FI-RADS: an imaging-based scoring system for skeletal fluorosis-adjunctive clinical and biochemical analysis.

BACKGROUND: Skeletal fluorosis is a chronic bone disease caused by excessive fluoride exposure, leading to osteosclerosis, osteoporosis, and ectopic ossifications. Its radiological features and their links to age, joint function, and metabolism remain unclear, necessitating further study. This study aimed to develop a Fluorosis Imaging Reporting and Data System (FI-RADS) based on X-ray radiographs of multiple skeletal sites for the diagnosis of skeletal fluorosis in endemic regions. METHODS: This cross-sectional study aimed to establish the FI-RADS grading system by analyzing radiographic dat

A Distinctive Skeletal Phenotype in an Adult With IFITM5 -Associated Osteogenesis Imperfecta due to p.Ser40Leu Variant.

More than 100 patients have been reported with osteogenesis imperfecta due to disease-causing variants in IFITM5. Distinct features of this condition include ossification of the interosseous membrane and hypertrophic calluses. Most of them have a recurrent heterozygous variant in the 5'-UTR (c.-14C>T) of IFITM5. A few patients have a heterozygous missense variant in the first exon of IFITM5 [(c.119C>T) p.Ser40Leu]. These patients do not have the distinctive features that results from c.-14C>T variant. Fewer than 10 patients with the c.119C>T variant have been reported so far with most of them

Treatment of forearm fractures.

Fractures of the forearm represent common injuries. Understanding the anatomy and function of the radius, ulna, interosseous membrane, proximal and distal radioulnar joints is critical to appropriate management. Diagnosis can readily be made by examination and radiographs. Well established surgical approaches including the anterior Henry, dorsal Thompson, and ulnar approaches provide excellent access to both the radius and ulna. Multiple fracture patterns are recognized including isolated radius and ulna fractures, combined fractures, Galeazzi fractures, and Monteggia fractures. Surgical manag

The distal radioulnar joint.

A phylogenic review shows the DRUJ to be a highly evolved and specialized structure. The diarthrodial joint, in combination with the TFCC, ligament complex, and oblique fibers of the distal interosseous membrane, permit rotation while preserving stability and transmitting the load borne by the radiocarpal joint to both bones of the forearm. Our improved, yet imperfect, understanding of the anatomy, biomechanics, and pathophysiology of the region, abetted by new imaging modalities and wrist arthroscopy, permit more precise diagnosis and thus more rational treatment. The persistent inconsistency

Concomitant scapholunate dissociation and Kienböck's disease.

Six men had concurrent scapholunate dissociation and Kienböck's disease, a finding suggestive of a common cause. Five patients attributed the onset of wrist pain to a single traumatic event. Three had x-ray evidence of scapholunate dissociation before the onset of lunate osteonecrosis. Biomechanical factors that may be of significance are ulnar minus variance, lesser compliance of the triangular fibrocartilage, ulnar translation of the carpus at impact with shear fracture through the lunate, and disruption of the scapholunate interosseous membrane occurring under similar stress. Lunate osteone

[The Sarmiento method as a conservative treatment of tibial fractures (author's transl)].

We have treated 77 tibial fractures according to Sarmiento's principles. This technic was applied two weeks after the initial time of the fracture, and allowed a total weight bearing with complete joint mobility. It needed a functional below-the-knee brace which ends on the knee like a PTB prosthesis. The role of soft tissues (periosteum and interosseous membrane) was important to be considered in the stabilisation of fractures. The results were satisfactory, healing times being comparable to those observed after other orthopaedic methods. Sarmiento's method seems worthwhile because of it's in

Treating acute Essex-Lopresti injury with the TightRope device: a case study.

Essex-Lopresti injury consists of a fracture or dislocation of the radial head, rupture of the interosseous membrane (IOM), which is the main pathology, and a dislocated distal radio-ulnar joint. There are several reports in the literature, including cadaveric studies, which suggest an operative solution for this complicated injury. The torn IOM is not treated during the traditional operative repair. In the following paper, we suggest a treatment for the IOM by unloading it with the TightRope device. This device temporarily takes the tension off the torn IOM and assists in reduction and mainte

Posterior interosseous nerve palsy related to rheumatoid synovitis of the elbow.

