[Hemispherectomy and Hemispherotomy in the Treatment of Drug-Resistant Epilepsy].

Hemispherectomy/hemispherotomy is an important surgical procedure for patients with drug-resistant epilepsy who have a unilateral large epileptogenic lesion associated with neurological deficits, such as hemiparesis. Hemispherotomy consists of corpus callosotomy and dissection of projection fibers into the internal capsule. Hemispherotomy has two major approaches: periinsular lateral hemispherotomy(PIH)and vertical parasagittal hemispherotomy(VH). The white matter is incised along the periinsular sulcus to disrupt the internal capsule before corpus callosotomy is performed within the lateral v

Hypersomnolence-hyperkinetic movement disorder in a child with compound heterozygous mutation in 4-aminobutyrate aminotransferase (ABAT) gene.

Deficiency of gamma-amino-butyrate aminotransferase (ABAT) is a rare inherited disorder. A six-month-old girl presented with hyper-somnolence, hyperkinetic movements of distal extremities during wakefulness, hypotonia, bi-pyramidal signs, and impaired response to sound and visual stimuli. Brain MRI at five months showed restricted diffusion along the internal capsule and genu of corpus callosum. A follow up MRI at 18months, showed hyperintensities in brainstem, external and internal capsule, 'trilaminated' appearance of posterior limb of internal capsule and dysmyelination of sub-cortical whit

Parakinesia Brachialis Oscitans and Excessive yawning From Tumefactive Demyelination.

We present a case and video of a 31-year-old man with biopsy-confirmed tumefactive demyelination affecting the right internal capsule causing left hemiplegia, excessive yawning, and the curious but well-described phenomenon of parakinesia brachialis oscitans (PBO) with transient tonic elevation of his paralyzed arm while yawning. PBO is most commonly reported in ischemic stroke with internal capsule or pontomedullary brainstem lesions. Our case uniquely demonstrates this phenomenon in the case of tumefactive demyelination. We also highlight excessive yawning which has also been described in mu

Spontaneous rotation and propulsion of suspended capsules in active nematics.

We investigate the dynamics of elastic capsules suspended in two-dimensional active nematic fluids using lattice Boltzmann simulations. The capsules, modeled as flexible membranes enclosing active internal regions, exhibit a rich variety of behaviors shaped by their geometry and the interplay between internal and external activity. Circular capsules with active interiors undergo persistent rotation driven by internally confined +1/2 topological defects. Axisymmetric capsules, such as boomerangs, develop directed motion along their axis of symmetry due to unbalanced active forces generated by d

Quantitative DTI metrics in a canine model of Krabbe disease: comparisons versus age-matched controls across multiple ages.

Purpose The purpose of this study was to compare quantitative diffusion tensor imaging metrics in dogs affected with a model of Krabbe disease to age-matched normal controls. We hypothesized that fractional anisotropy would be decreased and radial diffusivity would be increased in the Krabbe dogs. Methods We used a highly reproducible region-of-interest interrogation technique to measure fractional anisotropy and radial diffusivity in three different white matter regions within the internal capsule and centrum semiovale in four Krabbe affected brains and three age-matched normal control brains

[Hypoglycemic Hemiplegia].

Hypoglycemia may cause acute hemiplegia. The most common diffusion-weighted MR imaging finding in patients with hypoglycemic hemiplegia is the hyperintense lesion involving the internal capsule, mimicking acute ischemic stroke. Thus, in patients with acute onset hemiplegia, it is important to differentiate hypoglycemia on arrival by immediate blood glucose measurement. It has recently been shown that hypoglycemic brain injury start in large white matter tracts such as internal capsule and spread throughout the whole brain, including the gray matter. However, it is still unclear why focal signs

[Ultrasonic diagnosis of liver hydatid disease].

The experience of the ultrasonic diagnosis of intrahepatic secondary changes in 36 cases with liver hydatidosis was reported. The ultrasonograms were classified into five types: I. separated internal capsule, II. disruptive internal capsule; III. mixed echo; IV. thick-wall calcification; and V. consolidation. We studied the ultrasonogram character of the five types, and considered that the occurrence of type I, type II, and type IV resulted from single cyst pattern, while type III and type V resulted from multiple cyst pattern. Analysing the pathological foundation of the ultrasonogram changes

Role of the testicular capsule in seasonal modulation of the testis.

While the seasonal testicular cycle has been well studied regarding internal components, no attention has been given to the testicular capsule (tunica albuginea and tunica serosa). This study elucidated the structure-function modulations of intra-testicular functions by its capsule in the finch red munia (Amandava amandava) during the annual testicular cycle. The birds were studied during breeding (preparatory and breeding) and nonbreeding (regressive and quiescent) reproductive phases using hematoxylin-eosin and acridine orange-ethidium bromide capsule staining, hormonal ELISA (LH and testost

Posterior superior internal impingement: an evidence-based review [corrected].

Over the last decade, the concept of internal impingement has continued to evolve, and the frequency with which this condition is recognised continues to increase. This syndrome should be clearly differentiated from the classical (external) impingement that is thought to be caused by compression of the subacromial bursa, long head of the biceps tendon and rotator cuff (RC) by the coraco-acromial arch. Internal (posterosuperior) impingement syndrome is typified by a painful shoulder due to impingement of the soft tissue, including the RC, joint capsule and the posterosuperior part of the glenoi

X-linked hereditary hemihypotrophy hemiparesis hemiathetosis.

