Hypersexuality in two patients with epilepsy treated with lamotrigine.

PURPOSE: Lamotrigine (LTG) is a novel anticonvulsant drug that exerts an antiepileptic effect by decreasing glutamate release through inhibition of voltage-sensitive sodium channels. LTG has no effect on serum levels of most female reproductive hormones, but its effect on male reproductive hormones still remains unclear. Improvement in sexual function after LTG treatment has been reported, and could have been caused by reduction of seizures, inhibition of focal discharges, or an unknown effect of LTG on reproductive hormones and protein levels. CASES: Two male patients exhibited acute hyperse

Use of lamotrigine in glossopharyngeal neuralgia: a case report.

We describe a case of a patient with glossopharyngeal neuralgia (refractory to treatment with carbamazepine, amytriptyline, diazepam, and indomethacin) treated with lamotrigine as monotherapy, the first described, who responded completely to the therapy and did not complain of side effects. The complete analgesic effect was reached at the lamotrigine daily dose of 200 mg per day and was maintained at that dose for additional 6 months, with the blood concentration within the reference range.

Lack of pharmacokinetic interaction between oxcarbazepine and lamotrigine.

Epilepsy and bipolar disorder are commonly treated by combination drug therapy, such as lamotrigine and oxcarbazepine. To ensure the safety of this combination, information on pharmacokinetics and tolerability must be available. The objective of study was to evaluate the pharmacokinetics and tolerability of coadministered lamotrigine and oxcarbazepine in healthy subjects. This randomized, single-blind, parallel-group study comprised three cohorts: lamotrigine (200 mg daily) plus oxcarbazepine (600 mg twice daily), lamotrigine (200 mg daily) plus placebo, and oxcarbazepine (600 mg twice daily)

Long-term treatment of bipolar disorder with lamotrigine.

Bipolar depression is as debilitating as mania in bipolar disorder, but the treatment of bipolar depression has historically received less attention. To date, there is no mood stabilizer (liberally defined as a medication that decreases episode severity, duration, or frequency in one phase of bipolar illness without producing a negative effect in other phases) that demonstrates similar efficacy in both the depressive and the manic phases of bipolar disorder. However, bipolar depression--which is prevalent, sometimes chronic, and associated with a low quality of life and a high risk of suicide-

Lamotrigine use in geriatric patients with bipolar depression.

OBJECTIVE: To study the effectiveness of adding lamotrigine to the treatment of inpatient geriatric patients with bipolar disorder (BD) who were in the depressed phase and had been on lithium and valproate for at least 3 months. METHOD: Lamotrigine was started at 25 mg given at bedtime, with weekly incremental increases of 12.5 mg daily until a total dosage of either 75 mg or 100 mg was obtained. Improvement was measured by clinical interview and Hamilton Depression Rating Scale (HDRS) scores. Patients were reassessed at 6 weeks, and if their HDRS score had decreased by at least 50%, they wer

Lamotrigine in the management of neuropathic pain: a review of the literature.

OBJECTIVE: The purpose of this review is to examine the accumulating evidence indicating that lamotrigine is effective in the treatment of neuropathic pain. METHOD: A review of the available literature. RESULTS: Neuropathic pain is a debilitating series of conditions that are often poorly controlled. The molecular action of lamotrigine in terms of its effects in preclinical models of pain and hyperalgesia are considered along with the accumulating evidence suggesting that lamotrigine may be effective in the clinical management of neuropathic pain. CONCLUSION: A review of the literature sugg

Hyponatraemia associated with lamotrigine in cranial diabetes insipidus.

We report the cases of two children with cranial diabetes insipidus who were treated with lamotrigine for seizures and who had accompanying changes in desmopressin requirements. Lamotrigine is a new anticonvulsant chemically unrelated to other existing antiepileptic drugs. Studies suggest it acts at voltage-sensitive sodium channels and also decreases calcium conductance. Both of these mechanisms of action are shared by carbamazepine, which can cause hyponatraemia secondary to inappropriate secretion of antidiuretic hormone. It is possible that the effect of lamotrigine on fluid balance in the

Lamotrigine and gabapentin. Alternative in the treatment of bipolar disorder.

The anticonvulsants carbamazepine and sodium valproate have efficacy in hypomania, bipolar depression and in the prophylaxis of bipolar disorder. This paper reviews published evidence on the effects of the new anticonvulsants gabapentin and lamotrigine in bipolar disorder. The conclusion is that there is good evidence from open studies to indicate that randomised double-blind trials of these compounds should be carried out. The pharmacology of these anticonvulsants is reviewed and putative predictors of good response identified.

Effective removal of lamotrigine using colloidal activated carbon.

While granular activated carbon (GAC) exhibits strong adsorption performance for the removal of pharmaceuticals in ex-situ-controlled environments, its efficiency in the in-situ groundwater remediation is limited. Therefore, this study investigates the efficiency of using colloidal activated carbon (CAC) for the removal of lamotrigine in the lab. The adsorption performance of the CAC for lamotrigine under different operational parameters was evaluated through a series of fixed-bed columns packed with sand and CAC, mimicking a subsurface environment. Breakthrough curves were plotted, and the ef

Clinical effects of acute lamotrigine overdose (ATOM-10).

