[Spinal muscular atrophy and respiratory failure. How do primary care pediatricians act in a simulated scenario?].

INTRODUCTION: Spinal muscular atrophy type 1 (SMA-1) tends to be fatal in the first year of life if there is no ventilatory support. The decision whether to start such support is an ethical conflict for healthcare professionals. MATERIAL AND METHODS: A scenario of acute respiratory failure in an infant with SMA-1 has been included in a training program using advanced simulation for Primary Care pediatricians (PCP). The performances of 34 groups of 4 pediatricians, who participated in 17 courses, were systematically analyzed. Clinical, ethical and communication aspects with parents were evalua

Interplay of pulmonary circulation, cardiac filling pressures, and sympathovagal balance in exercise-induced hyperventilation among cardiopulmonary disorders.

Although chemosensitivity plays a role in exercise-induced hyperventilation in cardiopulmonary disorders, the integration of the underlying reflexes with cardiac hemodynamics is not fully understood. We aimed to explore the interplay between right- and left-heart pressure overload, right ventricular-pulmonary arterial (RV-PA) coupling, cardiovascular autonomic modulation, and ventilatory response in patients with cardiopulmonary disorders. Forty patients underwent echocardiography, cardiopulmonary exercise testing, and right heart catheterization. Spectral analysis of heart rate variability (H

Basic concepts of mechanical ventilation: Normal breathing pattern.

Mechanical ventilation is a cornerstone in the management of critically ill pediatric patients, ensuring adequate gas exchange and reducing the work of breathing. Its appropriate use requires a solid understanding of respiratory physiology, lung mechanics, and patient-ventilator interaction. This article reviews the basic concepts of mechanical ventilation, including respiratory system anatomy and physiology, as well as key mechanical properties (compliance, resistance, and time constant) that determine respiratory dynamics. Ventilatory modes are classified according to control variable, breat

Neurally adjusted ventilatory assist in critical care patients with and without obesity: a prospective randomized crossover study.

BACKGROUND: Neurally Adjusted Ventilatory Assist (NAVA) compared to Pressure Support Ventilation (PSV) improves patient-ventilator interactions in intensive care unit. No study has evaluated NAVA in patients with obesity. We aimed to assess the feasibility and safety of NAVA in patients with obesity, and to compare NAVA in patients with versus without obesity. METHODS: In this randomized cross-over study, all respiratory cycles during 1 h of mechanical ventilation from 10 patients with obesity and 11 without obesity were analyzed. Patients underwent 30 min of NAVA and 30 min of PSV in a rando

The role of muscle mechano and metaboreflexes in the control of ventilation: breathless with (over) excitement?

Mechanically and metabolically sensitive thin fibre (group III and IV) muscle afferents are activated during exercise, causing reflex cardiovascular responses that are essential to normal cardiovascular control. Impaired exercise performance in some disease states can be linked to abnormal muscle mechanoreflex and muscle metaboreflex activity. A role for this same afferent feedback in contributing to the hyperpnoea of exercise and the dyspnoea experienced by some patient groups on exercise has recently received increased attention. Evidence is summarised here that supports a role for muscle me

HIF-1 and ventilatory acclimatization to chronic hypoxia.

Ventilatory acclimatization to hypoxia (VAH) is a time-dependent increase in ventilation and ventilatory O2-sensitivity that involves plasticity in carotid body chemoreceptors and CNS respiratory centers. Hypoxia inducible factor-1alpha (HIF-1alpha) controls the expression of several genes that increase physiological O2 supply. Studies using transgenic mice show HIF-1alpha expression in the carotid bodies and CNS with chronic sustained and intermittent hypoxia is important for VAH. Other O2-sensitive transcription factors such as HIF-2alpha may be important for VAH by reducing metabolic O2 dem

Heart-lung interactions during positive-pressure ventilation.

