Experience with dorsal column stimulation for relief of chronic intractable pain: 1968-1973.

Dorsal column stimulators (DCS) have been implanted in 130 patients with various chronic pain syndromes at the University of California, San Francisco, between 1969 and 1973. Preoperative psychiatric evaluation and percutaneous dorsal column stimulation testing were of value in rejecting those patients most likely to have unsatisfactory long-term results with DCS. Best results occurred in patients with phantom limb or peripheral nerve pain and worst results in patients with paraplegic pain, documented arachnoiditis, pancreatitis and arthritis. The need is stressed for careful preoperative sele

Botulinum Toxin Injection for Intractable Pain in Cancer Patients With Psoas Muscle Invasion.

Muscle invasion (MI) in patients with far-advanced cancer is often accompanied with pain. Conventional treatments used for pain relief may be associated with several side effects. We describe two cases of botulinum toxin injection (BTI) for relieving intractable pain in patients of far-advanced cancer having MI. The patients presented with persistent intractable pain that did not respond to conventional treatments, such as analgesic medications, epidural steroid injection, and radiotherapy. BTI relieved the intractable pain in these cases for at least 9-12 weeks without any reported side effec

The factors driving self-efficacy in intractable chronic pain patients: a retrospective study.

BACKGROUND: The fear-avoidance model is a theoretical paradigm for explaining acute and chronic pain. In this model, pain catastrophizing plays an important role. On the other hand, self-efficacy influences whether patients view their pain optimistically, ultimately preventing the conversion of pain into intractable pain. The aim of the present study was to evaluate the factors that influence self-efficacy in patients with chronic pain. METHODS: Study participants included 147 outpatients (35 men, 112 women) with intractable chronic pain who visited our hospital between September 2014 and Jul

Slipping Rib Syndrome in a Female Adult with Longstanding Intractable Upper Abdominal Pain.

Slipping rib syndrome is a rare cause of abdominal or lower chest pain that can remain undiagnosed for many years. Awareness among health care personnel of this rare but significant disorder is necessary for early recognition. Prompt treatment can avoid unnecessary testing, radiographic exposure, and years of debilitating pain. A 52-year-old female was evaluated for a 3-year history of recurrent abdominal and lower chest pain. Pain was sharp, primarily located in the lower chest and subcostal region left more than right, waxing and waning, nonradiating, and aggravates with specific movements.

Sporadic NF2 Mosaic: Multiple spinal schwannomas presenting with severe, intractable pain following pregnancy.

The aim of the present paper is to report undiagnosed sporadic neurofibromatosis type 2 presenting with symptomatic compressive spinal tumors following pregnancy. A 36-year-old woman experienced progressive, severe lumbar radicular pain in the second trimester of pregnancy which became intractable soon after delivery. Magnetic resonance imaging revealed a complex heterogeneous hypointense mass lesion around the conus. There were two small punctate lesions in the cauda equina suggestive of myxopapillary ependymoma with 'drop metastases.' The patient underwent surgical resection of two cystic co

Pulsed radiofrequency for chronic intractable lumbosacral radicular pain: a six-month cohort study.

BACKGROUND AND OBJECTIVES: There is little evidence concerning the medical management of lumbosacral radicular pain. The prognosis for patients suffering pain for more than 3 months is poor. Pulsed radiofrequency (PRF) treatment of the dorsal root ganglion (DRG) has been suggested as a minimally invasive treatment. We studied the effect on pain and quality of life of PRF treatment of the DRG in patients with chronic, severe lumbosacral radicular pain. METHODS: Patients with lumbosacral radicular pain were screened to select a homogeneous population. PRF treatment of the DRG was performed at L

Cordotomy for Intractable Cancer Pain: A Historical and Technical Narrative Review with Modern Perspectives.

<p>Background: Percutaneous cervical cordotomy (PCC) is a lesioning procedure that targets the anterolateral quadrant of the spinal cord to interrupt spinothalamic pain pathways. It provides rapid and durable analgesia for patients with unilateral, medically refractory cancer pain below the C5 dermatome. Although its utilization has declined with the expansion of pharmacological and neuromodulatory therapies, PCC remains a critical palliative intervention for patients in whom opioid therapy is ineffective or intolerable. Summary: This narrative review delineates the neuroanatomical underpinnin

Multiple myeloma with intractable lumbar pain and diagnostic challenge with MRI: A case report.

We present a 63-year-old male patient with intractable bone pain and rapidly progressive osteoporosis, who was diagnosed with multiple myeloma (MM) by CT despite normal magnetic resonance imaging (MRI) findings. The gold standard diagnostic modality for MM is MRI as it can be used to sensitively evaluate bone marrow, however, the current case highlights that MRI is not always accurate in evaluating MM. CT in combination with MRI could be used for secondary osteoporosis with intractable bone pain in order to determine the diagnosis, treatment, and prognosis.

