Arthroscopically assisted treatment of adolescent knee joint tuberculosis.

OBJECTIVE: To evaluate the role of knee arthroscopy in diagnosis and treatment of knee joint synovitis and total joint tuberculosis (TB) of adolescence. METHODS: Forty-one patients with knee TB, aged 7-16 years, were treated using knee arthroscopy from June 2002 to December 2006. Clinical data were reviewed at followed up and evaluated using the Lysholm score. RESULTS: Among the 41 patients, 36 were diagnosed as having TB on the basis of preoperative clinical manifestations and laboratory results, and the remaining 5 as synovitis preoperatively, TB being confirmed postoperatively by arthrosc

Effects of age on neuromuscular knee joint control.

Age-related changes in neuromuscular function may have adverse effects on hamstring reflex activity and may impair knee joint stability. The aim of this study was to investigate whether increasing age affects hamstring short latency responses (SLR), medium latency responses (MLR), and anterior tibial translation. In 40 healthy subjects ranging from 20 to 70 years of age we assessed hamstring muscle latencies and integrals for both hamstring reflex components (SLRs and MLRs) as well as anterior tibial translation in order to quantify knee joint stability in response to induced tibial translatio

Joint distraction and reconstruction in complex knee contractures.

We retrospectively reviewed the results of joint distraction and reconstruction in complex knee contractures in 6 patients with 9 knees. The circular Ilizarov fixator was used in all cases. Extensor mechanism alignment with frame in situ was performed in 4 cases and alignment of mechanical axis of the lower limb was done in 2. All patients had a mean total arc of movement of 79 degrees before operation; however, this was nonfunctional because of the degree of flexion contracture. Three of the 6 patients were nonambulant before surgery. After joint distraction and reconstruction, mean total arc

Multicompartmental pressures in the knee joint during arthroscopy.

It is well known that fast changes of position of the knee joint cause high instantaneous intraarticular pressures. During arthroscopy, rotation and flexion of the leg can create a closed-off compartment, increasing the risk of capsular rupture. To evaluate the risk of compartmentation, a study has been conducted in which the intraarticular pressure in 12 knee joints of three women and nine men was measured with Myopress catheters (ATOS Medical AB, Hörby, Sweden) located in the suprapatellar pouch, and the anteromedial and the posteromedial recesses during arthroscopy. This study showed that a

[Tendon injuries of the knee joint extensor mechanism].

Diagnosis, aetiology and therapy of injuries to the extensor mechanism of the knee joint are presented. 22 cases of ruptures and discisions of the lig. patellae or the quadriceps tendon could be reexamined. All cases but one, with operation within the first week after injury, suturing with absorbable or non absorbable material, immobilisation for 6 weeks and following physical therapy, had a good functional result. No rerupture occurred. In one case a limitation of the range of movement had to be seen afer an open discision of the lig. patellae. After transposition of the tuberositas tibiae, d

Exercise-induced knee joint laxity in distance runners.

The objectives of this study were to evaluate the effect of exercise on knee joint laxity. If exercise induced laxity is physiological, incorporation of this quality into a ligament replacement material would be indicated. Twenty recreational long distance runners average age 41 (range 24 to 50 yr) were tested before and immediately after 30 minutes of running. Using a computerized goniometer type instrument (Acufex KSS), knee flexion, axial tibial rotation and anterior-posterior tibial displacement were simultaneously recorded, while the runners underwent tests of static as well as dynamic kn

Knee-joint rupture with giant antefemoral cyst formation.

A case of a patient with Reiter's disease, whose ruptured knee-joint effusion extended proximally along the femur to just below the inguinal ligament, and simulated an iliofemoral thrombosis, is reported. The mechanisms of joint rupture, and the need to differentiate it from thrombophlebitis, are discussed.

[Posttraumatic recurrent luxation of knee joint (author's transl)].

