Apraclonidine, a selective alpha(2)-agonist, was developed to lower intraocular pressure and minimize the systemic side effects associated with the use of its parent drug, clonidine.(1) An investigation of the site of action of apraclonidine incidentally uncovered a reversal of anisocoria in patients with absent sympathetic innervation of one pupil (Horner syndrome) due to its alpha(1)-effect on a pupil with denervation supersensitivity.(2) It has been used as a diagnostic test for Horner syndrome.(3,4) We report adverse effects of topical apraclonidine when used in the diagnosis of Horner syn

Three-week-old cesarean-derived colostrum-deprived (CD/CD) pigs were inoculated with porcine circovirus type 2 (PCV2, n = 19), porcine reproductive and respiratory syndrome virus (PRRSV, n = 13), concurrent PCV2 and PRRSV (PCV2/PRRSV, n = 17), or a sham inoculum (n = 12) to compare the independent and combined effects of these agents. Necropsies were performed at 7, 10, 14, 21, 35, and 49 days postinoculation (dpi) or when pigs became moribund. By 10 dpi, PCV2/PRRSV-inoculated pigs had severe dyspnea, lethargy, and occasional icterus; after 10 dpi, mortality in this group was 10/11 (91%), and

Citrullinemia type 1 (CTLN1) is a rare inherited disorder caused by mutations in the argininosuccinate synthetase 1 (ASS1) gene, disrupting the urea cycle and leading to toxic ammonia accumulation. It typically presents in newborns with lethargy, vomiting, and poor feeding. Stroke-like neurological presentations are very rare. We report a 19-month-old boy presenting with sudden right-sided weakness, lethargy, and difficulty standing. Laboratory tests revealed elevated ammonia and plasma citrulline, while brain imaging showed extensive cortical and subcortical abnormalities with cerebral edema

Geotrichum candidum was cultured from the tonsils of a free-ranging weaner pig that was presented with lethargy and inappetence. Histopathology indicated pyogranulomatous tonsillitis with intralesional fungal hyphae and arthrospores. Geotrichosis is a rare, opportunistic mycosis of immunocompromised hosts, usually human beings.

A 60-year-old man with cerebral palsy and epilepsy was admitted with acute lethargy and deterioration in coordination. He was noted to be hypothermic at 35°C on admission. Routine work-up revealed toxic levels of phenytoin. No cause of hypothermia could be identified but as his phenytoin levels normalised, his body temperature also improved. There are three other reported cases of phenytoin- induced hypothermia in the literature. Could this be a rare cause of hypothermia?

We present a case of Cronkhite-Canada syndrome, which is a rare disease classically characterised by hyperpigmentation, alopecia and onychodystrophy associated with clinical gastrointestinal symptoms and hamartomatous gastrointestinal polyps. Management primarily involves immunosuppression and nutritional support, which led to remission in this case. The recognition of cutaneous features may lead to early diagnosis and reduce morbidity and mortality.

Oxycodone and oxycodone-containing analgesics are often used for the relief of pain. In the presence of renal dysfunction, the half-life of oxycodone and metabolites can be prolonged. We describe the case of a 41-year-old chronic hemodialysis patient who received multiple doses of oxycodone/acetaminophen resulting in accumulation of the medication and consequent lethargy, hypotension and respiratory depression. These adverse effects were reversed with multiple bolus doses of naloxone, followed by a continuous infusion administered for 45 hours. Utilizing the Naranjo probability scale, the pati

BACKGROUND AND OBJECTIVES: Blood culture is the gold standard to diagnose bloodstream infection but is usually time-consuming. Prediction models aim to facilitate early preliminary diagnosis and treatment. We systematically reviewed prediction models for health care-associated bloodstream infection (HABSI) in neonates, identified superior models, and pooled clinical predictors.
DATA SOURCES: LibHub, PubMed, and Web of Science.
METHODS: The studies included designed prediction models for laboratory-confirmed HABSI or sepsis. The target population was a consecutive series of neonates with susp

BACKGROUND: Veterinary telehealth can provide veterinary care to various species, geographic locations, client demographics, and practice types. Factors influencing the veterinarian decision when teletriage and teleadvice are administered to a large number of dogs with gastrointestinal (GI) signs have not been previously reported.
AIM: To report on dogs evaluated via a veterinarian-led teletriage and teleadvice synchronous video service for GI signs to determine factors associated with referral recommendation for in-person emergency (ER) evaluation.
METHODS: The number of animals was 1,575 d

OBJECTIVES: Pediatric patients with cerebrospinal fluid shunts frequently present to the emergency department for evaluation of possible shunt malfunction. Most shunt studies appear in the neurosurgical literature. To our knowledge, none have reviewed presenting signs and symptoms of shunt malfunction in patients who present to the pediatric emergency department. The study objective was to evaluate the medical record of children with cerebrospinal fluid shunts who presented to a pediatric emergency department to determine if any signs and/or symptoms were predictive of shunt malfunction.

A patient presented with shortness of breath, lethargy and weight loss. A computerized tomography and echocardiogram showed a mass in the right ventricle nearly obstructing the pulmonary valve during systole and prolapsing into the main pulmonary artery. The mass was completely excised. Histology was that of a typical myxoma.

Neoplasia is rarely reported in European hedgehogs (Erinaceus europaeus). A retrospective search was conducted by contacting multiple veterinary diagnostic laboratories for cases of lymphoma in European hedgehogs. This resulted in 5 cases, from which clinical, gross, histologic, and immunophenotyping findings were recorded. Most animals (3/5) had skin masses involving the cervical region, 1 hedgehog had dyspnea and lethargy associated with hydrothorax, whereas another exhibited icterus and lethargy. The primary site of the lymphoma was the skin, particularly the neck or head (3/5), the thymus

INTRODUCTION: In clinical perspectives, how to distinguish a small proportion of children at risk of developing neurological complications from a large number of children with mild symptoms still remains a challenge for primary care doctors.
METHODOLOGY: From January 2012 to December 2015, 225 cases with severe hand, foot and mouth disease (HFMD) matched with 492 controls were enrolled in the age-matched, case-control study. Continuous variables were examined by univariate analysis using a chi-squared or Fisher's exact test, and categorical variables were reported by relative risks (odd's rat

Infantile acute lymphocytic leukaemia (ALL) seldom presents within the first month of life. Most are diagnosed before birth. Postnatal diagnoses are easily recognisable when characteristic features are present, namely hepatosplenomegaly, leukaemia cutis or infiltrative disease of the extramedullar and central nervous system. However, some children present with vague and non-specific symptoms masquerading as other diseases. We report an unusual presentation of infantile ALL in a 19-day-old infant, who struggled with feeding after a diagnosis of gastro-oesophageal reflux disease since birth. To
