A Hybrid Rule-Based and Machine Learning-Based Clinical Decision Support System to Support Prescription Review: Development and External Validation Study.

BACKGROUND: Anticoagulants are high-alert medications with substantial risk of serious bleeding, yet dosing and prescribing errors remain common. Although clinical decision support systems (CDSSs) can mitigate these errors, their impact is constrained by alert fatigue and limited interpretability. OBJECTIVE: We developed and validated a hybrid CDSS that integrates rule-based logic with machine learning to improve the safe use of anticoagulants. METHODS: This multicenter study used electronic health record data on anticoagulant prescriptions from 3 tertiary hospitals (1 for system development

Transformative Learning Through Implementation: A Realist Case Study of Frostbite Care Protocol Implementation.

BACKGROUND: Learning Health Systems (LHS) emphasize continuous improvement through cycles of evidence generation, implementation, and evaluation, yet relatively little attention has been paid to the educational processes through which clinicians and organizations learn during implementation. This study examined how implementation of a frostbite treatment protocol functioned as a process of transformative learning within a LHS. METHODS: We conducted a realist case study of implementation of a frostbite treatment protocol across four tertiary-care hospitals within a Canadian health system. Semi

[The impact of education on chronic wound care improvement].

Although not fully understood, close relationship between health and education ensures unambiguous health and quality of life advantages to educated individuals. Education ensures different thinking and decision making processes and man is enabled to receive information from the external world. Even though the process of education and learning still relies on banking principles and coping of common knowledge, modern and technological society drives the system as well as education opportunities towards the new learning sources. In the developed world, the impact of chronic wounds on health syst

Scaling up tobacco cessation within TB programmes: findings from a multi-country, mixed-methods implementation study.

BACKGROUND: Brief behavioural support can effectively help tuberculosis (TB) patients quit smoking and improve their outcomes. In collaboration with TB programmes in Bangladesh, Nepal and Pakistan, we evaluated the implementation and scale-up of cessation support using four strategies: (1) brief tobacco cessation intervention, (2) integration of tobacco cessation within routine training, (3) inclusion of tobacco indicators in routine records and (4) embedding research within TB programmes. METHODS: We used mixed methods of observation, interviews, questionnaires and routine data. We aimed to

Cross-Pollinators and Boundary Spanners: Contributions of Ontario Health Team Fellows to Quality Improvement and Evaluation Capacity for Health Systems.

Despite knowledge mobilization efforts between academic researchers and health system decision-makers, the evidence-to-action gap persists. Given that the learning health system is a cycle of knowledge to action to knowledge, the implementation of embedded health service researchers within host health system organizations is a promising approach to bridge the evidence-to-action gap. The contributions of embedded researchers to quality improvement (QI) and evaluation research capacity for integrated health systems are promising areas that warrant investigation. This paper describes the roles an

When formal governance persists but resilience does not: health system governance and resilience in Yemen across a decade of conflict and crisis (2014-2025).

BACKGROUND: Yemen's health system has faced prolonged shocks, including war, epidemics, and COVID-19, challenging both governance and resilience. This study examines how formal governance structures and institutional capacities shaped the system's ability to anticipate, absorb, adapt, learn, and transform across a decade of crisis (2014-2025). METHODS: We conducted a longitudinal qualitative analysis based on interviews with health system leaders and technical actors. Using a resilience-capacity framework, we analyzed governance functions across four phases: pre-conflict period (pre-2014), co

Machine learning for an expert system to predict preterm birth risk.

OBJECTIVE: Develop a prototype expert system for preterm birth risk assessment of pregnant women. Normal gestation involves a term of 40 weeks, but because 8-12% of the newborns in the United States are delivered prior to 37 weeks' gestation, problems associated with prematurity continue to plague individuals, families, and the health care system. DESIGN: A knowledge-base development methodology used machine learning, statistical analysis, and validation techniques to analyze three large datasets (18,890 subjects and 214 variables). The dependent (i.e., decision) variable studied was weeks of

Temporal Integrative Machine Learning for Early Detection of Diabetic Retinopathy Using Fundus Imaging and Electronic Health Records.

