Sirtuin 1 Is a Potential Target for the Treatment of Neurogenic Intermittent Claudication by Modulating Pyroptosis.

Neurogenic intermittent claudication (NIC) pathogenesis associated with lumbar spinal stenosis (LSS) remains unclear. However, pyroptosis has been implicated in the pathogenesis of various central nervous system disorders. Therefore, the present study aimed to explore the potential role of pyroptosis in NIC progression. Additionally, the present study investigated the possible involvement of Sirtuin 1 (Sirt1), a protein recognized for its neuroprotective properties, in mitigating the progression of NIC by alleviating pyroptosis. In the current study, a rat model of NIC associated with LSS was

Association of Smoking With Postprocedural Complications Following Open and Endovascular Interventions for Intermittent Claudication.

IMPORTANCE: Smoking is a key modifiable risk factor in the development and progression of peripheral artery disease, which often manifests as intermittent claudication (IC). Smoking cessation is a first-line therapy for IC, yet a minority of patients quit smoking prior to elective revascularization. OBJECTIVE: To assess if preprocedural smoking is associated with an increased risk of early postprocedural complications following elective open and endovascular revascularization. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used nearest-neighbor (1:1) propensity score matc

Supervised Exercise Therapy is Effective for Patients With Intermittent Claudication Regardless of Psychological Constructs.

OBJECTIVE: According to current guidelines, supervised exercise therapy (SET) is the treatment of choice for intermittent claudication (IC). Little is known about the potential consequences of psychological factors on the effectiveness of treatment. The aim of this study was to determine possible associations between a set of psychological constructs and treatment outcomes, and to investigate whether self efficacy increased after SET. METHODS: This was a substudy of the ELECT Registry, a multicentre Dutch prospective cohort study in patients with IC receiving primary SET. A complete set of va

Symptoms of anxiety and depression and their relationship with barriers to physical activity in patients with intermittent claudication.

OBJECTIVES: Although the practice of physical exercise in patients with intermittent claudication (IC) is often encouraged, adherence is low. The difficulty in performing physical training may be related to the psychological characteristics of patients with claudication. To verify the association between anxiety and depression symptoms and barriers to physical exercise and walking capacity in patients with IC. METHODS: One-hundred and thirteen patients with a clinical diagnosis of IC were included in the study. Patients underwent clinical evaluation by a vascular surgeon, answered the Beck De

A Case of Polyarteritis Nodosa Presenting as Rapidly Progressing Intermittent Claudication of Right Leg.

BACKGROUND: Polyarteritis nodosa (PAN) is a medium vessel vasculitis which causes significant morbidity and mortality. Usually, it presents with constitutional symptoms with angiographic evidence of aneurysms or segmental stenosis of arteries of mesenteric or renal vasculature. It is exceedingly uncommon for PAN to present with symptomatic progressive intermittent claudication. CASE PRESENTATION: We describe a 60-year-old male who presented with rapidly progressive intermittent claudication of his right leg. He did not have any significant atherosclerotic risk factors. He had recent onset hyp

[A 56-year-old man with abdominal pain, intermittent claudication and a retroperitoneal mass].

OBJECTIVE: To describe an additional case of idiopathic retroperitoneal fibrosis. METHODS/RESULTS: A 56-year-old man consulted for abdominal pain and previous symptoms of intermittent claudication. A CT scan disclosed a retroperitoneal mass, for which reason the patient was submitted to surgery. Analysis of the biopsy specimen demonstrated retroperitoneal fibrosis. CONCLUSION: Although the clinical diagnosis of this disease is usually based on the CT findings, a histological analysis of the biopsy specimen provides a definitive diagnosis and permits intraperitonization of the ureters, which

Sex differences in outcomes of exercise therapy for patients with intermittent claudication: A scoping review.

Exercise therapy is first-line treatment for intermittent claudication due to peripheral artery disease. We sought to synthesize the literature on sex differences in response to exercise therapy for the treatment of intermittent claudication due to peripheral artery disease. A scoping review was performed (1997 to 2023) using Ovid MEDLINE, CINAHL (Cumulative Index to Nursing and Allied Health Literature), Embase, SPORTDiscus, and Web of Science. Articles were included if they were a scientific report of any measures of health-related quality of life or walking performance after an intervention

Radio-iodination and biological evaluation of pentoxifylline as a novel probe for diagnosis of intermittent claudication.

The diagnosis of intermittent claudication (IC) is challenging. Imaging with radiopharmaceuticals provides a new method for detecting acute IC. Pentoxifylline improves blood flow to ischemic tissues via increasing erythrocyte elasticity and inhibiting platelet aggregation. Pentoxifylline was radio-iodinated with radioiodine-131 (131I) through a direct electrophilic substitution reaction. Furthermore, various factors that might influence the radiolabeling strategy were investigated. The radiochemical yield of [131I]iodopentoxiphyline was evaluated by using paper chromatography and HPLC methods.

Neurogenic intermittent claudication caused by vasculitis in the cauda equina: an autopsy case report.

PURPOSE: Intermittent claudication (IC) refers to leg pain that is induced by walking and relieved by rest. Neurogenic IC is usually associated with lumbar canal stenosis (LCS). We present rare findings from an autopsied patient who had neurogenic IC caused by vasculitis in the cauda equina. METHODS: We performed antemortem neurological and electrophysiological assessments, sural nerve biopsy, and post-mortem examination of the spinal cord and brain. RESULTS: A 61-year-old man noted sudden-onset leg pain that was not associated with any traumatic trigger. His leg pain consistently appeared w

Supervised exercise therapy in intermittent claudication: a systematic review of clinical impact and limitations.

INTRODUCTION: The global prevalence of Peripheral Arterial Disease is growing. This condition is associated with serious hallmarks, such as impaired functional capacity and decreased quality of life. Reduced blood flow and oxygen delivery are its dominant mechanisms, although additional ones contribute to it. Exercise therapy is a cornerstone in the management of intermittent claudication and current guidelines recommend supervised exercise therapy as first-line treatment. EVIDENCE ACQUISITION: A search was conducted on PubMed database to identify studies which included supervised exercise th

12-Year-old girl with systemic lupus erythematosus complicated by gangrene and intermittent claudication.

Systemic lupus erythematosus (SLE) can present with various symptoms, including rare manifestations such as gangrene. This report describes a 12-year-old girl with SLE who presented with intermittent claudication and gangrene. Although intermittent claudication is rare in paediatric cases, it is essential to consider vascular diseases including those associated with SLE as a potential cause. The patient initially experienced pain, redness, and cold sensations in the right great toe accompanied by intermittent claudication, with symptoms worsening over time. Diagnostic imaging, including contra