INTRODUCTION: Lamotrigine overdose is not typically associated with severe toxicity. However, both severe toxicity and serotonin toxicity is occasionally reported following large ingestions. We aimed to investigate the clinical effects of lamotrigine overdose. METHODS: This was a prospective observational study from July 2020-March 2024. Patients >14 years-old with acute lamotrigine overdose (≥2 g ingestion) were recruited from the Australian Toxicology Monitoring study or identified from three toxicology units. Data extracted included clinical features, lamotrigine concentrations, management

Massive Lamotrigine Intoxication Refractory to Sodium Bicarbonate Treatment.

BACKGROUND: Acute lamotrigine (LTG) overdose can cause refractory cardiac arrhythmias, hypotension, altered mental status, and seizures. Here, we present the case of a patient admitted for massive LTG intoxication refractory to sodium bicarbonate treatment, which improved with lipid rescue therapy. METHODS AND RESULTS: Serum concentrations of therapeutic drugs were measured using liquid chromatography-tandem mass spectrometry. The results showed the LTG level of 63 mg/L. The other quantified drugs were propranolol (441 ng/mL), methylphenidate (0.7 ng/mL), and ritalinic acid (140 ng/mL). The p

Lamotrigine induced lymphadenopathy: Case report and literature review.

Lamotrigine is an anti-epileptic drug often prescribed to children and young females. Side effects include rash, dizziness and diplopia. Over the last twenty years, two cases of lymphadenopathy due to lamotrigine have been reported. We will present the case of a 17-year old female with persistent lymphadenopathy due to lamotrigine. The purpose of this case report is to inform clinicians that lymphadenopathy is a possible side effect of lamotrigine and to highlight the need for further research.

Lamotrigine effects sensorimotor gating in WAG/Rij rats.

INTRODUCTION: Prepulse inhibition (PPI) is a measurable form of sensorimotor gating. Disruption of PPI reflects the impairment in the neural filtering process of mental functions that are related to the transformation of an external stimuli to a response. Impairment of PPI is reported in neuropsychiatric illnesses such as schizophrenia, Huntington's disease, Parkinson's diseases, Tourette syndrome, obsessive compulsive disorder, and temporal lobe epilepsy with psychosis. Absence epilepsy is the most common type of primary generalized epilepsy. Lamotrigine is an antiepileptic drug that is prefe

Ethinyl estradiol, not progestogens, reduces lamotrigine serum concentrations.

PURPOSE: To study the interaction between lamotrigine (LTG) and hormonal contraception. METHODS: LTG serum concentrations of female patients using either no hormonal contraception (n=18), an ethinyl estradiol (EE)-containing (n=11), or a progestogen (PG)-only-containing compound (n=16) were analyzed. Patients were recruited prospectively, and blood samples were drawn during drug fasting and at steady-state conditions. Comedication with enzyme inducers, valproate, topiramate, or sertraline was not allowed. Some patients changed groups and thus served as their own controls. Samples were analyze

Fatal toxic epidermal necrolysis related to lamotrigine administration.

Toxic epidermal necrolysis (TEN) and Stevens-Johnson syndrome (SJS) are severe life-threatening dermatologic conditions. To date, eight cases of TEN and one of SJS related to lamotrigine administration have been reported in the literature. Most patients were also taking concomitant valproic acid. It was hypothesized that valproic acid may interfere with glucuronidation of lamotrigine, leading to increased serum lamotrigine levels, or perhaps alter the drug's metabolism, resulting in accumulation of a toxic intermediate metabolite. Ultimately, this may possibly predispose a patient to increased

Time-Course Changes in Lamotrigine Concentration after Addition of Valproate and the Safety and Long-Term Tolerability of Lamotrigine-Valproate Combination Therapy.

The aim of this study was to evaluate the time-course changes in lamotrigine (LTG) concentration after addition of valproate (VPA) and the safety and tolerability of the combination therapy. We reviewed our therapeutic drug monitoring (TDM) database and found 345 patients on LTG who received add-on therapy with VPA. VPA had been added at least 12 weeks after patients finished stepwise LTG titration. Also, we retrospectively evaluated the LTG concentration after addition of VPA and the safety and long-term tolerability of LTG-VPA combination therapy. Plasma LTG concentration increased more than

Prediction of an Optimal Dose of Lamotrigine for Augmentation Therapy in Treatment-Resistant Depressive Disorder From Plasma Lamotrigine Concentration at Week 2.

BACKGROUND: The authors have previously shown that an early therapeutic response to lamotrigine augmentation therapy is dependent on its plasma concentration and that a plasma lamotrigine concentration of 12.7 μmol/L may be a threshold for a good therapeutic response in treatment-resistant depressive disorder. The present study investigated whether or not an optimal dose of lamotrigine could be predicted from plasma lamotrigine concentration at week 2. METHODS: The subjects were 37 depressed patients who had already shown insufficient response to at least 3 psychotropics including antidepress

Multicentre, double-blind, randomised comparison between lamotrigine and carbamazepine in elderly patients with newly diagnosed epilepsy. The UK Lamotrigine Elderly Study Group.

In a multicentre, double-blind trial 150 elderly patients (mean age 77 years) with newly diagnosed epilepsy were randomised in a 2:1 ratio to treatment with lamotrigine (LTG) or carbamazepine (CBZ). Following a short titration period, the dosage was individualised for each patient while maintaining the blind over the next 24 weeks. The main difference between the groups was the rate of drop-out due to adverse events (LTG 18% versus CBZ 42%). This was in part a consequence of the lower rash rate with LTG (LTG 3%, CBZ 19%; 95% CI 7-25%). LTG-treated patients also complained less frequently of so
