Perioperative considerations in the management of tracheotomy for the obstructive sleep apnoea patient. Three illustrative case reports.

The perioperative course of three patients with obstructive sleep apnoea who underwent tracheotomy is presented as illustrative of the potentially life-threatening complications of management. It is concluded that this disease demands strict adherence to basic principles of airway management and close observation extending into the period after operation.

Anesthesia for meningioma surgery.

Patients receiving treatment for a meningioma require anesthesia when undergoing open craniotomy and, in some cases, during preoperative tumor embolization and radiosurgery. Adequate anesthesia management is integral to patients' perioperative care, which consists of the three phases of preoperative assessment and optimization, intraoperative care, and postoperative recovery. The preoperative anesthesia evaluation focusses on the cardiorespiratory and neurologic systems, as well as the airway, but also extends to ensure the optimal treatment of significant comorbidities before surgical interve

Perioperative and Intraoperative Considerations for Gynecology Patients With Disabilities: A Review.

In 2022, ∼30% of adult women in the United States reported living with a disability, and yet, there has been a paucity of research performed for this population, particularly regarding perioperative/operative gynecologic care. In this review, we sought to compile existing research as well as extrapolate findings from other surgical fields. Our review is separated by physical and intellectual disabilities with contributions from Case Reports to Expert Opinion to Scoping Reviews. The most consistent evidence-based recommendation was the benefit of a multidisciplinary team. Otherwise, most recomm

Optimizing pre-, intra-, and postoperative management of patients with sellar pathology undergoing transsphenoidal surgery.

OBJECTIVE: Perioperative management of patients with sellar lesions is complex, requiring input from a multidisciplinary team of specialists for ongoing management of both endocrinological and neurosurgical issues. Here, the authors reviewed the experience of a single multidisciplinary center over 10 years to identify key postoperative practices that ensure positive outcomes for patients with sellar lesions who undergo transsphenoidal surgery. METHODS: The authors performed a retrospective review of all transsphenoidal operations carried out by the senior author at a single center from April

Intraoperative imaging of the common bile duct: a systematic review.

BACKGROUND: Benign biliary disease is a common gastrointestinal condition often requiring surgical intervention. Serious complications during minimally invasive cholecystectomy, such as bile duct injury (BDI) and retained common bile duct stones, must be minimized to improve outcomes and optimize resource use. This systematic review summarizes data comparing intraoperative cholangiography (IOC), fluorescent imaging (FI) with indocyanine green (ICG), and laparoscopic ultrasound (LUS) during laparoscopic cholecystectomy. METHODS: A systematic review was conducted to address key questions (KQs)

Short interruptions between pre-warming and intraoperative warming are associated with low intraoperative hypothermia rates.

BACKGROUND: Prevention of inadvertent hypothermia is recommended for procedures >30 minutes because hypothermia increases the risk of myocardial ischemia, intraoperative blood loss, transfusion and wound complications. Therefore, short warming interruptions between pre-warming and intraoperative warming might result in lower hypothermia rates. The aim of this retrospective investigation was to determine whether the incidence of inadvertent intraoperative hypothermia was affected by the warming interruption. METHODS: The lowest intraoperative body core temperature value and the warming interru

Head trauma. Perioperative nursing implications.

1. The highest mortality rate in the adult trauma patient population is among those presenting with acute head trauma and associated multiple trauma. 2. Motor vehicle crashes are the leading cause of head trauma, with alcohol being a primary associated factor. The other major mechanisms of head trauma are falls and assaults. 3. Secondary brain injury is a result of brain swelling and intracranial hemorrhage causing hypoxia and ischemia of the brain tissue. 4. The goals of surgical intervention are to evacuate intracranial hematoma and prevent increases in intracranial pressure.

Standard Intraoperative Fluid Practices During the 2024 Intravenous Fluid Shortage.

IMPORTANCE: A nationwide intravenous fluid shortage in 2024 prompted hospitals to substantially reduce intraoperative crystalloid administration, creating a natural experiment to examine standard intraoperative fluid practices. The clinical consequences of such abrupt reductions in fluid use remain unclear. OBJECTIVE: To evaluate the associations among a marked reduction in intraoperative crystalloid administration, perioperative outcomes, and costs of care. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted from September 2 to December 31, 2024, across a hospit

Living liver donor safety: Intraoperative considerations in living liver donation-Guidelines from the ILTS-iLDLT group consensus conference 2025.

