Investigation of morbidity, length of stay, and healthcare costs of inpatient paediatric burns.

Burn injuries are the third most common cause of death in children due to trauma. Hospitalizations related to burn injuries are common. Prolonged hospitalization associated with burn treatment can result in increased resource utilization, leading to higher costs. Thus, it is essential to investigate these areas to reduce costs. The study investigated the morbidity and length of hospital stay of paediatric burn patients, as well as calculated the social security costs of hospitalization and treatment. The retrospective observational descriptive study examined the medical records of 774 paediatr

Length of Stay of Emergency Department Patients with Stimulant Intoxication Receiving Intravenous Fluid.

INTRODUCTION: Intravenous (IV) fluids are routinely administered empirically in the emergency department (ED) for patients presenting with stimulant intoxication (eg, cocaine, methamphetamine, synthetic marijuana), although the literature is sparse regarding the benefits and risks of this practice. Our primary objective in this study was to assess whether empiric administration of IV fluids in the ED is associated with increased discharge length of stay (LOS) among ED patients presenting for stimulant intoxication who were subsequently discharged. METHODS: This single-center, retrospective co

Pancreatic surgery and length of hospital stay: meta-analysis of randomized clinical trials.

BACKGROUND: Length of hospital stay (LOS) is a commonly reported postoperative outcome measure in pancreatic surgery. However, LOS may be influenced by various factors, including healthcare systems, cultural traditions, and readmission rate. This systematic review and meta-analysis investigated LOS in pancreatic surgery. METHODS: A meta-analysis was conducted to identify all randomized clinical trials (RCTs) of pancreatic surgery reporting LOS. Analyses were conducted using a random-effects model with mean differences as the effect estimator for the continuous outcome LOS. RESULTS: In all, 1

The Effect of Insurance Type on Length of Stay Following Total Knee Arthroplasty.

BACKGROUND: With the expansion of the Affordable Care Act in 2014, there has been a growing interest in how the Medicaid population will affect postoperative outcomes following total knee arthroplasty (TKA). Studies have shown that lower socioeconomic status, non-Caucasian race, women, cardiac and renal disorders, and younger age have been associated with increased lengths of stay (LOS) after TKA. The primary purpose of our study was to compare the total complications and LOS among patients undergoing TKA who have cash, commercial, government, Medicaid, and Medicare insurances. METHODS: We qu

Preoperative Hemodynamics Impact the Benefit of Fenestration on Fontan Postoperative Length of Stay.

BACKGROUND: Utilization of Fontan fenestration varies considerably by center. OBJECTIVES: Using a multicenter Pediatric Heart Network dataset linking surgical and preoperative hemodynamic variables, the authors evaluated factors associated with use of Fontan fenestration and the impact of fenestration on post-Fontan length of stay (LOS). METHODS: Patients 2 to 6 years old at Fontan surgery from 2010 to 2020 with catheterization<1 year prior were included. Factors associated with fenestration were evaluated using multivariable logistic regression adjusting for key covariates. Restrictive cubi

Trends and Predictors of Short Length of Stay Following Transcatheter Aortic Valve Replacement.

BACKGROUND: Minimalist approaches to Transcatheter aortic valve replacement (TAVR) have allowed for improved efficiency in care of patients. We hypothesized that improved efficiencies in care process may have led to increased adoption of a one night length of stay (LOS) in this patient group. OBJECTIVES: The authors aimed to study temporal trends in short length of stay following TAVR. METHODS: This is a nationwide temporal trends study using the 2016-2019 National In Patient Sample (NIS) registry. Short stay was defined as LOS of one night or less. Trends in proportion of patients with shor

The Influence of Psychosocial Factors on Hospital Length of Stay Among Aging Canadians.

Background: Hospital stays that are prolonged due to non-clinical factors are costly to health care systems and are likely suboptimal for patient well-being. We assessed the influence of psychosocial factors on hospital length of stay (LOS) for older Canadians in a retrospective cohort study. Data and Methods: Data from the Canadian Community Health Survey were linked with the Discharge Abstract Database. Analyses were stratified by age, 55-64 (n = 1,060) and 65 and older (n = 2,718). Main predictor variables of interest included four measures of social support, sense of belonging, and living

Predictors of Extended Postoperative Length of Stay After Uncomplicated Transcatheter Aortic Valve Replacement.

BACKGROUND: The current study aims to identify predictors of extended postoperative length of stay (PLOS) after uncomplicated transcatheter aortic valve replacement (TAVR). METHODS: Patients who underwent TAVR at a single center between June 2012 and June 2016 were analyzed. Patients were stratified by time into an early cohort (EC; 2012-2014) and current cohort (CC; 2015-2016). Those who had complications post procedure were excluded. The CC group was dichotomized based on its median PLOS. Factors associated with a longer PLOS were investigated by using multivariable logistic regression anal

Impact of early mobilization on length of stay after primary total knee arthroplasty.