A posttraumatic, recurrent luxation of the knee joint is presented. The cause for the first luxation was a severe trauma. As the treatment of the ligament and capsule ruptures of the first luxation was inadequate, the effect was a completely unstable knee joint. By hyperextension trauma or hyperextension following weakness of the muscles of the thigh recurrent luxations of the knee were caused. The importance of a sufficient treatment of ligament and capsule damage following a knee luxation is emphasized.

[Treatment of knee joint deformities in spina bifida].

182 cases of surgical treatment of knee deformities in children with spina bifida between 1968 and 1977 are reviewed. The most number of cases had very good or good success. 138 supracondylar osteotomies produced very good results in 18 cases; reosteotomy was necessary in 10 cases. EGGERs procedure in 26 cases made very good results and good results in 6 cases. Quadriceps-Z-plasty with anterior capsulotomy produced very good results in 11 cases, only one bad result here was produced by skin-breakdown.

Intra-articular rheumatoid nodule of the knee joint.

A 44-year-old woman with a long-standing history of classical rheumatoid arthritis had an intra-articular rheumatoid nodule of the knee joint. The history, symptoms, and signs of this case were, however, almost identical to those cases previously reported. This entity has a classic presentation and distinct findings on examination which distinguish it from other intra-articular lesions of the knee.

An Abbreviated Rabbit Knee Model of Joint Contracture.

Experimental analyses of knee joint contractures have traditionally utilized a 6-month rabbit model as the gold standard. However, this model is time-intensive and costly. The purpose of this study was to develop an abbreviated rabbit model of knee contractures and compare it to the well-established longer model. Twenty female New Zealand White rabbits were divided into two equal groups and prospectively studied to assess knee passive extension angles (PEA), contracture angles (CA), and terminal posterior capsular stiffness. Experimental knees were immobilized for either 4 weeks (n = 10) with

Prosthetic Knee Joint Infection Caused by Mycobacterium kansasii.

Mycobacterium kansasii is a nontuberculous mycobacterium that is a rare cause of prosthetic joint infections (PJIs). This case report presents a 58-year-old man who developed rapidly progressive arthritis after exposing his right knee to an unknown fluid at a microbial pharmaceutical company. Within a year, he underwent a right total knee arthroplasty (TKA). At 5 months postoperatively, he presented with pain and swelling of that knee. Imaging revealed extensive periprosthetic osteolysis with diffuse intracapsular and posterior extracapsular fluid collections. Multiple knee aspirates had negat

Understanding the patellofemoral joint in total knee arthroplasty.

Total knee arthroplasty (TKA) is one of the most successful procedures in orthopedic surgery. Nevertheless, postoperative patellofemoral complications remain a challenging problem, affecting a substantial proportion of patients. Complications involving the patellofemoral joint (PFJ) can occur in both resurfaced and nonresurfaced patellae. Types of PFJ complications include anterior knee pain, maltracking, fracture, avascular necrosis and patellar clunk. The causes of patellofemoral complications can be categorized into patient-, surgeon- and implant-related factors. Patient characteristics suc

Lyme periprosthetic joint infection in total knee arthroplasty.

Lyme arthritis, caused by the spirochete Borrelia burgdorferi sensu stricto, is a common tick-borne illness in New England and the upper Midwest. Most often, the disease affects the knee and has typically been reported as a cause of native joint infection. There has been only 1 case of Lyme periprosthetic joint infection (associated with a total knee arthroplasty) reported in the literature, and to our knowledge, no other reported cases of Lyme periprosthetic joint infections exist. In this article, we report on 2 patients diagnosed with prosthetic joint infections who were subsequently found

Biomechanical Implications of an Oblique Knee Joint Line.

Surgical correction of multiapical deformities of the lower limb requires careful preoperative planning. Surgeons must account for the potential creation of secondary deformity, such as knee joint line obliquity, and the risks associated with accepting these changes in limb alignment. In this study, we evaluate the effect of knee joint obliquity on tibial plateau contact pressures and knee instability. Three cadaveric knees were dissected and put through biomechanical testing to simulate loading of an oblique knee joint. We observed < 1 mm femoral displacement (proxy measure of instability) be