The success of living donor liver transplantation (LDLT) has bridged the growing gap between organ demand and availability in patients with liver failure or liver cancer. Safety of the living liver donor (LLD) is paramount, and astute intraoperative management plays a critical role in ensuring optimal outcomes for them. Given that intraoperative surgical and anesthesia practices vary widely across centers, the International Liver Transplantation Society (ILTS) and the International Living Donor Liver Transplantation Group (iLDLT Group) convened a global consensus conference in Toronto in March

PeriOperative Quality Initiative (POQI) international consensus statement on perioperative arterial pressure management.

Arterial pressure monitoring and management are mainstays of haemodynamic therapy in patients having surgery. This article presents updated consensus statements and recommendations on perioperative arterial pressure management developed during the 11th POQI PeriOperative Quality Initiative (POQI) consensus conference held in London, UK, on June 4-6, 2023, which included a diverse group of international experts. Based on a modified Delphi approach, we recommend keeping intraoperative mean arterial pressure ≥60 mm Hg in at-risk patients. We further recommend increasing mean arterial pressure tar

[Difficult airway management in a patient with history of tracheotomy].

A 12-year-old girl was scheduled to undergo tympanoplasty. She had received tracheotomy at the age of 10 months. As preoperative bronchofiberscope could reach the glottis with no difficulty, we chose rapid induction. Suddenly after the administration of thiopental, fentanyl and vecuronium bromide, we could not keep mask ventilation. As sniffing position and insertion of airway could not improve difficult ventilation, we intubated quickly. The intubation was easy. We consider that difficult ventilation was not due to upper airway trouble but due to tracheotomy. Muscles around the granuloma were

Monitoring and cerebral protection during carotid endarterectomy.

Two recently published multicentre trials have confirmed the overall benefit of carotid endarterectomy in symptomatic patients with severe carotid artery disease. The key to improving further the long-term advantages of carotid endarterectomy, however, remains the continued reduction of the initial operative risk. While the principal responsibility for this continues to be borne by the surgeon, specifically in reducing technical error, the time is perhaps approaching when he or she might also be able to apply some of the recent advances in cerebrovascular research to reduce operative morbidity

Managing Opioid-Tolerant Patients in the Perioperative Surgical Home.

Management of acute postoperative pain is important to decrease perioperative morbidity and improve patient satisfaction. Opioids are associated with potential adverse events that may lead to significant risk. Uncontrolled pain is a risk factor in the transformation of acute pain to chronic pain. Balancing these issues can be especially challenging in opioid-tolerant patients undergoing surgery, for whom rapidly escalating opioid doses in an effort to control pain can be associated with increased complications. In the perioperative surgical home model, anesthesiologists are positioned to coord

Present indications and future expectations of ultrasound in surgery.

With the improvement of resolution in the ultrasound image, this technique has become more and more popular as a diagnostic means in various fields of medicine. Surgeons use diagnostic ultrasound pre-, intra- and postoperatively. Preoperatively, it is mainly employed for tumour staging, differential diagnosis of the acute abdomen, assessment of intra-abdominal and intrathoracic fluid in polytrauma and lately for arthrosonography. Intraoperative ultrasound has developed into one of the most important tools in intraoperative decision making in surgery of the liver, the bile ducts and the pancrea

Cystoscopy at the time of benign hysterectomy: a decision analysis.

BACKGROUND: Gynecologists debate the optimal use for intraoperative cystoscopy at the time of benign hysterectomy. Although adding cystoscopy leads to additional up-front cost, it may also enable intraoperative detection of a urinary tract injury that may otherwise go unnoticed. Prompt injury detection and intraoperative repair decreases morbidity and is less costly than postoperative diagnosis and treatment. Because urinary tract injury is rare and not easily studied in a prospective fashion, decision analysis provides a method for evaluating the cost associated with varying strategies for us