Diabetic Retinopathy (DR), a prevalent diabetes complication leading to blindness, often goes undetected until late stages due to patients seeking help only when symptoms manifest and limited experts' availability. To address these challenges, we present a novel temporal integrative machine learning system that harnesses both fundus images and electronic health records (EHR) for early and enhanced DR detection. Our system uniquely processes EHR data by focusing on temporal trends and long-term patient histories, creating thousands of temporal features that capture their evolving dynamics over

The role of psychological assessment and family support in enhancing emotional well-being and academic performance of children with learning disabilities.

Children with learning disabilities often face significant emotional and academic challenges due to inadequate assessment and support systems. This study examines the impact of psychological assessment on anxiety, emotional well-being, and academic performance, with the moderating influence of intervention strategies, physical health, and family support. In this study, the term 'psychological assessment of children with learning disabilities' refers to scores obtained from validated standardized scales capturing students' academic difficulties, rather than clinical diagnosis of learning disabi

Developing a Program Evaluation Training Within a National Federal Healthcare Setting: Veterans Affairs Evaluation Bootcamp Training.

INTRODUCTION: Learning Health Systems (LHSs) depend on the ability to effectively evaluate programs and translate data into practice. Within the Veterans Affairs (VA) health system, formal mandates require programmatic decision-making to be data-driven; however, opportunities for staff to develop applied program evaluation skills that support LHS learning cycles remain limited. METHODS: To address this need, the synchronous virtual Evaluation Bootcamp Training (EBcT) was developed to equip learners with practical, applied evaluation skills. This manuscript describes the design rationale, stru

Adapting public health response through lessons learnt: Nigeria's experience from Lassa fever and COVID-19.

Response to public health emergencies requires continued adaptation and innovation. The Nigeria Centre for Disease Control (NCDC) is the country's public health institute with the mandate to protect the health of Nigerians. Achieving such mandate in resource-limited settings with divergent demographic characteristics of the citizens, necessitates the readiness to learn from experience and to develop policies and activities in line with lessons learnt and best practices. This practice paper describes the initiatives of the NCDC towards adapting its public health response activities by establish

Early exposure to a primary care course: a co-created transformative approach in health systems science.

As health systems science has become the third pillar of medical education (along with basic and clinical sciences), Thailand has been developing medical curricula to focus more on its health systems. Despite primary care being the driving force of Thailand's health systems, their integration into the curricula remains a challenge, resulting in medical students having limited exposure and understanding. To address this, a course was co-created by stakeholders to provide students with early exposure to and understanding of primary care mechanisms. Forty-one first- and second-year medical studen

Implementation of a Mental Health Nursing Practicum in an Urban Homeless Shelter.

BACKGROUND: People experiencing homelessness suffer from deficient access to health care and disproportionately poor health outcomes. The American Association of Colleges of Nursing (AACN) maintains learning competencies for prelicensure nursing students. Shelters are rich environments for students to garner experiences with the inequities plaguing our health care system and to fulfill AACN competencies. METHOD: We established a psychiatric and mental health nursing practicum at a homeless shelter. Following a retrospective pretest methodology, we evaluated student learning with the Health Ca

If we keep doing what we're doing we'll keep getting what we're getting: A need to rethink "academic" medicine.

PURPOSE: For generations there have been warnings of the need to reform medical education at all levels. Today the voices pushing reform are louder, the need is greater, and there is an urgency not seen before. Approaches that have worked in the past to train physicians are no longer as relevant today as demographics, disease patterns, human resources, practice behaviors, technology, and attention to costs demand new collaborative approaches to clinical practice. To prepare for this practice tomorrow's doctors will need a different type of educational model, a different type of learning, in di