Patterns of chronic inflammation in extensively treated patients with arachnoiditis and chronic intractable pain.

OBJECTIVE: To use biomarkers to gain insight into and gauge the residual (post-treatment) level of inflammation in two groups of intensively treated patients with severe chronic pain. METHODS: Three study groups were analyzed, and included: (i) patients (n = 90) with chronic intractable pain (CIP), (ii) patients (n = 26) with chronic pain and MRI-documented arachnoiditis (ARC) and (iii) normal subjects without a diagnosis of chronic pain (n = 86). We determined and compared the serum concentrations of Alpha-1 Antitrypsin (A1AT), Myeloperoxidase (MPO) and soluble Tumor Necrosis Factor receptor

The management of a person with haemophilia who has a fixed flexed hip and intractable pain.

The clinical picture of a fixed flexed hip associated with pain in a person with haemophilia is suggestive of a haemorrhage in that area. Sonography facilitates differentiation between a haemarthrosis, intraperitoneal haemorrhage, subperiosteal bleed, a bleed into the soft tissue around the hip joint or a psoas haematoma. All these aforementioned causes may result in the same clinical presentation. Two cases are described in which coxhaemarthrosis resulted in a flexion contracture of the joint associated with severe intractable pain. Narcotic drugs failed to alleviate the severe pain. Joint as

Experience with dorsal column stimulation (DCS) in the operative treatment of chronic intractable pain.

The results of DCS in 50 patients are altogether favourable; they are better for the group of amputation and phantom pain than for the group of peripheral nerve lesions. For patients with malignoma pain no valid comment can be given. The influence of DCS is mostly immediate in bouts of acute pain, slower in chronic permanent pain, where a longer period of stimulation is needed. The observation of such longer stimulation periods together with the experience that during DCS there is a shift in the pain threshold, even in segments above the stimulus including also the face, seems to indicate a pa

Buprenorphine for the Management of Intractable Pain in Skin Ulcers Associated With Calcific Uremic Arteriolopathy.

BACKGROUND: Calcific uremic arteriolopathy, or calciphylaxis, is a rare and severe condition marked by calcification of small-to-medium blood vessels, causing tissue ischemia and necrosis. The pain is excruciating, emanating from tender lesions, severely impacting movement and touch. No approved treatment exists, making effective pain management vital for improving quality of life and outcomes. CASE REPORT: A woman with end-stage renal failure and calciphylaxis-related ulcers on the inferior pannus, left medial thigh, and right gluteus experienced refractory pain despite opioid therapy. Dexmedetomidine use for patients in palliative care with intractable pain and delirium: A retrospective study.

Patients seen by the palliative care team often have difficult and intractable symptoms. The current standard of practice to manage these symptoms is the deeply sedating midazolam continuous subcutaneous infusion for patients who are expected to expire within hours to days. Dexmedetomidine provides sedation but lacks evidence in palliative care use. This study describes continuous subcutaneous infusion of dexmedetomidine's effect on refractory pain and delirium. Retrospective, observational chart review and conducted in accordance with SQUIRE (quality improvement study). Twenty adult patients

Intractable Neck Pain in an Oncologic Palliative Care Setting: Is Cancer Always the Answer?

Refractory pain is a common manifestation in an oncologic palliative care setting and represents a major challenge for health care professionals involved in care provision. The underlying neoplasm and its dissemination are the foremost pathophysiologic mechanism for the development of pain in patients with advanced cancer. Nonetheless, other etiologies such as trauma and infections need to be considered by clinicians in this particular care setting. The authors present the case of a patient with a recent diagnosis of hepatocellular carcinoma, suffering from intractable neck pain, progressive w

[Effect of the sacroiliac ligament block on intractable low back pain in elderly patients].

BACKGROUND: Lumbar spine disease in the elderly people is complicated by a variety of pathophysiology in the spine and the cause of the pain is unclear. Diagnosis of pain may be difficult in patients with pain in the thigh and groin area. Sacroiliac joint is supporting the trunk and movable joint. We examined the effect of the sacroiliac joint block for intractable low back pain. METHODS: Retrospectively we examined the duration of disease in patients with hip and leg pain visiting the hospital for eight months, and we questioned the site of pain awareness. Cryoanalgesia in the management of intractable pain in the temporomandibular joint: a five-year retrospective review.

Cryoanalgesia is a controversial adjunct to the management of chronic pain, but we know of no studies that have investigated its effect in the management of temporomandibular joint (TMJ) pain. In this five-year retrospective study we treated 17 patients who had severe pain that had failed to respond to all forms of conventional conservative treatment and were not appropriate for simple open operation. None had a clear indication for open operation on the joint or had too severe disease to warrant a simple procedure. Preliminary diagnostic injections of bupivacaine to the TMJ relieved the pain.
