[Therapy and results of fresh lateral instability of the upper ankle joint].

During a retrospective study we especially examined the late outcome in therapy of injuries of the lateral capsula ligament-lesions in 210 patients. In addition we scrutinized the literature on this subject within the last 60 years, concerning the results of different therapy methods were checked. 203 collective writings on this subject, 23,279 patients and our own results achieved in the BG Accident Hospital in Murnau (Germany) all added up to 23,389 patients. We did not only observe the subjective opinion regarding the outcome of the therapy but also the socio-economic factors, such as the d

[Arthrosis of the wrist joint due to carpal instability. Therapeutic alternatives].

A kinematic analysis of the movement of the scaphoid, the lunatum and the triquetrum has shown that their turning axes do not correspond with each other. Without arthritis, and within the range of normal motion, the bones move without imposing any appreciable load on the ligaments. They merely guide the bones and hold them together. In the case of arthritic disorders and in states where abnormal frictional resistance occurs, or following trauma, the ligaments are highly loaded. This could lead to increased wear and ruptured ligaments. A series of 20 arthroscopies has shown that LT and SC ligam

Chronic Acromioclavicular Joint Instability: Outcomes after Stabilization without Tendon Graft Augmentation.

INTRODUCTION: Chronic acromioclavicular joint (ACJ) instability only rarely causes significant symptoms. In the uncommon cases with substantial pain and impairment, surgical treatment may become necessary. Many surgeons recommend tendon-graft augmentation to reconstruct the coracoclavicular (CC) ligaments. However, it remains unclear whether graft use is mandatory in all chronic cases. This study evaluates clinical and radiographic outcomes after open reduction and stabilization of chronic ACJ dislocations without grafting. MATERIALS AND METHODS: Sixteen consecutive patients with chronic, sym

Postoperative Rehabilitation of Anterior Glenohumeral Joint Instability Surgery: A Systematic Review.

Anterior glenohumeral instability is the most common form of shoulder instability. The systematic review summarizes the latest research on rehabilitation after Bankart repair. Inclusion criteria included postoperative rehabilitation and published in English between 2000 and 2019. Studies were excluded if they were addresses, comments, or editorials, or included other shoulder injuries or cadaver models. Two rounds of review using Rayyan QCRI software were performed for screening and full text search, and the articles were graded for levels of evidence. Of the 1982 articles, 14 articles were in

Neonatal screening for hip joint instability. Its clinical and economic relevance.

Effort to determine the efficacy of neonatal hip examination began in 1967. During the subsequent 23 years, there have been three analyses of the study's data: (1) a retrospective review reported in 1981, (2) a prospective examination in 1989, and (3) an economic evaluation in 1990. The retrospective review concluded that a neonatal hip screening program effectively screens for acetabular dysplasia, and early treatment seems to alter the outcome of congenital dislocation of the hip. The prospective examination, involving 62 newborns diagnosed as having dislocated or dislocatable hips, establis

Assessment of Relationships Between Joint Motion Quality and Postural Control in Patients With Chronic Ankle Joint Instability.

Study Design Controlled laboratory study, cross-sectional. Background Lateral ankle sprains are among the most common injuries encountered during athletic participation. Following the initial injury, there is an alarmingly high risk of reinjury and development of chronic ankle instability (CAI), which is dependent on a combination of factors, including sensorimotor deficits and changes in the biomechanical environment of the ankle joint. Objective To evaluate CAI-related disturbances in arthrokinematic motion quality and postural control and the relationships between them. Methods Sixty-three

Altered neuromuscular control and ankle joint kinematics during walking in subjects with functional instability of the ankle joint.

BACKGROUND: The ankle joint requires very precise neuromuscular control during the transition from terminal swing to the early stance phase of the gait cycle. Altered ankle joint arthrokinematics and muscular activity have been cited as potential factors that may lead to an inversion sprain during the aforementioned time periods. However, to date, no study has investigated patterns of muscle activity and 3D joint kinematics simultaneously in a group of subjects with functional instability compared with a noninjured control group during these phases of the gait cycle. PURPOSE: To compare the p

Improving Metacarpophalangeal Joint Instability by Joint Implantation in Parallel From a Supernumerary Thumb in Severely Hypoplastic Duplicated Thumbs.

