflation of air in the obstructed uterus, and which is useful in desobstruction treatment, too. For diagnosis, CO2 was introduced inside of uterus. The authors used for treatment PBS (saline phosphate buffer) in addition with penicillin G, hydrocortisone and trypsin. Before air insufflation in uterus there will be infused 10-20 ml

Sleep-related upper airway obstruction was treated by continuous insufflation of the pharynx (CIP) in 20 children. All had symptoms but only 1 qualified for a diagnosis of the obstructive sleep apnoea syndrome. Passage of warm humidified air through a thin nasopharyngeal tube at 2-10 litres/min (mean 3.5) relieved obstruction immediately in all patients. Relief was always clinically apparent and accompanied by reduced pleural pressure excursions during breathing. An index of the work of breathing (the product of breathing frequency and pleural pressure change per breath) fell by nearly 60% whi

PURPOSE: Obstructive sleep apnea (OSA) is an independent risk factor for gastric insufflation during the induction of general anesthesia. We aimed to compare the impact of oropharyngeal airway-assisted face-mask ventilation and transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) on gastric insufflation in patients with high-risk OSA.
PATIENTS AND METHODS: Patients at high risk for OSA scheduled for elective major abdominal surgery under general anesthesia were randomized 1:1 to either Group T (THRIVE throughout induction) or Group M (face-mask preoxygenation followed by oro

INTRODUCTION: Epicardial access for ventricular tachycardia (VT) ablation is challenging in patients with prior cardiac surgery or dense pericardial adhesions. Although carbon dioxide (CO₂) insufflation has been used to improve visualization, concerns remain regarding its hemodynamic and electrophysiological effects.
METHODS AND RESULTS: We report the first clinical use of helium to facilitate epicardial access in three patients undergoing VT ablation. All patients had ischemic cardiomyopathy, prior coronary artery bypass grafting, and failed or incomplete endocardial ablation. Helium was del

Minimally invasive surgery, including robotic-assisted laparoscopic procedures, has largely replaced open surgery due to its numerous benefits. However, this approach introduces unique complications such as intraoperative pneumothorax, particularly during upper gastrointestinal (GI) surgeries near the diaphragmatic hiatus. We report the case of a 50-year-old male patient undergoing robotic paraesophageal hernia repair with Toupet fundoplication who developed a tension pneumothorax intraoperatively. Clinical signs included rising peak airway pressures, progressive hypoxemia, and absent right-si

Endoscopy represents a commonly employed technique for canine enteropathies. Different trials in human intestinal endoscopy have suggested that the introduction of water for luminal distension, in place of air, improves the visualization of the mucosal texture and decreases pain. The aim of the study was to compare water immersion (WI) vs. air insufflation (AI) during duodenoscopy in anesthetized dogs in terms of mucosal visualization and nociception. Twenty-five dogs undergoing duodenoscopy were included. The same image of the descending duodenum was recorded applying WI and AI. Each pair of

A 25-year-old male with Duchenne muscular dystrophy and a 73-year-old male with motor neurone disease both presented with chest pain and increasing dyspnoea following routine mechanical insufflation-exsufflation or lung volume recruitment, on a background of long-term non-invasive ventilation. In each case, chest radiograph revealed a pneumothorax. In both cases the pneumothorax fully resolved following insertion of an intercostal catheter. There was no immediate recurrence and the patients were discharged home and ceased ongoing prophylactic respiratory therapy, although one person had recurr

PURPOSE: During a laparotomy, the peritoneum is exposed to the cold, dry ambient air of the operating room (20°C, 0%-5% relative humidity). The aim of this review is to determine whether the use of humidified and/or warmed CO2 in the intraperitoneal environment during open or laparoscopic operations influences postoperative outcomes.
METHODS: A review was performed in accordance with PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. The PubMed, OVID MEDLINE, Cochrane Central Register of Controlled Trials and Embase databases were searched for articles pub

