STUDY OBJECTIVE: To describe a series of intraperitoneal perforated intrauterine contraception devices (IUDs) and to discuss associated findings, methods for diagnosis, and management of this complication.
DESIGN: Retrospective review of surgical database between 1998 and 2012 (Canadian Task Force classification II-2).

BACKGROUND: Uterine perforation by intrauterine devices (IUDs) is a rare but well recognized complication. In the past, the presence of adhesions and perforation of viscera often resulted in the need for a laparotomy to remove the IUD. However, advances in laparoscopic technique have allowed surgeons to safely retrieve perforated IUDs. In this review, we analyze uterine perforation by an IUD and assess laparoscopic vs. open methods for removal of a perforated IUD.

A woman in her 40s presented with persistent lower urinary tract symptoms (LUTS) over 6 months, including suprapubic pain and dysuria. Initial investigations were inconclusive until non-contrast CT revealed a large bladder stone encapsulating an intrauterine contraceptive device (Copper-T). Pelvic pain is a common chief complaint in the acute care setting. As intrauterine devices continue to grow in prevalence, incidence of complications will naturally increase. The intrauterine contraceptive device (IUD) has been in use for many years as an effective means of birth control. Migration of the device from the uterus to the pelvic cavity is not uncommon and has been reported previously, however intravesical migration and secondary calculus formation is relatively rare. We report a 28-year-old woman in whom an intrauterine contraceptive device (LNG-IUS) migrated from the uterus to the bladder and resulted in stone formation. This case shows that the newer hormone releasing IUDs may also cause bladder perforation.

Intravesical migration of intrauterine device (IUD) is a rare complication. We report a case of IUD that migrated into the bladder of a 31-year-old woman who had it for four years, and presented with irritative bladder symptoms following a miscarriage. Since its introduction in 1959 the plastic intra-uterine contraceptive device (IUCD) has been accepted as a safe and convenient means of preventing conception. Nevertheless, use of the IUCD has been attended by complications, notably perforation of the uterus by the device. A case of uterine perforation by an Essure microinserts placed in proximity to the tubal ostia mimicking proper microinsert placement and bilateral tubal occlusion on imaging techniques is presented. The presence of Essure microinserts did not interfere with implantation and pregnancy, and in this case, no adjacent intraabdominal tissue damage was caused by the misplaced device, which was finally removed almost 3 years after insertion.

The use of colposcopy as an office procedure is shown to be an effective method in localizing the IUD's string deep in the cervical canal, and for following those patients who wish to continue with IUD, thus avoiding replacement of the device. In 24 patients with missing string, colposcopy was used, and was found to be an effective procedure which obviates the use of unnecessary x-ray exposure, invasive procedures and expensive ultrasonography.