Here, we present 2 cases of a severely hypoplastic duplicated thumb classified as Wassel types 5 and 6 and discuss whole-joint implantation from a supernumerary thumb as an alternative to stabilize the hypoplastic metacarpophalangeal (MCP) joint. The aim of the surgical treatment of thumb polydactyly is to reconstruct a functional and aesthetically pleasing thumb. Hypoplasia of joint components and abnormal tendon alignment lead to unpleasing results with angular deformity of the reconstructed thumb. In 2 cases, the MCP joint of the dominant digit was hypoplastic and unstable in all directions

Cervical facet joint effusion: A sign of instability in cervical degenerative spondylolisthesis.

AIMS: The aim of this study was to trace the association between cervical facet joint effusion and cervical degenerative spondylolisthesis (CDS). CDS has not received as much attention as its lumbar counterpart. Identification of features of instability on magnetic resonance imaging (MRI) is crucial to avoid missing presence of CDS. MATERIALS AND METHODS: The authors retrospectively reviewed cervical spine MRI scans and upright lateral flexion-extension radiographs of 17 consecutive patients at a single institution between January 2017 and June 2018 that revealed CDS. Presence of cervical fac

Seeing Beyond Morphology-Standardized Stress MRI to Assess Human Knee Joint Instability.

While providing the reference imaging modality for joint pathologies, MRI is focused on morphology and static configurations, thereby not fully exploiting the modality's diagnostic capabilities. This study aimed to assess the diagnostic value of stress MRI combining imaging and loading in differentiating partial versus complete anterior cruciate ligament (ACL)-injury. Ten human cadaveric knee joint specimens were subjected to serial imaging using a 3.0T MRI scanner and a custom-made pressure-controlled loading device. Emulating the anterior-drawer test, joints were imaged before and after arth

Joint Coordination and Stiffness During Landing in Individuals With Chronic Ankle Instability.

The purpose of the present study was to examine the effect of chronic ankle instability (CAI) on lower-extremity joint coordination and stiffness during landing. A total of 21 female participants with CAI and 21 pair-matched healthy controls participated in the study. Lower-extremity joint kinematics were collected using a 7-camera motion capture system, and ground reaction forces were collected using 2 force plates during drop landings. Coupling angles were computed based on the vector coding method to assess joint coordination. Coupling angles were compared between the CAI and control groups

Computational approach to correcting joint instability in patients with recurrent patellar dislocation.

Patellar dislocation is a debilitating injury common in active adolescents and young adults. Conservative treatment after initial dislocation is often recommended, but almost half of these patients continue to suffer from recurrent dislocation. The objective of this study was to compare preoperative patellofemoral joint stability with stability after a series of simulated procedures, including restorative surgery to correct to pre-injury state, generic tibial tubercle osteotomy, patient-specific reconstructive surgery to correct anatomic abnormality, less invasive patient-specific surgery, and

Radial shaft fracture obliquity as a predictor of distal radioulnar joint instability.

UNLABELLED: We assessed whether radial shaft fracture obliquity measurements on radiographs could predict intra-operative distal radioulnar joint instability. We also clinically validated previously described predictors of distal radioulnar joint instability, which included a fracture line within 7.5 cm of the lunate fossa, radial shortening >5 mm, and ulna styloid fracture. We retrospectively analysed the radiographs of all surgically managed patients in our unit with radial shaft fractures from 2006 through 2016. The degree of obliquity was analysed on the basis of the maximum fracture-line

Novel posterior artificial atlanto-odontoid joint for atlantoaxial instability: a biomechanical study.

OBJECTIVE Atlantoaxial instability is usually corrected by anterior and/or posterior C1-2 fusion. However, fusion can lead to considerable loss of movement at the C1-2 level, which can adversely impact a patient's quality of life. In this study, the authors investigated the stability and function of a novel posterior artificial atlanto-odontoid joint (NPAAJ) by using cadaveric cervical spines. METHODS The Oc-C7 regions from 10 cadaveric spines were used for anteroposterior (AP) translation and range of motion (ROM) tests while intact and after destabilization, NPAAJ implantation, and double-ro