Artificial ventilation using intermittent positive airway pressure is the mainstay support of patients in respiratory failure. By maintaining alveolar ventilation and alveolar stability, positive airway pressure can sustain respiratory gas exchange between the lungs and circulation, thereby supporting pulmonary homeostasis in patients who would otherwise be unable to maintain oxygen transfer and CO2 elimination. However, positive-pressure ventilation (PPV) also results in complex cardiovascular interactions. More often than not, these interactions impede blood flow through ventilated lungs and

Ventilatory Complexity Persists in Phox2b Mutant Mice Lacking the Retrotrapezoid Nucleus/Parafacial Respiratory Group (RTN/pFRG) and in Humans With Congenital Central Hypoventilation Syndrome.

Breathing is inherently variable due to the nonlinear dynamics of its brainstem neural control. In amphibians, a gill and a lung oscillator interact to produce breathing but the lung oscillator is necessary and sufficient to produce a mathematically complex behavior. In mammals, where the preBötzinger complex (preBötC) is considered homologous to the amphibian lung oscillator and the retrotrapezoid nucleus/parafacial respiratory group (RTN/pFRG) homologous to the amphibian gill oscillator, the origin of ventilatory complexity is not known. We address this question by characterizing ventilation

Pathophysiology of human ventilatory control.

We review the substantial recent progress made in understanding the underlying mechanisms controlling breathing and the applicability of these findings to selected human diseases. Emphasis is placed on the sites of central respiratory rhythm and pattern generation as well as newly described functions of the carotid chemoreceptors, the integrative nature of the central chemoreceptors, and the interaction between peripheral and central chemoreception. Recent findings that support critical contributions from cortical central command and muscle afferent feedback to exercise hyperpnoea are also rev

TIGIT and SNRPA1 as novel diagnostic and predictive biomarkers in obstructive ventilatory dysfunction combined with pulmonary nontuberculous mycobacterial infection patients.

BACKGROUND: Patients with obstructive ventilatory dysfunction are prone to NTM (nontuberculous mycobacterial) colonization and infection. Our investigation employs an integrative bioinformatics approach to elucidate critical molecular signatures linked to obstructive ventilatory dysfunction combined with NTM infection, and constructing a clinical diagnostic model using core differentially expressed genes. METHODS: The GSE97298 dataset from the GEO database was analyzed via GEO2R to identify differentially expressed genes (DEGs). Enrichment analysis of the DEGs was conducted using Gene Ontolog

Ventilatory inefficiency in obstructive lung disease reveals emergent [Formula: see text] equilibrium during exercise.

Ventilatory "inefficiency" during exercise in obstructive lung disease-such as that resulting from increased dead space or ventilation-perfusion mismatch-is commonly interpreted as a perturbation that requires an additional increase in minute ventilation and thus in V̇e/V̇co2 to preserve [Formula: see text] homeostasis. This observation raises a fundamental question: how could an increase in V̇e/V̇co2 during exercise be actively directed toward defending [Formula: see text] stability if no known neural signal, governing respiration, directly encodes ventilatory "inefficiency"? We retrospective

Patient-ventilator interaction.

Patient-ventilator interaction has been the focus of increasing attention from both manufacturers and researchers during the last 25 years. There is now compelling evidence that passive (controlled) mechanical ventilation leads to respiratory muscle dysfunction and atrophy, prolonging the need for ventilatory support and predisposing to a number of adverse patient outcomes. Although there is consensus that the respiratory muscles should retain some activity during acute respiratory failure, patient-ventilator asynchrony is now recognized as a cause of ineffective ventilation, impaired gas exch

Automated Detection of Ventilator Asynchronies: A Clinical and Technological Perspective.

Patient-ventilator asynchrony is highly prevalent during invasive mechanical ventilation, yet its detection at the bedside remains limited. Conventional waveform inspection is intermittent, operator-dependent, and insufficient to capture the complexity and temporal variability of patient-ventilator interaction. Automated systems based on advanced signal processing and artificial intelligence represent a paradigm shift, enabling continuous, objective, and scalable detection of asynchrony. Recent approaches highlight the value of entropy-based metrics to quantify the irregularity of ventilatory

Interleukin 8-related neutrophil elastase and the severity of the adult respiratory distress syndrome.