BACKGROUND: Decreasing length of stay (LOS) following primary total knee arthroplasty (TKA) has been an important focus by all the stakeholders involved in the delivery of care. LOS is dictated by both the patient and hospital-related factors. The purpose of this study was to determine if early mobilization on post-operative day 0 (POD 0) following primary TKA has an effect on hospital LOS and discharge to home vs. rehabilitation facilities. METHODS: An analysis was performed of consecutive primary TKAs performed at a single institution over one year. Patients were assigned to two groups: POD

Clinical predictors of protracted length of stay in Ontario Complex Continuing Care hospitals.

BACKGROUND: Post-acute care hospitals are often subject to patient flow pressures because of their intermediary position along the continuum of care between acute care hospitals and community care or residential long-term care settings. The purpose of this study was to identify patient attributes associated with a prolonged length of stay in Complex Continuing Care hospitals. METHODS: Using information collected using the interRAI Resident Assessment Instrument Minimum Data Set 2.0 (MDS 2.0), a sample of 91,113 episodes of care for patients admitted to Complex Continuing Care hospitals betwee

Predictors of prolonged length of stay after major elective head and neck surgery.

OBJECTIVE/HYPOTHESIS: Longer length of stay (LOS) after elective surgery is associated with an increased use of health care resources and higher costs. The objectives of this study were to determine the perioperative factors that predict a prolonged LOS after elective major head and neck operations and to test the hypothesis that factors related to process of care (intra- and postoperative) independently predict prolonged LOS after adjustment for preoperative patient characteristics. STUDY DESIGN: Prospective hospital-based cohort study. METHODS: The National VA Surgical Quality Improvement

Factors affecting length of hospital stay for femoropopliteal bypass. Implications of the DRGs.

We compared the length of hospitalization of 329 patients undergoing femoropopliteal bypass during 1982 with that allowed under Medicare's diagnosis-related groups (DRGs) for major reconstructive vascular surgery (DRGs 110 and 111). The mean length of stay was found to be related to the patients' age, increasing at a rate of about 1 percent per year. The site of distal anastomosis, a factor not considered in establishing the criteria for these DRGs, was also important. The lengths of stay for above-the-knee popliteal, below-the-knee popliteal, and tibioperoneal anastomoses were significantly d

Impact of amyloid cardiomyopathy diagnosis on heart failure readmission and length of stay.

BACKGROUND: Amyloid cardiomyopathy (CM) portends poor outcomes in heart failure (HF). Whether establishing a diagnosis of amyloid CM influences healthcare utilization remains unclear. METHODS: We identified patients admitted with a primary diagnosis of HF between January 2018 and December 2020, using medical record-linked administrative data. Amyloid CM was diagnosed using technetium-99m pyrophosphate scintigraphy and assessment for paraproteinemia, with endomyocardial biopsy as needed. Annualized HF readmission rates and length of stay (LOS) were compared between patients with confirmed amyl

Pattern Analysis of Comorbidity Undercoding and Its Association with Hospital Length of Stay.

This study evaluates diagnostic coding consistency across sequential hospitalizations using a longitudinal analysis of the CMS Inpatient Dataset with more than five million records. By transforming ICD-10-CM codes into CCS categories, the research tracks 9 chronic and 7 acute conditions using State Transition Matrices to identify patterns: Continuous, Fading, Intermittent Gap, and Late Onset. Significant diagnostic gaps were observed. Diabetes and Multiple Myeloma showed 'Intermittent Gap' rates of approximately 14%. Critically, for most chronic conditions, the 'Intermittent Gap' pattern corre

The Impact of Emergency Department Geriatric Nurse Practitioners on Patient Length of Stay.

INTRODUCTION: Limited research exists that examines the impact of geriatric-focused nurse practitioners practicing within United States emergency departments on the reduction of inpatient admission length of stay for older patients. This represents a significant gap in the provision of care. Our study aimed to assess the efficacy of geriatric-focused nurse practitioners' integration into an emergency department setting and the subsequent effect on the length of stay of inpatient hospital admissions. METHODS: We conducted a retrospective analysis of medical records for patients aged ≥65 years

Association of low-value operative management with mortality, length of stay and complications.

BACKGROUND: Significant inter-hospital variation in potentially low-value operative management of blunt solid organ injuries (SOI) has been observed but data on the impact on patient outcomes is lacking. Our primary objective was to estimate the association between potentially low-value operative management of blunt SOI and hospital mortality, complications, and length of stay (LOS). A secondary objective was to identify determinants, independent of patients' health status on arrival. METHODS: We conducted a retrospective cohort study using the National Trauma Data Bank (2016-2019). We includ
