OBJECTIVE: To describe the effect of knee symptoms and radiographic osteoarthritis (ROA) on the risk of falls, recurrent falls, and fractures.
DESIGN: Participants from the Osteoarthritis Initiative were classified as having 'no', 'unilateral' or 'bilateral' knee symptoms (≥19 on a 0-96 Western Ontario and McMaster Universities Osteoarthritis Index) and ROA (Kellgren-Lawrence grade ≥2) for each visit. Self-reported falls and fractures in the past 12 months were extracted at baseline and follow-up visits until month 96. Recurrent falls were defined as having ≥2 falls in the past 12 months. Haz

Distal femoral periprosthetic fractures (DFPF) are an increasingly prevalent complication following total knee arthroplasty (TKA), representing a significant challenge to the orthopedic community. As the volume of primary and revision TKA continues to rise globally, a clear understanding of the epidemiological landscape and standardized classification are paramount for appropriate treatment, clinical communication, and research. This review provides a focused analysis of the current epidemiological propensities of DFPF, identifying the specific patient populations and temporal trends that defi

Dual-energy CT (DECT) has emerged as a valuable imaging modality in musculoskeletal radiology, offering unique capabilities through virtual non-calcium reconstructions and spectral analysis. This narrative review examines the current evidence and clinical applications of DECT as an alternative to MRI in acute knee trauma evaluation. We discussed the technical principles of DECT, including virtual non-calcium imaging for bone marrow edema detection, metal artifact reduction techniques, and soft tissue assessment capabilities. The review covers six key clinical applications: complex fractures wi

BACKGROUND: Femoral condyle insufficiency fractures following total knee arthroplasty (FCIF-TKA) are rare but significant complications. These fractures, characterized by atraumatic bone insufficiency near the femoral component, present unique challenges in postoperative care, often necessitating femoral component revision.
METHODS: This study retrospectively reviewed 835 primary total knee arthroplasties performed by a single surgeon, identifying six cases of FCIF-TKA. Patient demographics, implant details, radiographic findings, and clinical outcomes were analyzed with up to four years of f

The purpose of this study is to examine patients undergoing primary total knee arthroplasty (TKA) with and without prior history of statin use. We specifically evaluated (1) 90-day to 2-year periprosthetic fractures, (2) revisions, and (3) respective risk factors. We queried a national, all-payer database for patients undergoing primary TKA between 2010 and 2020. Chronic statin exposure was then identified and defined as more than three prescriptions filled within 1 year prior to TKA (statin users). A control cohort of patients undergoing TKA without the prior history of statin use was then cr

Surgical outcomes of subtypes of periprosthetic tibia fractures after total knee arthroplasty were evaluated by using the Felix et al. classification system. Type 3 fractures were the most common classification of periprosthetic tibial fractures. Type 2 fractures had the highest rates of revision and nonunion. Type 3 fractures exhibited longer healing times than types 2 and 4. Far type 3 fractures showed the longest healing time of all fracture types but had very minimal complications. Type 4 fracture managed by K-wire/cerclage wire may require hardware removal or debridement but exhibited the

PURPOSE: Acetabular fractures are frequently associated with other skeletal injuries, particularly knee injuries. This study aims to evaluate the relationship between posterior acetabular fractures and knee injury, analyzing its patterns and association with other injuries.
METHODS: This is a retrospective monocentric study in a tertiary referral centre. We reviewed the data (age, sex, type of fracture, and concomitant injury) of patients hospitalized for a posterior acetabular fracture. We recorded concomitant knee injury diagnosed up to one year following acetabular fracture.

BACKGROUND: The aim of open reduction and internal fixation (ORIF) of fractures around the knee joint is the exact anatomic reconstruction of joint surfaces in order to achieve an early and load stable bone situation. Primary endoprosthetics as the initial treatment can represent an alternative treatment option for a closely selected number of geriatric patients.
OBJECTIVES: The chances and risks of primary endoprosthetics in comparison to ORIF as the gold standard for initial treatment of fractures close to the knee joint in the elderly are presented

OBJECTIVE: Treatment of distal femur fractures by long-stemmed total knee arthroplasty (TKA) is challenging, because of poor bone stock, decreased blood supply, history of multiple knee surgeries and an absence of standard treatment. Few published studies are available concerning this. The purpose of this study was to share surgical technique and better describe our patients' comorbidities, which add to the challenge of managing individuals with these fractures.
METHODS: Between August 2008 and September 2013, seven patients presented to our level I trauma center with distal femoral fractures