The interaction between activated neutrophils and pulmonary endothelium is thought to contribute to the pathogenesis of the adult respiratory distress syndrome (ARDS), but its relation to ARDS severity, which may support a pathogenetic role, is unclear. Therefore, circulating inflammatory mediators, including the neutrophil chemoattractant and activator interleukin 8 (IL-8), the acute phase cytokine IL-6, and the neutrophil product elastase complexed to alpha 1-antitrypsin (alpha 1-AT), were measured prospectively, together with gas exchange, ventilatory and radiographic variables, in 13 mecha

Right ventricular dysfunction in acute respiratory distress syndrome: from cardiopulmonary mechanisms to precision management: a narrative review.

Acute respiratory distress syndrome (ARDS) is defined by severe hypoxaemia and bilateral pulmonary infiltrates, yet clinical outcomes are not determined by gas exchange alone. Increasing evidence suggests that right ventricular (RV) dysfunction is a frequent and clinically relevant complication, reflecting the combined effects of pulmonary vascular injury and mechanical ventilation. In this narrative review, we examine the mechanisms through which inflammatory lung injury, pulmonary vascular dysfunction, and mechanical ventilation contribute to RV-pulmonary arterial uncoupling. We discuss how

Hyperadditive ventilatory response arising from interaction between the carotid chemoreflex and the muscle mechanoreflex in healthy humans.

Physical exercise potentiates the carotid chemoreflex control of ventilation (VE). Hyperadditive neural interactions may partially mediate the potentiation. However, some neural interactions remain incompletely explored. As the potentiation occurs even during low-intensity exercise, we tested the hypothesis that the carotid chemoreflex and the muscle mechanoreflex could interact in a hyperadditive fashion. Fourteen young healthy subjects inhaled randomly, in separate visits, 12% O2 to stimulate the carotid chemoreflex and 21% O2 as control. A rebreathing circuit maintained isocapnia. During ga

Balancing lung and brain: physiological strategies for ARDS management in acute brain injury.

Acute respiratory distress syndrome (ARDS) is a common and clinically significant complication in patients with acute brain injury (ABI), affecting up to one-third of critically ill individuals and contributing to increased mortality, prolonged mechanical ventilation, and worse neurological outcomes. The coexistence of ARDS and ABI creates a fundamental therapeutic dilemma: strategies that protect the lung may adversely affect cerebral physiology, whilst neuroprotective targets may compromise respiratory management. This narrative review examined the pathophysiological interactions between the

Assessment of Respiratory Drive in the Mechanically Ventilated Patient.

Respiratory drive is a critical yet often underappreciated determinant of outcome during mechanical ventilation. Both insufficient and excessive drive can cause harm: Low drive promotes diaphragm atrophy, whereas high drive may contribute to lung and diaphragm injury. A key clinical challenge is the potential dissociation between drive and effort in patients with respiratory muscle weakness, where high drive may not translate to adequate muscular output. Simple bedside tools such as airway-occlusion pressure (P0.1) and whole-breath occlusion pressure (POCC) can identify patients at risk and gu

Carotid Body-Mediated Chemoreflex Function in Aging and the Role of Receptor-Interacting Protein Kinase.

Ventilatory impairment during aging has been linked to carotid body (CB) dysfunction. Anatomical/morphological studies evidenced CB degeneration and reductions in the number of CB chemoreceptor cells during aging. The mechanism(s) related to CB degeneration in aging remains elusive. Programmed cell death encompasses both apoptosis and necroptosis. Interestingly, necroptosis can be driven by molecular pathways related to low-grade inflammation, one hallmark of the aging process. Accordingly, we hypothesized that necrotic cell death dependent on receptor-interacting protein kinase-3 (RIPK3) may