Posterior interosseous nerve palsy (PINP) is a rare complication of rheumatoid arthritis of the elbow. A 58-year-old woman with rheumatoid arthritis, who complained of an inability to extend her left fingers, was referred to our hospital. After a series of studies, extensor tendon ruptures were excluded, and PINP was diagnosed. By means of the Henry anterolateral approach, the radial nerve was exposed, which was compressed by swollen synovial membrane at the Frohse arcade. Elbow synovectomy was performed, and the arcade was opened to release the nerve. The PINP has been recovered completely wi

Phenotype and genotype analysis of Chinese patients with osteogenesis imperfecta type V.

Osteogenesis imperfecta (OI) type V is an autosomal-dominant disease characterized by calcification of the forearm interosseous membrane, radial head dislocation, a subphyseal metaphyseal radiodense line, and hyperplastic callus formation. The causative mutation, c.-14C>T in the 5'-untranslated region of IFITM5, was recently discovered to be involved in this disease. However, in spite of the little genotypic variability, considerable phenotypic variability has been recognized in two cohorts of patients, the majority of whom were Caucasians. Using exome sequencing, we identified the same hetero

The forearm complex: anatomy, biomechanics and clinical considerations.

The forearm complex is comprised of the proximal radioulnar joint, middle radioulnar joint/interosseous membrane, and the distal radioulnar joint. These three areas function in a coordinated manner to rotate the hand in space and allow performance of functional tasks. If a structure or structures in one of these three areas is disrupted, this can adversely affect the function at any of the other two remaining areas. Surgical intervention focuses on restoring anatomical alignment to preserve the function of the forearm complex. Rehabilitation is guided by the relationships between the three are

[Spinal cord compression in bone fluorosis. Apropos of 4 cases].

The authors report four cases of spinal cord compression (three at cervical level and one at dorsal level) due to vertebral osteosclerosis secondary to chronic fluoride intoxication. Roentgenograms showed typical diffuse densification of vertebral bodies, calcifications of bony insertions of many ligaments, discs and interosseous membranes. Urinary fluoride was markedly increased in two cases. In the other two cases the bone biopsy was suggestive of skeletal fluorosis. Spinal computed tomography showed severe cord compression due to posterior osteophytes. Good improvement was observed after su

Posterior tibial-tendon transfer in patients with cerebral palsy.

The results of fifty-seven posterior tibial-tendon transfers through the interosseous membrane to the dorsum of the foot that were performed in fifty-one patients who had cerebral palsy, and who were followed for a mean of 9.3 years (range, five to twenty-six years), were evaluated in terms of pattern of gait, alignment of the foot, formation of callus on the sole of the foot, and requirements for bracing. A good or excellent result was achieved in twenty-seven of thirty feet in the hemiplegic patients, twelve of sixteen feet in the paraplegic patients, and two of eleven feet in the quadripleg

Missed Essex-Lopresti Injury-Development of a Combined Proximal and Distal Radio-Ulnar Joint Prosthesis as a Treatment Option and Proof of Concept.

Essex-Lopresti injuries are characterized by injuries to the proximal radio-ulnar joint, the distal radio-ulnar joint, and the interosseous membrane. This can lead to osteoarthritis, impaction syndrome, or instability. If all three structures are injured and lead to instability, the situation is almost unmanageable and many times ends in a one-bone forearm. In this article, we demonstrate a new way to reconstruct the proximal and distal radio-ulnar joint with two patient-specific coupled prostheses. These have been developed with the biomechanical conditions of the forearm in mind, where there

[Treatment of the radial shaft loss with pedicled graft from the ulna--a case report].