BACKGROUND: We evaluated the efficiency of treatment of comminuted intraarticular fractures of the knee, involving direct reduction and rigid fixation for articular components, and indirect reduction and biological fixation for metaphyseal-diaphyseal components.
METHODS: Twelve patients (9 males, 3 females; mean age 46 years; range 22 to 71 years) with distal femoral (n=6) and proximal tibial (n=6) intraarticular fractures were included. There were seven closed and five open fractures. Intraarticular fractures were fixed directly through a lateral parapatellar incision; while comminuted metap

BACKGROUND: Fractures in patients with hip or knee prostheses (periprosthetic fractures, PPF) are rare but clinically significant complications whose absolute number is steadily increasing as implantation rates rise. In Germany, approximately 200 000 hip and 170 000 knee joints are primarily replaced each year: PPFs account for approximately 15% of all revisions of hip surgery and 3% of all revisions of knee surgery.
METHODS: This review is based on publications retrieved by a selective search in the PubMed/MEDLINE and Google Scholar databases up to December 2025. Registry analyses, meta-anal

INTRODUCTION: Floating hip and floating knee are high-velocity injuries associated with high mortality and morbidity. The occurrence of both floating hip and floating knee in the same limb is rare and requires proper resuscitation and orthopedic care for the management and for providing a functional limb to the patient.
CASE REPORT: A 32-year-old male presented with a history of road traffic accident and sustained ipsilateral acetabulum anterior column fracture, sacroiliac joint disruption, double segmental fracture of femur, and segmental fracture of tibia (left side). The patient also susta

Intramedullary nailing is often the treatment of choice for fractures of the tibia, but postoperative knee pain is common after this procedure. Potential etiologies include implant prominence, injury to intra-articular structures, patellar tendon or fat pad injury, damage to the infrapatellar branch of the saphenous nerve, and altered biomechanics. Depending on the etiology, described treatment options include observation, implant removal, assessment and treatment of injured intra-articular structures, and selective denervation. Careful attention to appropriate starting point and implant selec

BACKGROUND: Locked-plate fracture-fixation techniques and designs continue to evolve. Polyaxial locking plates that allow screw angulation and end-point locking have become available; however, there are no clinical data documenting their strength and efficacy, to our knowledge. The purpose of this study was to evaluate the clinical performance of a variable-axis locking plate in a multicenter series of periarticular fractures about the knee.
METHODS: Between 2003 and 2005, fifty-four patients with a total of fifty-six fractures were treated with a polyaxial locked-plate fixation system (DePuy

Management of periprosthetic fractures around the knee is often difficult because of poor bone quality, comminution, and constraints imposed by the existing prosthesis. Locked condylar plates may provide more reliable fixation for these fractures than traditional methods of fixation. Eleven patients with periprosthetic fractures around the knee were treated with open reduction and internal fixation using a locked condylar plate. All 9 acute fractures and 1 of 2 periprosthetic nonunions healed at an average of 21 weeks, and no patient required additional surgery. Nine fractures healed in anatom

Fractures of the distal femur in the geriatric population are associated with a high incidence of postoperative complications and poor results. Nonunion, loss of fixation, and malunion of these fractures occur with all types of treatment. The postoperative treatment of these patients demands a lengthy period of limited weightbearing that can increase the rate of medical complications. Our experience with these challenging fractures caused us to consider the use of a primary distal femur replacement total knee arthroplasty with the goals of elimination of fracture healing issues, early mobiliza

BACKGROUND: Meniscal injury and/or extrusion have been indicated as causes of subchondral insufficiency fracture (SIFK). However, mediolateral knee laxity has not been discussed as a risk factor. This is a case report of SIFK potentially caused by mediolateral laxity in the absence of a meniscal disorder.
CASE PRESENTATION: A 59-year-old male patient presented with SIFK in the medial femoral condyle without meniscal injury or extrusion. Tibial condylar valgus osteotomy (TCVO) was performed, and the preoperative total mediolateral laxity of 8° in extension and 6° in 80° flexion decreased to 6°

BACKGROUND: Ipsilateral femur and tibia fractures around the knee (floating knee) are rare injuries that threaten both limb viability and patient life. A correct surgical strategy is essential to reduce complications and sequelae. The aim of this study was to evaluate characteristics and results of treatment in patients with a floating knee treated at a single trauma center.
MATERIAL AND METHODS: This is a retrospective and non-consecutive case series of 18 floating knees occurred in 17 patients. All patients were operated in a single third-level public and university hospital from December 2