BACKGROUND: Laparoscopic surgery is the main trend method in a variety of surgical fields. Post-operative shoulder pain remains a bothersome issue although many surgical techniques have been applied to minimize it. A simple novel approach to reduce shoulder pain without adverse effects during and after laparoscopic surgery is desired.
METHODS: This prospective randomized controlled study was conducted to enroll a total of 140 patients to evaluate the efficacy of low flow rate (1 L/min) for induction followed by high flow rate (10 L/min) for maintaining 12 mmHg pneumoperitoneum (group A, n = 7

INTRODUCTION: Avoiding tissue desiccation is a common recommendation to reduce postoperative complications following open abdominal surgery, although difficult to achieve delicately without damaging the peritoneal mesothelium. Insufflation of humidified-warm CO2 into the abdomen during open abdominal surgery is proposed as an invisible, effortless way to prevent desiccation. We hypothesized that desiccation during open abdominal surgery would cause loss of peritoneal mesothelium that would be prevented by insufflation of humidified-warm CO2.

AIM: To evaluate whether children with spinal muscular atrophy (SMA) type II were able to learn glossopharyngeal pistoning for lung insufflation (GI), and to evaluate the effects of GI on pulmonary function and chest expansion.
METHODS: Eleven children with SMA type II were recruited. They performed 10 cycles of GI, four times per week, for 8 weeks. Lung function and chest expansion were measured before and after the 8-week period.
RESULTS: Five of the 11 children learned the technique. The median GI volume was 0.28 (range 0.15-0.98) L. Four of the children who completed the study showed a m

PURPOSE: A new method for laparoscopic-assisted sigmoid colectomy and lymphadenectomy is presented. The purpose of this method is to avoid complications and restrictions associated with pneumoperitoneum.
METHODS: Abdominal wall is lifted up with Kirschner wires, which are placed in the middle and left lower abdomen; then a minilaparotomy, which can be created because air-tightness is not necessary, is extended at the beginning of the operation to later remove the resected specimen. Two laparoscopic instruments are inserted through the right lower abdomen. Through the minilaparotomy, the surge

Acid-base status of the blood and tissue fluids and pulmonary gas exchange were continuously observed during intra-abdominal CO2 insufflation for 2 h in 8 paralyzed and artificially ventilated patients who underwent laparoscopic cholecystectomy or resection of the sigmoid colon. Carbon dioxide pressure in the arterial and mixed venous blood as well as in end-tidal air started to increase already at 5 min CO2 insufflation. On the other hand, appreciable elevation in VCO2 and tissue PCO2 (PTCO2) was noted only after 15 min. In vivo buffer value (beta) was well within the normal physiological ran

BACKGROUND: Intra-abdominal pressure during laparoscopic insufflation with pressurized carbon dioxide (CO2) gas is strongly influenced by mechanical ventilation. Resulting pressure fluctuations can destabilize the surgical workspace and potentially cause harm associated with high insufflation pressures. To address this, a novel CO2 insufflator was developed using a radial fan and a gas reservoir to generate and maintain continuously stable insufflation pressures (radial fan-based insufflator, RFBI). This first-in-human study evaluated its safety and feasibility during laparoscopic surgery.

Chronic scaphoid fractures are conventionally treated with arthroscopic bone grafting. Traditional dry wrist arthroscopy avoids graft displacement caused by fluid irrigation but faces challenges such as obscured visibility from bleeding and limited operative space. This study introduces a carbon dioxide insufflation technique using pulsed gas perfusion at 5 kPa pressure and 10 mL/min flow rate. By replacing the dry environment, carbon dioxide insufflation dynamically clears intraoperative bleeding, stabilizes the visual field, and expands the joint cavity for improved instrument maneuverabilit

BACKGROUND: Since the start of the coronavirus disease 2019 pandemic, transnasal humidified rapid-insufflation ventilatory exchange ('THRIVE') has been classified as a high-risk aerosol-generating procedure and is strongly discouraged, despite a lack of conclusive evidence on its safety.
METHODS: This study aimed to investigate the safety of transnasal humidified rapid-insufflation ventilatory exchange usage and its impact on staff members. A prospective study was conducted on all transnasal humidified rapid-insufflation ventilatory exchange cases performed in our unit between March and July