Hereditary hemihypotrophy hemiparesis hemiathetosis syndrome (HHHH; OMIM 306960) was first reported in a family with congenital left hemiplegia in two males and moderately affected females. We describe a family with three males demonstrating congenital right hemiplegia with porencephalic lesions of the left internal capsule and putamen, or the periventricular white matter just above the internal capsule. Clinical findings within each family are limited to the same side, though sidedness differed between families. Both pedigrees were most consistent with X-linked inheritance. Genome-wide linkag

Brain microvascular endothelial cell transplantation ameliorates ischemic white matter damage.

Ischemic insults affecting the internal capsule result in sensory-motor disabilities which adversely affect the patient's life. Cerebral endothelial cells have been reported to exert a protective effect against brain damage, so the transplantation of healthy endothelial cells might have a beneficial effect on the outcome of ischemic brain damage. In this study, endothelin-1 (ET-1) was injected into the rat internal capsule to induce lacunar infarction. Seven days after ET-1 injection, microvascular endothelial cells (MVECs) were transplanted into the internal capsule. Meningeal cells or 0.2% b

A novel microporous polyurethane vascular graft: in vivo evaluation of the UTA prosthesis implanted as infra-renal aortic substitute in dogs.

A novel microporous polyurethane blood conduit developed at the University of Texas at Arlington was implanted as an infra-renal substitute in dogs. The prosthesis was fabricated by precipitating a solution of the polymer with dry nitrogen onto a rotating mandrel. The grafts were sterilized either by gamma radiation (series I) or ethylene oxide (series II); they were implanted for the following prescheduled periods: 4, 24, 48 hours, and 1 week (short-term) and 2, 4 weeks, 3 and 6 months (medium-term). The thrombohematological characteristics of each animal were evaluated prior to implantation

Microstructural changes in normal-appearing white matter in male sleep apnea patients are reversible after treatment: A pilot study.

Visually appreciable white matter (WM) changes have been described in obstructive sleep apnea (OSA). However, few data exist on the involvement of silent WM abnormalities. This prospective study investigated the microstructural integrity of normal-appearing white matter (NAWM) in male OSA patients before and after continuous positive airway pressure (CPAP) treatment, using a neuroimaging approach. Magnetic resonance imaging (MRI) was acquired from 32 participants (16 severe never-treated OSA and 16 controls). Diffusion tensor imaging (DTI) and Tract-Based Spatial Statistics (TBSS) were used to

Myelin Water Imaging Demonstrates Lower Brain Myelination in Children and Adolescents With Poor Reading Ability.

Magnetic resonance imaging (MRI) provides a means to non-invasively investigate the neurological links with dyslexia, a learning disability that affects one's ability to read. Most previous brain MRI studies of dyslexia and reading skill have used structural or diffusion imaging to reveal regional brain abnormalities. However, volumetric and diffusion MRI lack specificity in their interpretation at the microstructural level. Myelin is a critical neural component for brain function and plasticity, and as such, deficits in myelin may impact reading ability. MRI can estimate myelin using myelin w

White Matter Tract Pathology in Pediatric Anoxic Brain Injury from Drowning.

BACKGROUND AND PURPOSE: Although drowning is a leading cause of mortality and morbidity in young children, the neuropathologic consequences have not been fully determined. The purpose of this article was to quantitatively characterize white matter microstructural abnormalities in pediatric anoxic brain injury from nonfatal drowning and investigate the correlation with motor function. MATERIALS AND METHODS: Whole-brain T1-weighted and diffusion-weighted MR imaging datasets were acquired in 11 children with chronic anoxic brain injury and 11 age- and sex-matched neurotypical controls (4-12 year

Altered white matter integrity in individuals with cognitive vulnerability to depression: a tract-based spatial statistics study.

The microstructure of white matter in patients with major depressive disorder (MDD) has been demonstrated to be abnormal. However, it remains unclear whether these changes exist prior to the onset of disease. In this study, diffusion tensor imaging was used to evaluate white matter integrity in individuals who exhibited cognitive vulnerability to depression (CVD), MDD, and healthy controls (HC). Compared with the HC, MDD exhibited a lower fractional anisotropy (FA) in ten brain regions: the cerebral peduncle, the anterior and posterior limbs of the internal capsule (ALIC and PLIC), the externa

Central pain from cerebral abscess: thalamic syndrome in AIDS patients with toxoplasmosis.

We describe two patients with acquired immunodeficiency syndrome (AIDS) who developed classic thalamic syndrome (TS) due to Toxoplasma abscesses in the thalamic region. Treatment with amitriptyline provided substantial relief in both patients. Postmortem examination in one case revealed a lesion in the internal capsule and thalamic reticular nucleus. These observations indicate that (1) TS can result from an isolated lesion in the internal capsule and reticular nucleus of the thalamus, (2) cerebral abscess can cause classic TS, (3) central pain can be added to the many pain syndromes that affl

Relationship of ASPECTS Lesion Topography with Clinical Outcomes in Acute Ischemic Stroke Treated with Endovascular Thrombectomy: A Single-Center Cohort Study.

Objective: Although an Alberta Stroke Program Early CT Score (ASPECTS) < 7 is known to be associated with poor clinical outcomes in patients with acute ischemic stroke (AIS), the relationship between regional differences in infarct location within the ASPECTS territory and clinical outcome has not been fully clarified. The aim of this study is to evaluate the association between infarct area localization and clinical outcomes in AIS patients with large vessel occlusion and to investigate whether these regional patterns can be used to predict prognosis independently of the total ASPECTS.