A case of 68-year-old woman with severe deformation of the left forearm and disability of the hand as a consequence of total dissociation and partial loss of radial shaft, after fracture sustained one year earlier is reported. The restoration of the length and shape of the radius was performed with the use of 4 cm long fragment of the ulna pedicled on the interosseous membrane, fixated with plate. Normal shape of the radius was achieved and the hand was held in the axis of the forearm. Bone healing was achieved at one year, and at the 2.5 years follow-up the patient was free of pain, had full

Tripolar Dislocation: Elbow Terrible Triad-Essex-Lopresti Injury-Trans-scaphoid Perilunate Dislocation: A Case Report.

CASE: A 23-year-old man presented with a right upper limb injury after a 10-m fall. Initial evaluation demonstrated a terrible triad elbow injury associated with a trans-scaphoid perilunate dislocation. Elbow stabilization with radial head replacement and carpal fixation was performed. Radiographs on postoperative day 7 demonstrated an ipsilateral Essex-Lopresti injury, which had been initially missed, and revision surgery was performed to reconstruct the interosseous membrane. CONCLUSION: Surgeons should maintain a high degree of suspicion for an ipsilateral Essex-Lopresti injury in patients

[Peroneal Nerve Palsy Management by Tibialis Posterior Muscle Transposition - Anatomical and Clinical Study].

PURPOSE OF THE STUDY Transposition of tibialis posterior muscle ranks among the methods of dorsiflexion restoration in patients with peroneal nerve palsy. Even though this method is commonly used, anatomical variations are still encountered which make us modify the established procedures. The purpose of this study is to evaluate the functional outcomes of operated patients and based on the clinical experience to define by cadaver preparation the anatomical causes preventing the use of the standard transposition technique. MATERIAL AND METHODS The clinical group includes 21 patients (15 men, 6

Traumatic medial malleolar fracture of a fibula flap donor leg.

A 50-year-old male presented with a T4 N0 squamous cell carcinoma of the floor of the mouth and alveolus. Treatment included a partial mandibulectomy with a free osseocutaneous fibula flap reconstruction. He made a good postoperative recovery and was given adjuvant radiotherapy. No problems were reported with the donor site or ankle. Five months after harvesting the flap the patient sustained a fracture of the medial malleolus having jumped over a ditch playing golf. This was treated successfully with a below knee plaster of Paris cast. The interosseous membrane was not ruptured in the acciden

Avoiding pitfalls in ankle fracture-dislocations: A case report of a dislocated tibialis posterior tendon causing persistent ankle subluxation.

Tibialis posterior tendon dislocation, a rare complication in ankle fracture-dislocations, can impede anatomical reduction of the ankle mortise. We report on a 59-year-old healthy male with an open fracture-dislocation of the right ankle. Despite multiple attempts under direct vision of the anterior syndesmosis, anatomical reduction of the ankle mortise was not possible. Soft tissue windows in a computed tomography (CT) scan revealed the dislocated tibialis posterior tendon to be the impeding structure. At the level of the fibula fracture the tendon passed through the interosseous membrane ant

MRI of the tibioastragalus anticus of Gruber muscle: a rare accessory muscle and normal anatomical variant.

We present the case of a 31-year-old man who sustained a hyperplantar flexion injury of his right ankle, and was evaluated using computed tomography and MRI to assess for osseous and ligamentous injury. The MRI and CT studies demonstrated a tibioastragalus anticus of Gruber (TAAG) muscle in the lower limb's anterior compartment. To our knowledge, the imaging of this muscle has not been previously described. The TAAG muscle arises from the lower third of the anterolateral tibia and the interosseous membrane. Its tendon passes laterally, deep to the tibialis anterior and extensor hallucis longus

Wide excision of the distal ulna: biomechanical testing of a salvage procedure.

PURPOSE: To study changes in the relationship of the distal radius and ulna after progressive resection of the distal ulna. METHOD: Six-degree-of-freedom motion sensors were used to determine relative motion and a displacement transducer was used to determine strain changes in the interosseous membrane (IOM). These were applied in cadaveric specimens loaded to simulate wrist motion and axial compressive forces. RESULTS: Strain in the IOM decreased with increasing resection of the distal ulna. The distance between the radius and ulna decreased with increasing resection up to 20% of ulna lengt
