Discordance Between Textual Reasoning and Visual Interpretation in Large Language Models for Low Back Pain: Cross-Sectional Quantitative Evaluation and Exploratory Multimodal Stress Test.

BACKGROUND: Large language models (LLMs) are rapidly evolving from text-based agents to multimodal systems capable of interpreting medical images. While their textual reasoning has improved, the safety implications of this shift remain underexplored, specifically regarding the alignment between visual interpretation and textual advice in low back pain (LBP) management. OBJECTIVE: This study aims to evaluate the performance of 10 commonly used and highly representative LLMs, including paid models such as ChatGPT 5 Plus (OpenAI) and reasoning-oriented models like Qwen3-Thinking (Alibaba/Qwen te

Leveraging Natural Language Processing to Identify Veterans Who Inject Drugs to Assess Preexposure Prophylaxis and Sexually Transmitted Infection Testing Services at the Veterans Health Administration.

BACKGROUND: People who inject drugs (PWID) face disproportionate risks for infectious diseases yet remain difficult to identify within health care systems. Natural language processing (NLP) offers potential solutions for identifying PWID to improve access to harm reduction services. METHODS: We evaluated an NLP dashboard designed to identify Veterans with evidence of injection drug use across 6 Veterans Health Administration facilities between August and October 2024. Four independent reviewers assessed electronic health records to confirm recent injection drug use and evaluated preventive ca

Developing and testing a framework for coding general practitioners' free-text diagnoses in electronic medical records - a reliability study for generating training data in natural language processing.

BACKGROUND: Diagnoses entered by general practitioners into electronic medical records have great potential for research and practice, but unfortunately, diagnoses are often in uncoded format, making them of little use. Natural language processing (NLP) could assist in coding free-text diagnoses, but NLP models require local training data to unlock their potential. The aim of this study was to develop a framework of research-relevant diagnostic codes, to test the framework using free-text diagnoses from a Swiss primary care database and to generate training data for NLP modelling. METHODS: Th

The use of the Vineland Social Maturity Scale, the Merrill-Palmer Scale of mental tests (non-verbal items) and the Reynell Developmental Language Scales with children in contact with the services for severe mental retardation.

Psychological tests were administered to a complete population of severely retarded children aged 0-14, from one area of south-east London. The fifty-six children selected for the present study included all those who had obtained scores on measures of social maturity, visuo-spatial skills not involving symbolic concepts, and level of language comprehension. The results show very low correlations between the age-related quotients obtained for each measure. This suggests that, in severely retarded children, marked discrepancies can occur between different areas of cognitive and social developmen

Two functional magnetic resonance imaging f(MRI) tasks that may replace the gold standard, Wada testing, for language lateralization while giving additional localization information.

Two tasks were used to lateralize and localize language functions noninvasively, using functional magnetic resonance (fMRI BOLD sequences). fMRI images produced during comprehension of the gist of a tale derived from an ordered series of inferential questions were used to lateralize and locate the center of and margins within language dominant hemisphere near posterior temporal-parietal-occipital (TPO) cortical area(s), e.g., Wernicke's. A silent noun-generating task was used to lateralize and localize naming functions within and along superior temporal gyrus (STG) and/or the basal temporal la

Evidence Use and Identifier-Conditioned Prior Knowledge in Large Language Model Classification of Oncology Trials Assessed Through Progressive Content Removal and Counterfactual Testing: Comparative Analysis.

BACKGROUND: Large language models (LLMs) can accurately classify biomedical documents, but strong benchmark performance does not establish that predictions are grounded in the supplied text. In biomedical literature tasks, titles, abstracts, digital object identifiers (DOIs), journal metadata, and trial identifiers may have been seen during pretraining and can trigger parametric knowledge or learned associations. OBJECTIVE: This study aimed to test whether oncology randomized trial success classification is driven by abstract evidence or by identifier-conditioned prior knowledge, and assess w

Assessing Stigma Towards Substance Use in Pregnancy: A Randomized Study Testing the Impact of Stigmatizing Language and Type of Opioid Use on Attitudes Toward Mothers With Opioid Use Disorder.

OBJECTIVE: To examine the extent to which colloquial phrases used to describe opioid-exposed mother-infant dyads affects attitudes toward mothers with opioid use disorder (OUD) to assess the role stigmatizing language may have on the care of mothers with OUD. METHODS: We employed a randomized, cross-sectional, case vignette of an opioid-exposed dyad, varying on 2 factors: (1) language to describe newborn ("substance-exposed newborn" vs "addicted baby") and (2) type of maternal opioid use (injection heroin vs nonmedical use of prescription opioids). Participants were recruited using an online

Improving language mapping in clinical fMRI through assessment of grammar.

INTRODUCTION: Brain surgery in the language dominant hemisphere remains challenging due to unintended post-surgical language deficits, despite using pre-surgical functional magnetic resonance (fMRI) and intraoperative cortical stimulation. Moreover, patients are often recommended not to undergo surgery if the accompanying risk to language appears to be too high. While standard fMRI language mapping protocols may have relatively good predictive value at the group level, they remain sub-optimal on an individual level. The standard tests used typically assess lexico-semantic aspects of language,

Language Competence in Parkinson's Disease: A Systematic Review of 20 Years of Research.

PURPOSE: Parkinson's disease (PD) is the second most common neurodegenerative condition worldwide. Although diagnostic criteria heavily rely on the motor component, nonmotor symptoms of PD, including language deficits, are now recognized to emerge several years before its motor manifestations. Assessing the role of language impairment as a nonmotor marker of PD is thus an opportunity to contribute to an effective and timely diagnosis. METHOD: In this systematic review, we synthesize the state of knowledge on language competence in PD and provide a systematic and comprehensive overview of the

Discrimination of normal and aphasic subjects on a test of syntactic comprehension.

An Anglophone population consisting of 37 aphasic patients and 23 normal control subjects, and a Francophone population consisting of 49 aphasic patients and 23 control subjects were given a task requiring the comprehension of syntactic structures for the correct assignment of thematic roles to nouns. Discriminant analysis was used to classify subjects into aphasic and normal groups according to their scores on the task. In both populations--Anglophone and Francophone--most of the subjects were classified into their actual groups except for an occasional normal subject classified with the apha

Spontaneous productions of infinitive clauses by English-speaking children with and without specific language impairment.

As a group, children with specific language impairment (SLI) have presented with lower levels of proficiency with infinitive clauses relative to comparison groups with typical language (TL). The presence of considerable individual variability within those affected by SLI, however, remains unexplained. Furthermore, the status of infinitive clause productions in children with language impairments that do not meet criteria for SLI, because of either low nonverbal abilities or other concomitant conditions, is unknown. Previous studies focused on children with SLI and have not included children who
